    Notes: The AbrB protein of Bacillus subtilis regulates expression of numerous genes, primarily through specific binding interactions to DNA regions containing transcriptional promoters. Although over 15 target regions for AbrB binding to chromosomally located sequences have been analysed by DNase I footprinting, no obvious consensus sequence or motif has yet emerged from their examination. Using in vitro selection techniques, we have isolated optimal AbrB-binding sites from oligonucleotides containing 22 or 44 random base pairs. The best of these sites have an apparent in vitro Kd which is fivefold lower than a similar-sized DNA fragment containing the sequence corresponding to the AbrB-binding site on the spoOE gene. We tested one of the sites in vivo and found that it confers AbrB-mediated control upon a promoter not normally regulated by AbrB. In each of four separate trials, the selected sites possess motifs that converge to a simple consensus. It is argued that the nature and spacing of these motifs produce a type of three-dimensional DNA structure recognizable by AbrB, and that known in vivo sites, which lack these motifs, possess an approximation of the optimal structural determinant.

Argues that most academics do not care about the economics of publishing; they want only to get their material published in a format that will make an impact on their profession. Even so, economic factors enter into the equation more and more.

    Description / Table of Contents: Zusammenfassung Die Untersuchung eines Todesfalls nach oraler Aufnahme einer größeren Menge eines Kombinationsinsektizids bestehend aus Propoxur und Tetramethrin sowie den Lösungsmitteln Isopropanol und Benzylalkohol ergab als wesentlichen morphologischen Befund eine koagulationsnekrotische Verhärtung der Speiseröhre und der Magenwand. Die in Venenblut, Leber, Niere, Glaskörperflüssigkeit und Liquor durch HPLC bzw. GC/MS bestimmten Konzentrationen lagen für Propoxur (〈 0,3– 18 μg/g) im komatös-lateralen Bereich, während Tetramethrin (bis 0,1 μg/g) nahezu vollständig abgebaut war. Weiterhin wurden als Propoxurabbauprodukt o-Isopropoxyphenol (2,8–56 μg/g), Benzylalkohol (18–58 μg/g) sowie dessen Abbauprodukt Benzoesäure (85–140 μg/g), Isopropanol (0,45–1,1 mg/g) und Aceton (0,01–0,03 mg/g) gefunden. Die Werte im Herzblut und in der Lunge waren hingegen bis zu 30-fach höher. Als Ursache für diese immensen Konzentrationsunterschiede wird eine agonale Aspiration des erbrochenen, bis zu drei Größenordnungen höher konzentrierten Mageninhaltes diskutiert. Der hauptsächlich postmortal angenommene Abbau von Propoxur zu o-Isopropoxyphenol und von Benzylalkohol zu Benzoesäure ist in der Leber nahezu vollständing und in den übrigen Probenmaterialien sehr unterschiedlich ausgeprägt. Als Todesursache ergibt sich eine Intoxikation durch den Acetylcholinesterasehemmer Propoxur in Kombination mit der neurotoxischen Wirkung des Tetramethrins.
    Notes: Abstract In a fatality after oral intake of a large amount of an insecticide spray consisting of propoxur, tetramethrine and the solvents isopropanol and benzyl alcohol, a coagulation nectrotic fixation of the oesophagus and stomach were the main morphological findings. The concentrations determined by HPLC and GC/MS in femoral blood, liver, kidney, vitreous humor and liquor were in the comatose lethal range for propoxur (〈 0.3–18 μg/g), whereas tetrametrin (up to 0.1 μg/g) was almost completely decomposed. The propoxur decomposition product o-isopropoxyphenol (2.8–56 μg/g), benzyl alcohol (18–58 μg/g) and its oxydation product benzoic acid (85–140 μg/g), isopropanol (0.45–1.1 mg/g) and acetone (0.01–0.03 mg/g) were also found. On the other hand, the concentrations in heart blood and lung tissue were up to 30 times higher. As a reason for these immense concentration differences an agonale aspiration of the highly concentrated liquid stomach content after vomiting immediately before death is discussed. The extent of the metabolization of propoxur to o-isopropoxyphenol and of benzyl alcohol to benzoic acid, which is assumed to proceed mainly postmortem, was quite different in the materials investigated and was almost complete in the liver. The intoxication by the acetylcholine esterase inhibitor propoxur in combination with the neurotoxic effect of tetramethrine was determined as the cause of death.

    Description / Table of Contents: Abstract Purpose: Discussion in the literature of peripheral nerve lesions after surgical treatment is based mainly on orthopedic surgery or on ischemic syndromes. There is no information about the frequency of nerve lesions after revascularization of the lower limb. Methods: Between 1992 and 1996, 436 patients underwent peripheral arterial revascularization, including 147 cases of deep revascularization, 127 supra-genicular femoro-popliteal bypasses, 116 infra-genicular femoro-popliteal bypasses and 56 femoro-crural bypasses. Mean patient age was 70.5±5.8 years; 182 patients were female and 254 were male. There was a 38% rate (166/436) of reoperation. All patients were examined based on anamnestic information and a detailed clinical neurological investigation, including the methods of Weber and Moberg. Results: We found eleven patients (4%) with peripheral nerve lesions after the primary operation. After the reoperation 55 patients (33%) had nerve lesions. Conclusions: Peripheral nerve lesions with sensory disturbance were present in 15% of our patients on discharge from hospital. They had mainly dysesthesia and hypesthesia but no kind of motor dysfunction. In our opinion, detailed preoperative information is very important because of the risk of sensitive nerve lesions.
    Notes: Zusammenfassung Fragestellung: Periphere Nervenläsionen nach operativen Eingriffen beziehen sich in der Literatur überwiegend auf chirurgisch-orthopädische Eingriffe oder stehen ursächlich im Zusammenhang mit Ischämiesyndromen unterschiedlichster Genese. Bisher gibt es in der Literatur keine zuverlässige Aussage über die Häufigkeit des Auftretens von sensiblen Nervenläsionen nach arterieller Gefäßrevaskularisation an der unteren Extremität. Methodik: Wir untersuchten 436 Patienten nach peripherer Gefäßrevaskularisation in den Jahren 1992–1996 bezüglich postoperativer Nervenschäden, basierend auf anamnestischen Angaben und einer detaillierten klinisch neurologischen Befundung. Weiter erfolgten Untersuchungen der Zwei-Punkt-Unterscheidung nach Weber und der Ninhydrinprobe nach Moberg. Ergebnisse: Unter den 147 Profundarevaskularisationen, 127 Bypasses zum 1. Poplitealsegment, 13 Bypasses zum 2. Poplitealsegment, 93 Bypasses zum 3. Poplitealsegment und 56 femorokruralen Bypasses kam es nach der Primäroperation bei 4% (11/270) der Patienten zu peripheren Nervenschäden. Innerhalb der Gesamtzahl der Operationen waren 38% (166/436) Reoperationen. Nach Reoperation kam es bei 33% (55/166) der Patienten zu Nervenschädigungen. Schlußfolgerung: Periphere Nervenläsionen mit sensiblen Ausfällen finden sich in 15% (66/436) zum Entlassungszeitpunkt, wobei Patienten nach Reoperationen ein 8mal häufigeres Risiko haben als Patienten nach Primäroperation. Im Vordergrund stehen sensible Funktionsausfälle mit Parästhesien, die gerade bei erfolgreicher Revaskularisation vom Patienten als nachhaltig beeinträchtigend empfunden werden. Die detaillierte präoperative Aufklärung über das Risiko sensibler Nervenläsionen ist daher unverzichtbar.

    Notes: Abstract Lethal injuries of the thorax due to shots fired from blank cartridges calibre 8 mm are reported in three cases. The muzzle of the weapon was in contact with the left side of the breast (contact discharge) and injuries to bones were absent in all three cases. In two of the cases the pericardium was not involved but the anterior wall of the right heart ventricle was ruptured and death was due to cardiac tamponade. In the third case the pericardial sac and the left ventricle were both ruptured and the victim died due to rapid exsanguination. The cases demonstrate that the gas pressure from the exploding propellent of blank ammunition can be powerful enough to penetrate the thoracic wall.
