    Notes: Cervical glandular atypia (CGA) is now increasingly recognized in cone biopsy specimens removed for suspected cervical intra-epithelial neoplasia, although little is known of its natural history or pre-malignant potential. One hundred and eighty-three cone biopsy specimens received in our department during 1977 and 1978 from the City and Women’s hospitals, Nottingham were reviewed and assessed for the presence of squamous CIN and CGA. One hundred and sixty-seven cone biopsies contained CIN and 28 of these also contained CGA. Ten cone biopsies contained high-grade CGA, the remaining 17 containing low-grade CGA. One cone biopsy contained high-grade CGA only. Twenty-one of the 28 cases of CGA had been followed up by annual pelvic examination and cervical cytology. None of these cases have developed abnormal cervical cytology or invasive cervical carcinoma after 13 years follow-up, even though 13 cone biopsies contained CGA which appeared to be incompletely excised. We concluded that if CGA is pre-malignant, the latent period for its progression to invasive carcinoma is in excess of 13 years. CGA of either grade is not, in itself, an indication for hysterectomy, allowing a conservative approach to management in the first instance.

    Notes: Abstract: UC11 cells, derived from a human astrocytoma, have a high density of functional substance P receptors. Radioligand binding studies were conducted with the highly selective neurokinin-1 receptor ligand [3H][Sar9,Met(O2)11]-substance P. Kinetic binding experiments conducted at 4°C yielded an association rate constant k1 of 1.86 X 107M–1 min–1, a dissociation rate constant k–1 of 0.00478 min–1, and a calculated kinetic KD of 257 pM. Saturation binding experiments yielded average values of KD= 447 ± 103 pM, Bmax= 862 ± 93 fmol/mg of protein. This Bmax corresponds to more than 150,000 binding sites/cell. Competition binding experiments with unlabeled [Sar9,Met(O2)11]-substance P yielded average values of KD= 491 ± 48 pM and Bmax= 912 ± 67 fmol/mg of protein. In [3H]inositol-labeled cells, substance P induced a robust inositol phosphate formation. Inositol trisphosphate levels increased as much as 20-fold within ∼15 s of addition of substance P. This inositol trisphosphate formation was transient and had returned to baseline within the first 60–120 s. Inositol monophosphate formation, however, was linear for at least 2 h. Structure activity data on binding and inositol monophosphate formation confirmed the presence of a neurokinin-1 receptor subtype in these cells. Thus, the UC11 cell should be a useful model cell for delineating the physiological role of substance P receptors in astrocytes.

    Notes: Summary. Continuous fetal monitoring was achieved with a fetal scalp pulse oximetry sensor in 86 labours. The average recorded fetal oxygen saturation in early labour (cervical dilatation 〈 5 cm) was 68% (SD 13%). At the end of labour (cervical dilatation ≥ 9 cm) the recorded mean oxygen saturation was 58% (SD 17%). The largest range of readings during a single labour was 81%-11% but this drop was associated with cord compression. The average SD during 1 h of normal labour was 10%. A second group of 40 fetuses was monitored during induction of labour before and after elective amniotomy. Oxygen saturation did not appear to change after amniotomy (mean change −0.4%, SD 1.2%) and there was no difference between mean antenatal or early intrapartum readings. We excluded the amniochorionic membranes as a possible source of data corruption by measuring their in vitro absorption spectra and confirming that they do not preferentially absorb light of either 660 or 940 nm wavelength. Non-invasive pulse oximetry can be used to monitor the fetus before and during labour.

    Notes: Summary. Advances in oximetry have allowed the obstetrician to measure oxygen saturation in the fetus with non-invasive transcutaneous techniques. The influence of caput succedaneum formation on the oxygen saturation results obtained with a pulse oximeter was studied in 30 newborn infants. Caput was associated with a reduced oxygen saturation reading (mean reduction of 15%; P〈0.001). This effect is partly due to a true drop in local tissue oxygenation but is exacerbated by a systematic error intrinsic to the physics of spectrophotometry. If continuous intrapartum oximetry is ever to become a part of routine obstetric monitoring then probes that pass through the cervix beyond the caput of the presenting part will be required if erroneously low readings are to be avoided.

    Notes: In the Tokamak Fusion Test Reactor (TFTR) [Plasma Phys. Controlled Fusion 26, 11 (1984)], the highest neutron source strength Sn and D–D fusion power gain QDD are realized in the neutral-beam-fueled and heated "supershot'' regime that occurs after extensive wall conditioning to minimize recycling. For the best supershots, Sn increases approximately as P1.8b. The highest-Q shots are characterized by high Te (up to 12 keV), Ti (up to 34 keV), and stored energy (up to 4.7 MJ), highly peaked density profiles, broad Te profiles, and lower Zeff. Replacement of critical areas of the graphite limiter tiles with carbon-fiber composite tiles and improved alignment with the plasma have mitigated the "carbon bloom.'' Wall conditioning by lithium pellet injection prior to the beam pulse reduces carbon influx and particle recycling. Empirically, QDD increases with decreasing pre-injection carbon radiation, and increases strongly with density peakedness [ne(0)/〈ne〉] during the beam pulse. To date, the best fusion results are Sn=5×1016 n/sec, QDD=1.85×10−3, and neutron yield=4.0×1016 n/pulse, obtained at Ip=1.6–1.9 MA and beam energy Eb=95–103 keV, with nearly balanced co- and counter-injected beam power. Computer simulations of supershot plasmas show that typically 50%–60% of Sn arises from beam–target reactions, with the remainder divided between beam–beam and thermonuclear reactions, the thermonuclear fraction increasing with Pb. The simulations predict that QDT=0.3–0.4 would be obtained for the best present plasma conditions, if half the deuterium neutral beams were to be replaced by tritium beams. Somewhat higher values are calculated if D beams are injected into a predominantly tritium target plasma. The projected central beta of fusion alphas is 0.4%–0.6%, a level sufficient for the study of alpha-induced collective effects.

    Notes: The complete ion cyclotron range of frequency (ICRF) heating system for the Tokamak Fusion Test Reactor (TFTR) [Fusion Tech. 21, 1324 (1992)], consisting of four antennas and six generators designed to deliver 12.5 MW to the TFTR plasma, has now been installed. Recently a series of experiments has been conducted to explore the effect of ICRF heating on the performance of low recycling, supershot plasmas in minority and nonresonant electron heating regimes. The addition of up to 7.4 MW of ICRF power to full size (R∼2.6 m, a∼0.95 m), helium-3 minority, deuterium supershots heated with up to 30 MW of deuterium neutral-beam injection has resulted in a significant increase in core electron temperature (ΔTe=3–4 keV). Simulations of equivalent deuterium–tritium (D–T) supershots predict that such ICRF heating should result in an increase in βα(0)∼30%. Direct electron heating has been observed and has been found to be in agreement with theory. The ICRF heating has also been coupled to neutral-beam heated plasmas fueled by frozen deuterium pellets. In addition ICRF heated energetic ion tails have been used to simulate fusion alpha particles in high-recycling plasmas. Up to 11.4 MW of ICRF heating has been coupled into a hydrogen minority, high-recycling helium plasma and the first observation of the toroidal Alfvén eigenmode (TAE) instability driven by the energetic proton tail has been made in this regime.
