Zusammenfassung Schwere Knorpeldestruktionen im Bereich tragender Flächen des Kniegelenkes konnten in den letzten 3 Jahren bei 22 Patienten mit der Technik der autologen Knorpel-Knochen-Wechselplombe versorgt werden. Tragefähige Knorpel-Knochen-Zylinder, aus der dorsalen Region des Oberschenkelcondyls entnommen, sowie der entnommene destruierte Knorpel meist mit anhaftendem Knochenzylinder werden wechselseitig mit Fibrin eingeklebt. Arthroskopische und histologische Untersuchungen zeigten, daß Knorpel mit der Knochenbasis verpflanzt vital einheilt, im Gegensatz zu Rückklebungen abgescherter Knorpel ohne Knochenbasis. Insgesamt werden die Resultate wie folgt beurteilt: 6 Fälle sehr gut, 12 gut, 2 befriedigend, sowie 1 Fall nicht befriedigend.

Summary In the last 3 years severe cartilage defects of the knee joint were treated with the so-called transposed autologous cartillage bone grafts. Grafts with vital cartilage were taken proximal from the nonload area of the femur condyl and transposed with a similar graft taken from the cartilage defect. Fixation was performed with the fibrin adhesion technique. Arthroscopic and microhistologic investigations revealed that healing of cartilage with subchondralbone was excellent in contrast to fixation of cartilage without any subchondralbone. The results were characterized as follows: 6 cases excellent, 12 good, 2 satisfactory, and 1 not satisfactory.
