    Description / Table of Contents: Zusammenfassung Es wurden die Resultate der Mikropräparationen von frisch amputierten Fingern mit Kontrakturen 4. Grades zeichnerisch dargestellt. Zwei Präparate bestätigen bisher veröffentlichte Theorien, während am dritten Präparat ein kontrahierendes System vorlag, das einen weiteren, bisher noch nicht bedachten Entstehungsmodus der Kontraktur 4. Grades aufzeigt. Die Resultate von drei rekonstruktiv behandelten Fällen wurden gezeigt.
    Notes: Summary The results of micropreparations of freshly amputated fingers with 4th-grade contracture are presented in drawings. Two preparations confirm the theories published hitherto, while the 3rd preparation displays a different system with a different, up to now unconsidered, mode of development of the 4th-grade contracture. The results obtained in three cases subjected to reconstructive treatment are presented.

    Description / Table of Contents: ZusammenfaBung Die Exstirpation benigner oder maligner Tumoren des Gesichtes erfolgt unter Berücksichtigung des Charakters, der Lokalisation und der eventuellen Metastasierung. Zur Deckung der entstandenen Defekte wird defektnahes Gewebe in Form von Rotations-, Verschiebe-, Transpositions-und Insellappen bevorzugt verwendet. Die sofortige Wiederherstellung wird stets angestrebt. Ist dies nicht zu erreichen, so werden defektferne Lappen mit mehrzeitigem Vorgehen benötigt. Die Methoden zur Wiederherstellung werden unter Berücksichtigung der anatomisch-funktionellen Gegebenheiten gewählt.
    Notes: Summary The character, localization, and eventual metastases should be concidered before removal of benign or malignant tumors of the face. To cover the resulting defect, tIssue near the defect in the form of rotational, sliding, transpositional, or island flaps is preferred. Immediate reconstruction is always preferable. However, when this is not poBible, several operations involving flaps taken from distant sites are neceBary. Restoration methods are chosen on the basis of the anatomic-functional requirements.

    Notes: Abstract Envelope preparations of chemotrophically and phototrophically grown Rhodospirillum tenue were isolated and characterized on the basis of their contents and composition in phospholipids and fatty acids, as well as on the basis of their enzyme activities, absorption spectra and polypeptide patterns. Both preparations were similar in their contents in phospholipids and fatty acids. Their total fatty acids were characterized by a rather high percentage of saturated fatty acids. The activities of the respiratory reactions were considerably higher in chemotrophic than in phototrophic membranes. This is true especially for activities of the terminal oxidase which were over 20 times greater. The affinities of the corresponding enzymes to their respective substrates (K m ) differed with differences in the culture conditions. Under chemotrophic conditions the K m values for the NADH-dependent reactions were lower than those values under phototrophic conditions, whereas the K m values for the succinate dependent reactions were higher. The low temperature (77°K) near infrared spectrum of the phototrophic membrane showed a peak at 875 nm which was not detectable in the chemotrophic membrane. The polypeptide patterns, in the presence of sodium dodecyl sulfate, of both preparations were quite similar except for the presence of two low molecular weight proteins (M. Wt. 12,000 and 10,000) in the phototrophic membrane which were absent in the chemotrophic membrane. Both envelope preparations were further fractionated into enriched cytoplasmic membrane and outer membrane fractions which were identified on the basis of their contents in 2-keto-3-deoxyoctonate, ubiquinone 8 and bacteriochlorophyll (in the case of the phototrophic membrane) and their enzyme activities. The buoyant densities of the corresponding fractions from both envelope preparations were found to be equal. The data presented in this paper demonstrate that envelope preparations of chemotrophically and phototrophically grown R. tenue are similar in their contents in 2-keto-3-deoxyoctonate, ubiquinone 8, phospholipids and fatty acids, yet differ significantly in their spectra, protein patterns and enzyme activities.

    Notes: Summary The effects of the dihydropyridine derivative Bay K 8644 upon insulin secretion by perifused isolated mouse pancreatic islets were examined. At a non-stimulatory glucose concentration (5 mmol/l) Bay K 8644 (1 μmol/l) did not stimulate insulin release. However, the same drug concentration enhanced the insulin secretory responses to an intermediate (15 mmol/l) or high (30 mmol/l) glucose concentration by 80 or 90%, respectively. Bay K 8644 was half maximally effective at 0.1 μmol/l and maximally effective at 1 μmol/l. The results are compatible with the view that voltage-dependent calcium channels are essential for stimulus-secretion coupling in pancreatic B-cells.

Dupuytren ist eine chronisch progrediente Fibromatose der Palmaraponeurose und der Digitalfascien unbekannter Genese mit einer Morbidität von 1–3%. Der beste Zeitpunkt für die Operation ist gegeben, wenn die Summe der Streckdefizite an einem Finger 20–30° nicht überschreitet. Die Methode der Wahl ist die komplette Entfernung der Palmaraponeurose und der erkrankten Digitalfascien. Bei fortgeschrittenen Kontrakturen an den Fingern sind evtl. Z-Plastiken, freie Hauttransplantationen and Eingriffe an Bändern and Gelenkkapseln erforderlich. Ein Modell der Dokumentation zur Qualitätssicherung wird vorgestellt.
    Notes: Summary Dupuytren's contracture is a chronic and progressive fibromatosis of the palmar aponeurosis and digital fascia. Morbidity is about 1%-3% and the cause is not yet known. The best time for prophylactic surgery is when the degree of deformity has reached a finger flexion of 20°–30°. Complete fasciectomy and excision of the involved digital fascia is the best method. If there is severe extension of contracture, Z-plasties, full-thickness grafts, or operation at the articular capsules or ligaments may be necessary. A documentation model and control of the surgical success are demonstrated.

Kritik hinsichtlich des Ergebnisses veranlaßte uns neben der Technik nach Strömbeck zunehmend das Verfahren von Skoog anzuwenden. 110 Patientinnen wurden nach einem Protokoll, das objektiv-meßbare Parameter und subjektive Faktoren berücksichtigt, vergleichend nachuntersucht. Ferner wurde der Krebsvorsorgeuntersuchung nach Mammareduktionsplastik besondere Beachtung geschenkt.
    Notes: Summary Long-term results in reduction mammaplasty can be evaluated at the earliest after one year. Criticism about the result induced us to apply more often the technique of Skoog beside the procedure of Strömbeck. A follow-up study of 110 patients could be done. Objective parameters were considered as well as subjective factors. Special attention was paid to diagnosis of cancer after mammaplasty.

    Description / Table of Contents: Zusammenfassung 21 Patienten, 3–63 Jahre alt, mit traumatisch bedingten Haut-Subcutandefekten am Fuß wurden 1–13, im Durchschnitt 6 Jahre nach der plastisch-chirurgischen Rekonstruktion nachuntersucht. Die unterschiedlich auf die planta pedis, dorsum pedis und regiones calcanea und malleolares verteilten Defekte wurden mit defektnahen Haut-Subcutan- oder Muskellappen, defektfernen cutanen und myocutanen Lappen vom anderen Bein, Rundstiellappen und freier Haut gedeckt. Wir fanden, daß eine Belastung der rekonstruierten Flächen an der Ferse und Fußsohle von Erwachsenen vermieden wurde und Hyperkeratosen entsprechend der Fehlbelastung außerhalb der Lappen lagen. Die im Alter von 3, 3 1/2 und 6 Jahren operierten Kinder belasteten dagegen die wiederhergestellten HautSubcutandefekte voll.
    Notes: Summary Posttraumatic skin and soft tissue defects of the sole dorsum of the foot, heel, or ankle in 21 patients from 3–63 years of age were covered with local skin flaps, local myocutaneous flaps, cutaneous and myocutaneous cross-leg flaps, or split-skin grafts. The average follow-up was for 6 years. The long-term results were quite different, depending on the age of the patient. While the adults walked on the areas surrounding the flaps, with resultant hyperkeratosis or ulcers of the normal nonreconstructed skin, the children (aged 3, 3.5 and 6 years) walked on the whole sole, including the flaps, and hyperkeratosis developed on the flaps, but no ulcers.
