    Notes: Summary Plasma cholesterol has been shown to be correlated to the incidence of cardiovascular events. This observation and experimental data on the pathogenesis of atherosclerosis have led to a number of recommendations for the treatment of elevated plasma lipids by national and international consensus panels and organizations. However, the issue whether lowering cholesterol might increase the risk for other diseases including cancer is still controversial. In this article, an attempt is made to review the currently available data on the relation between plasma cholesterol and cancer. Special emphasis is given to the results of intervention trials aimed at lowering cholesterol by diet and/or drugs, because they apply best to the human situation and are particularly relevant for the clinician. Data from animal experiments are only briefly discussed.

    Notes: Summary Ungated cardio-CT provides good morphologic information in the case of congenital heart disease, volume or pressure loading, left ventricular aneurysm, tumours, thrombi, cardiac calcifications and pericardial diseases. It allows qualitative assessments of myocardial thickness and perfusion of coronary bypass grafts. ECG-gated cardio-CT allows a quantitative assessment of systolic and diastolic myocardial thickness, volume calculation of cardiac cavities and evaluation of functional parameters. The correlation of the calculated left ventricular ejection fraction comparing laevocardiography and ECG-gated cardio-CT was poor (r=0.7) mainly due to the comparable poor time resolution on the side of cardio-CT (approximately 0.1 s). The quantitative analysis of regional wall motion based on cardio-CT showed for the diagnosis of pathologic wall motion a specificity of 80.5%, sensitivity of 73.9% and accuracy of 75.7%. Side effects must be taken into account following the injection of 200–250 ml hyperosmolar contrast medium during 5–10 minutes which causes intravascular fluid retention and cardiac volume loading. Patients prone to cardiac decompensation should be excluded from cardio-CT examinations.

    Description / Table of Contents: Zusammenfassung Bei 72 Patienten mit ÖV wurde endoskopisch der intravasale ÖVDruck bestimmt, bei 31 von ihnen zusätzlich auch der Pfortaderdruck. Nur im Bereich des oesophagokardialen Übergangs stimmen Pfortaderdruck und ÖV-Druck überein. Je größer die ÖV sind, desto höher ist der intravasale ÖV-Druck. Ebenfalls steigt die Blutungsgefahr mit der Größe der ÖV. Eine prophylaktische Sklerosierung sollte bei zweit- und drittgradigen ÖV erfolgen.
    Notes: Summary In 72 patients with oesophageal varices the intravascular oesophageal variceal pressure was measured, in 31 of them the portal pressure as well. The portal pressure was identical with the variceal pressure only at the cardia. There was a positive relationship between the size of varices and the variceal pressure and between the size of varices and the occurrence of bleeding from varices. Prophylactic endoscopic sclerotherapy is indicated in grade II and III varices.

    Notes: Summary In six anesthetized dogs with myocardium partially damaged by ischemia (LAD occlusion), the effect of an i.v. bolus injection of 0.05 mg molsidomine/kg body weight followed by a 6-h i.v. infusion of 0.5 μg molsidomine/kg/min on the volume of myocardial ischemia, the relative remaining blood flow in the ischemic area, and the dynamics of the left ventricle were examined by means of computer tomography. The extent of the ischemic volume in the group treated with molsidomine was by far lower than in the control group; this difference was significant if one takes into account the individual heart size. The relative remaining blood flow in the ischemic region was not influenced by molsidomine. The reduction of preload and afterload resulted in corresponding changes in left ventricular areas, segments of these areas, the long axis, thickness of myocardium, ejection fraction and stroke volume. Aortic pressure was lowered insignificantly, heart rate remained nearly unchanged. Plasma analyses of molsidomine, SIN 1 and SIN 1C show that the applied dosage was sufficient to reach a constant concentration over the whole period of observation in the dog.
