    Notes: Summary Plasma cholesterol has been shown to be correlated to the incidence of cardiovascular events. This observation and experimental data on the pathogenesis of atherosclerosis have led to a number of recommendations for the treatment of elevated plasma lipids by national and international consensus panels and organizations. However, the issue whether lowering cholesterol might increase the risk for other diseases including cancer is still controversial. In this article, an attempt is made to review the currently available data on the relation between plasma cholesterol and cancer. Special emphasis is given to the results of intervention trials aimed at lowering cholesterol by diet and/or drugs, because they apply best to the human situation and are particularly relevant for the clinician. Data from animal experiments are only briefly discussed.

    Notes: Abstract Iodixanol (visipaque, 320 mgI/ml) was compared with iopamidol (Solutrast, 370 mfI/ml) in a double-blind, randomized, parallel group, intravenous DSA phase-III trial for evaluation of safety and efficacy. A total of 117 patients received iodixanol (n = 60) or iopamidol (n = 57). Diagnostic efficacy was evaluated using categoric and visual analogue scales. Discomfort and adverse evenets were recorded. A total of 39 patients collected urine up to 72 h after the examination for analysis. Diagnostic efficacy and radiographic density were similar in both groups. Discomfort was milder with iodixanol. The difference between the frequency of adverse events between both groups (iodixanol = 7, iopamidol = 2) was without statistical significance. Creatinine clearance was slightly more affected by iodixanol, whereas the increase in renal excretion ofN-acetyl-beta-glucosaminidase (NAG) in the first 24 h collection period after the examination was significantly higher (p 〈 0.01) with iopamidol. Iodixanol was of equal diagnostic efficacy compared with iopamidol despite its reduced iodine content. Both contrast media are well suited for IV DSA.

    Notes: Abstract. This is the first report of palliative percutaneous treatment of a malignant duodenal stenosis due to cancer of the pancreatic head with Palmaz stents. A 65-year-old male with a malignant tumour of the pancreatic head developed an abscess with fistular communication to the cutis. In the subsequent course of the disease, tumour growth led to a severe duodenal stenosis. To dilate the tumorous stenosis, three Palmaz stents were introduced coaxially into the duodenum percutaneously, via the preexisting fistula. A technique to pass an almost 90 ° kink is described. Symptomatic malignant duodenal stenosis was treated by insertion of three Palmaz stents. Due to their accurately controlled passive expansion at the level of the stenosis, and the resulting good adaptation to the individual anatomical situation, they were suitable for application in the duodenum.

    Notes: Abstract. This review presents the options and limitations of MRI in non-vascular diseases of the mediastinum and the chest wall. In numerous thoracic pathologies, MRI is a useful supplement to spiral CT. This imaging procedure also allows a contrast-media-free differentiation of solid tumors and vascular lesions (e. g., aortic aneurysms). The advantages of MRI over CT are particularly useful when multiplanar tumor imaging is required prior to surgery to establish the exact spatial relationship between tumor and the other mediastinal structures. Primary indications for MRI in diseases of the mediastinum and chest wall are therefore: (a) tumors of the posterior mediastinum for determining their position in relation to the neural foramina and the spinal canal; (b) chest wall tumors; (c) preoperative multiplanar imaging of primary mediastinal tumors; and (d) contraindications against CT exams with iodine contrast media.

    Notes: Abstract. The aim of this study was to evaluate somatosensory evoked potentials (SEP) as a simple neurophysiological monitoring method in supra-aortal balloon test occlusions (BTO). The 13 patients examined had carcinoma of the larynx (n = 6), nasopharyngeal malignancy (n = 3), sphenoid meningioma (n = 2), mycotic aneurysm (n = 1), and a tumor of the carotid body (n = 1). Transfemoral balloon occlusion of the internal carotid artery was maintained for 30 min and monitored both clinically and by SEP. None of the 13 patients showed neurological deficits or pathological changes in the SEP. No thromboembolic complications occurred during BTO. In 11 cases the cervical vessels could be spared or reconstructed during surgery. No neurological symptoms occurred during the clinical follow-up. One patient died the day after BTO due to cardiac arrest. Neurophysiological monitoring of preoperative BTO using SEP is a technically simple method. A similar method has been shown to have a high sensitivity and reliability in carotid surgery.

    Notes: Summary Patients with ventricular drainage may develop lesions of the corpus callosum. In order to study frequency, pathogenesis and clinical relevance of callosal lesions, 301 routine CT investigations of 79 patients with ventricular drainage were reviewed. Hypodense lesions in the anterior part of the corpus callosum were observed in 7 patients with longstanding hydrocephalus of variable origin, in 2 cases the corpus callosum was slightly swollen. All 7 patients showed signs of forced ventricular drainage. The callosal alterations were clinically inapparent and potentially reversible. Knowledge of these lesions is important to avoid misinterpretation.

    Description / Table of Contents: Résumé A partir de l'analyse rétrospective de portographies directes effectuées par angiographie percutanée chez 43 patients, différents critères anatomiques et radiologiques ont été précisés. Ces portographies ont été faites dans un but thérapeutique (embolisation des vaisseaux alimentant des varices œsophagiennes); la localisation et le trajet de toutes les veines hépatofuges ont été étudiés. A côté de la circulation collatérale porto-cave connue, il existe un certain nombre d'autres voies de dérivation actuellement incomplètement décrites et qui peuvent être précisées par angiographie.
    Notes: Summary In a retrospective study angiographic image material of percutaneous direct portographies carried out in 43 patients was evaluated according to anatomic and radiologic criteria. These examinations were performed for therapeutic purposes (embolisation of vessels supplying varices). All the hepatofugally perfused veins were analyzed according to their localisation and course. Apart from the known portocaval collateral pathways a number of other collaterals not yet described could be documented by means of angiography.
