    Description / Table of Contents: Zusammenfassung Bei der Anastomose zwischen Ösophagus und Dünndarm kommt der Gefahr einer Nahtinsuffizienz nach wie vor eine prognostisch maßgebliche Bedeutung zu. Um dieser Komplikation vorzubeugen, haben wir eine Methode entwickelt, bei der wir eine terminolaterale Ösophagojejunostomie mit einem überstehenden Darmsegment einmanschettieren. Wir haben unser Vorgehen bisher bei fünf Patienten störungsfrei, d. h. ohne Insuffizienz und auch ohne Stenose, erproben können. Wir interponieren ein langes oberes Jejunalsegment orthograd und isoperistaltisch zwischen Ösophagus und Duodenum. Dieses Vorgehen kommt unseres Erachtens der Erhaltung der operativ faßbaren Partial-funktionen des normalen Magens unter allen anderen vergleichbaren Methoden am nächsten.
    Notes: Summary To overcome the specific problems of esophagojejunostomy, leakage and reflux, a method of end-to-side anastomosis completely surrounded by the proximal jejunal stump was developed. A proximal jejunal loop is isolated and used for isoperistaltic interposition between esophagus and duodenum. About 15 cm of the jejunal segment are left orally to the proximal anastomosis which is performed end-to-side using a single layer of interrupted sutures. The blind loop is then wrapped completely and symmetrically around the anastomosis and sutured to itself and the esophagus to keep it in place. Complete envelopment of the leakage-prone anastomosis is thus achieved. The distal end of the jejunal segment is anastomosed to the duodenum end-to-end. The technique in addition provides protection against reflux into the esophagus by gaseous distension of the proximal jejunal segment surrounding the anastomosis without impeding the passage due to a valve-like action which allows unidirectional flow. Clinical results so far are promising and show absence of leakage and/or stenosis in all cases of esophagojejunostomy operated according to the method described.

    Notes: A simple differential, isothermal calorimeter has been built to study the thermodynamics of interactions associated with a variety of polymer solution processes. The calorimeter is readily operated at temperatures ranging from ambient to about 200°C., temperature adjustments are rapid, and the apparatus is rugged enough to permit application to commercial process studies. Though less sensitive than microcalorimeters, it represents an attractive combination of satisfactory accuracy, speed, and flexibility of operation. The operation of the calorimeter is demonstrated by measurements of the heat of solution of sodium chloride in water and the heats of solution of various polyolefins in Tetralin and α-chloronaphthalene. The latter tend to confirm the presence of polymer aggregates in chloronaphthalene solutions below the thermodynamic melting temperature of the polymer.
