    Description / Table of Contents: Zusammenfassung Im Experiment setzten wir bei 185 Tieren osteochondrale Transplantate ein. Wir verwendeten autogene, konservierte und frische allogene Transplantate. Auch nach 2 Jahren zeigten die ultrastrukturellen Untersuchungen eindeutig, daß die Chondrocyten der autogenen und frischen allogen Transplantate überleben. Die konservierten Transplantate werden resorbiert. Die klinischen Ergebnisse von 30 Patienten stimmen damit überein. Nach autogener und frischer allogener Transplantation sind Funktion und Röntgenbild des Kniegelenks normal. Technik und spezielle Instrumente der Transplantation wurden gezeigt.
    Notes: Summary In our experiments, osteochondral grafts were implanted into the knee joints of 185 animals. We transplanted autologous, and stored and fresh homologous grafts. Even after 2 years the ultrastructural investigations clearly indicate that the chondrocytes of the autologous and fresh homologous grafts survive. The stored grafts are absorbed. Clinical results from 30 patients agree with the experimental findings. The function and the X-ray photographs of the knee joints are normal after autologous and fresh homologous transplants. The technique of transplantation and some special instruments are demonstrated in detail.

    Description / Table of Contents: Zusammenfassung In unserer tierexperimentellen Arbeit untersuchten wir licht- und elektronenmikroskopisch die Struktur des Gelenkknorpels nach homologer Transplantation. In den rechten medialen Femurcondylus vom Kaninchen wurden Knochen-Knorpel-Defekte gesezt, die mit homologen, in flüssigem Stickstoff konservierten Knochen-Knorpel-Transplantaten ausgefüllt wurden. 2, 4 und 6 Monate nach Transplantation wurden die Tiere getötet. Während funktionell und röntgenologisch keine Unterschiede zum unbehandelten Kniegelenk festzustellen sind, sieht man im Licht- und Elektronenmikroskop zahlreiche strukturelle Veränderungen im Vergleich zum normalen Gelenkknorpel. So überwiegen in der ersten Phase des Einheilungsprozesses regressive Vorgänge. In der zweiten Phase werden Bindegewebszellen gebildet, die sich metaplastisch in Knorpelzellen umwandeln. Dieses neuentstandene Gewebe ist morphologisch hyalinem Knorpelgewebe zuzuordnen.
    Notes: Summary Our experimental work was concerned with the structure of articular cartilage as seen in the LM and EM after homotransplantation. In the right medial condylus of the femur of the rabbit bone-cartilage defects were produced and stuffed with bone-cartilage homografts which had been preserved by fluid nitrogen. 2, 4 and 6 months after the operations the animals were sacrificed. Although there were no functional and radiological differences between the treated knee joints and the controls, LM- and EM-studies revealed numerous structural alterations in the grafts compared to normal articular cartilage. A first phase of the healing process was marked by regressive reactions. In a second phase connective tissue cells appeared which underwent a metaplastic transformation to cartilage cells. This new tissue showed the morphological criteria of hyaline cartilage.

    Description / Table of Contents: Zusammenfassung Bei 52 durchschnittlich nach 21 Monaten nachuntersuchten Patienten überwiegen die Knieknorpelschäden. Die primäre Gelenkknorpelverletzungen kommt als Kontusion, Impression oder osteochondrale Fraktur vor. Daraus können Chondromalacie, Chondropathia patellae und traumatische Osteochondrosis dissecans entstehen. Die Latenzzeit der Beschwerden lag zwischen 1 und 2 Jahren. Experimentell wurden Heilung und Transplantation verfolgt. Operatives Vorgehen waren Abtragung, Herdausräumung, Spongiosa- oder Knorpeltransplantation, Ventralisation. Ergebnisse und Fälle werden beschrieben.
    Notes: Summary In our follow-up study on 52 patients after approximately 21 months, the lesions most frequently observed were lesions of articular cartilage of the knee. Primary injury to articular cartilage takes the form of contusion, impression or osteochondral fracture, from which chondromalacia patellae and traumatic osteochondrosis dissecans can develop. Latency of the symptoms was 1 to 2 years. We observed transplantation and the healing process under experimental conditions. The surgical procedure consisted in shaving down of the cartilage, excision of the osteochondral lesion, transplantation of cancellous bone or articular cartilage grafts, and ventral transposition of the tibial tubercle. Results are discussed.
