    Notes: Abstract  Purpose: The purpose is to present results of endovascular exclusion (stent-graft treatment) of aneurysms of the descending thoracic aorta both in elective cases and in emergencies. Methods: Indications for stent-graft treatment were dependent on multislice angio-CT evaluation revealing a proximal neck of at least 10 mm between the left common carotid artery and the onset of aneurysm. All stent grafts were inserted in the operating room; 43 transfemoral, 2 transiliac. The stent grafts used were Corvita, Stenford, Vanguard, AneuRx, Talent, and Excluder. Deployment was achieved under fluoroscopic control, endoleaks were checked for with D S A on the operating table and postoperatively by angio-CT. Long-term follow-up consisted of evaluation with angio-CT after 6 and 12 months, and from there on once a year and with plain chest X-rays. Follow-up was achieved in all patients. Results: Mean follow-up is 21 months (1–66); 30-day mortality is 3/45, no permanent neurologic deficit. Thirty patients were treated electively, 15 with contained rupture. Left subclavian artery overstenting proved to be necessary in 12 patients for proper proximal sealing of the aneurysm, type I endoleaks were observed in 10 patients, one early conversion, 7 proximal extension cuffs, one sealed spontaneously, one still at risk. Among patients where LSA had been overstented only one wanted a transposition, all others did well without left-hand ischemia or subclavian steal syndrome. Conclusion: Endovascular treatment is less invasive and has reasonable mortality and morbidity but is limited to well-defined morphologies. Mid-term results are promising but it has to be observed whether these will translate into long-term durability. (J Card Surg 2003; 18:367-374)

    Description / Table of Contents: Zusammenfassung Von 1969-1979 wurden 129 Patienten wegen akuter Iliofemoral-Venenthrombose operiert (Mortalität 0,6%). Bei 28% fanden sich phlebographisch nach 18 Monaten alle Venen durchgängig bei suffizienter Klappenfunktion und normaler Venendruckkurve bei Belastung. 64 % ließen teilweise Rethrombo-sierung entweder im Unter- oder Oberschenkelbereich erkennen, die Venendruckkurve zeigte jedoch normale Abflußverhältnisse. Nur bei 8% waren Oberschenkel- und Beckenvene rethrombosiert bei schwerster venöser Insuffizienz und Zeichen eines beginnenden postthrombotischen Syndroms.
    Notes: Summary From 1969-1979 surgical thrombectomy was performed in 129 patients; mortality rate was 0.6 %, intraoperative pulmonary embolism occurred in 1.6%. Late postoperative follow-up (18 months postop.) revealed fully patent iliofemoral and lower leg veins in 28% with normal venous pressure response curve during exercise. In 64% partial rethrombosis of either femoral or lower leg veins had occurred; nevertheless venous pressure curve was grossly normal suggesting normal venous drainage capacity. Only 8 % revealed iliofemoral reocclusion and beginning postthrombotic syndrome.

    Description / Table of Contents: Abstract Background: The implantation of a femoro-popliteal bypass graft represents the gold standard in the management of arteriosclerotic occlusive disease in the femoro-popliteal region. Application of interventional techniques has permitted the development of a novel, endovascular implantable bypass system. The device is placed within the lumen of the superficial femoral artery. The present study investigated the technical properties of the new system in cadavers, with subsequent evaluation of its application in humans. Methods: The endopass consists of a thin-wall polycarbourethane tube with a total length of 53 cm. The distal end is connected with a self-expanding stent prothesis 4 cm in length. Endovascular implantation is performed through a groin incision. Following antegrade ringstripper-TEA, the endopass is introduced into the femoral artery after placement of a guide wire. The proximal anastomosis is performed surgically. Results: The technical requirements and feasibility of the system were tested on 12 cadaver extremities. Implantation of the device proved technically successful in seven of eight extremities with significant occlusion of the superficial femoral artery. The most important problems included the complete ringstripper-TEA, the placement of the guide wire following the TEA, and the expansion of the endopass. Following approval by the ethics commission, the endopass was implanted in five patients between June 1997 and March 1998. In three patients, the endopass remained patent and has functioned well. One patient with poor vascular outflow suffered an early noncorrectable occlusion, while in the remaining case, perforation secondary to the ringstripper procedure forced us to abandon the attempt. Conclusion: Experience from the cadaver study and initial human applications confirm the feasibility of this method. Further development and methodological improvement, however, are necessary requiring additional, larger studies on patients.
    Notes: Zusammenfassung Fragestellung: Die Implantation eines femoropoplitealen Bypasses stellt heute den Goldstandard zur Behandlung der AVK bei Verschlußprozessen im femoropoplitealen Bereich dar. Durch Verwendung interventioneller Techniken konnte ein endovasculär implantierbares Bypassystem entwickelt werden. Im Rahmen einer Leichenstudie wurde die technische Funktionsfähigkeit des neuen Systems erprobt und danach die Evaluation in der humanen Anwendung begonnen. Methodik: Der Endopass besteht aus einer 53 cm langen, dünnwandigen Rohrprothese aus Polycarbourethan. Das distale Ende ist auf eine Länge von 4 cm mit einem selbstexpandierenden Stent armiert. Die Implantation erfolgt über eine Leisteninzision. Nach antegrader Ringsonden-TEA wird der Endopass über einem Führungsdraht in der A. femoralis superficialis plaziert. Die proximale Anastomose erfolgt chirurgisch. Ergebnisse: Die technische Durchführbarkeit und Funktionsfähigkeit wurde an 12 Leichenextremitäten erprobt. Bei 8 Extremitäten mit Verschlüssen der A. femoralis superficialis konnte das Verfahren in 7 Fällen technisch durchgeführt werden. Entscheidende Probleme sind die komplette Ringsonden-TEA, die Plazierung des Führungsdrahts nach TEA und die Entfaltung des Endopasses aus dem Introducer. Nach Änderung des Equipments und Freigabe der Anwendung durch die Ethikkommission wurde der Endopass von 6/97 bis 3/98 bisher bei 5 Patienten angewandt. Bei 3 Patienten ist der Endopass offen und zeigt eine gute Funktion. Ein Patient erlitt einen nicht korrigierbaren Frühverschluß bei schlechter Ausstrombahn und bei einem Patienten mußte das Verfahren bei Ringsondenperforation gewechselt werden. Schlußfolgerungen: Die Erfahrungen der Leichenstudie und der ersten humanen Anwendungen bestätigen die praktische Durchführbarkeit der Methode. Es sind jedoch weitere Entwicklungsarbeit und methodische Verbesserungen notwendig, die nur durch weitere Erfahrungen am Patienten gewonnen werden können.

    Description / Table of Contents: Abstract  Intraoperative complications can be divided into three categories on the basis of the time of their occurrence: Group I – Problems associated with access; group II: introduction of the endovascular stent (a: release of the main segment, b: anchoring the second limb); and group III: incidence of primary endoleaks. The objective of the present study was to describe the management of these complications. In a group of 130 patients undergoing endovascular treatment of aneurysms of the abdominal aorta, a total of 31 complications occurred in 26 patients (20.0%). The most frequently encountered complication in each group and its respective management was as follows: Group I: Correction was performed using dilatation and retroperitoneal stretching as well as surgical shortening of the external iliac artery with interposition; group IIa: overstenting the renal arteries was corrected either by conversion or tugging at the endograft; group IIb: the guide wire or docking system could not be placed. Such cases were managed either through conversion or use of fresh systems; group III: such complications were treated with repeated stent placement, postoperative coil embolizations, or conversion. The endovascular therapy of aortic aneurysms is associated with intraoperative complications in 20% of cases. Adequate management helps to keep both morbidity and mortality rates low.
    Notes: Zusammenfassung  Intraoperative Komplikationen können je nach Zeitpunkt des Ereignisses in 3 Gruppen definiert werden: Gruppe I – Zugangsprobleme, Gruppe II – Einbringen des endovaskulären Stents (a: Absetzen des Hauptteils, b: Andocken des 2. Schenkels), Gruppe III – Inzidenz primärer Endoleaks. Zielsetzung dieser retrospektiven Studie war es, das Management zur Behebung dieser Komplikationen darzustellen. Von 130 Patienten mit endovaskulärer Ausschaltung eines Aortenaneurysmas traten bei 26 Patienten 31 intraoperative Komplikationen auf (20,0%). Die führenden Komplikationen in jeder Gruppe sowie das daraus resultierende Management waren: Gruppe I: Die Korrektur erfolgte durch Dilatation, retroperitoneale Streckung, Kürzungsoperation der A. iliaca externa und Interponat. Gruppe II a: Bei Überstentung der Nierenarterien erfolgte die Konversion oder Zug am Endograft. Gruppe II b: Führungsdraht oder Andocksystem können nicht platziert werden. Die Behebung erfolgte durch Konversion und Anwendung neuer Systeme. Gruppe III: Hier wurden erneute Stentplazierungen, postoperativer Coil-Embolisationen und Konversionen durchgeführt. Die endovaskuläre Therapie aortaler Aneurysmen birgt eine Rate intraoperativer Schwierigkeiten von 20%. In 3,8% der Fälle musste eine Konversion durchgeführt werden. Durch adäquates Management konnten die Letalität (1/26) und Morbidität jedoch gering gehalten werden.

    Notes: Summary In a series of 133 dogs, 10–20 ml/kg arterial blood were removed and replaced by rapid intravenous infusion of an equivalent volume of various plasma substitutes. Following the infusion of gelatine solutions, an acute fall in arterial blood pressure was observed, but not following the infusion of equal quantities of Dextran 60 or of 0.9% NaCl solution. Measurement of the blood histamine levels as well as the fact that the circulatory effects could be modified by the previous administration of anti-histaminic drugs indicate that the hypotension observed following the rapid infusion of gelatine solutions is due to the release of histamine. Since histamine liberation has also been shown in preliminary studies in man, rapid infusions of gelatine solutions should be avoided in clinical practice, or patients receiving them should be pre-treated with anti-histaminic drugs.

    Notes: Zusammenfassung Native S35-markierte Hefe wurde an 34 weibliche Albinoratten verfüttert und die Aktivitätswerte des 10fach unterteilten Magen-Darmtraktes wie auch des Gesamtblutes und der Leber bestimmt. 1. Der S35-Gehalt der Intestinalwand innerhalb der ersten 10 Std stellt im wesentlichen ein Maß der lokalen Eiweißresorption dar. 2. Die Magen-Darmabschnitte erreichen über 2 bis 7 Std in der Reihenfolge Magen-Dünndarm, Colon und Rectum ihre Maximalwerte, es folgen Gesamtblut und Leber. 3. Der ganze Magen-Darmtrakt ist zur Resorption befähigt. Die überwiegende Menge des Eiweißes wird unter normalen Umständen im Dünndarm resorbiert, die Resorptionsleistung von Magen und Rectum ist unter diesen Bedingungen unbedeutend. 4. Bezogen auf ein gleiches Organtrockengewicht resorbiert das Duodenum am stärksten, die Resorptionsleistung fällt gleichmäßig zum Magen und Rectum hin ab. 5. Die angetroffene S35-Konzentration der Darmwand läßt einen intensiven Eiweißstoffwechsel in der Darmschleimhaut annehmen.
