    Description / Table of Contents: Zusammenfassung Im Rahmen einer AO-Sammelarbeit wurden aus 6 Kliniken Deutschlands insgesamt 135 Korrektureingriffe am oberen Sprunggelenk der Jahre 1962–1974 ausgewertet. Es wird über Indikationsstellung, Komplikationen und Behandlungsergebnisse berichtet. Die besten Ergebnisse wurden each Korrektureingriffen bei ursprünglichen Luxationsfrakturen vom Typ A und B erreicht. Hier beschränkten sich die Eingriffe vorwiegend auf die Stabilisierung einer Außen- oder Innenknöchelpseudarthrose. Bei Luxationsfrakturen vom Typ C überwiegen die Verängerungsosteotomien der Fibula — hier konnten in 43 % der Fälle gute Spätergebnisse erreicht wurden.
    Notes: Summary A total of 135 corrective procedures on the ankle joint performed in 6 German hospitals from 1962 to 1974 is reviewed. Indications, results of treatment, and complications are reported. Best results were achieved with corrective procedures following dislocation fractures of Type A or B. In most cases of this category, non-union of the lateral or medial ankle required stabilization. For old dislocation fractures of Type C, elongation osteotomy of the fibula was performed most frequently. Good longterm results were obtained in 43 % of cases.

    Description / Table of Contents: ZusammenfaBung Ca. 1 % aller Knochenbrüche sind Kniescheibenbrüche. Man unterscheidet entsprechend der Lokalisation der Fraktur verschiedene Typen von Brüchen. Hinsichtlich der Behandlung sollte bei einfachen Quer- und Schrägbrüchen die retropatellare Gleitfläche anatomisch exakt reponiert und anschlieBend eine funktionBtabile Osteosynthese mit einer Zuggurtung ausgeführt werden. Bei Polabbrüchen mit gröBerer Zertrümmerung ergibt die Teilentfernung der Kniescheibe mit entsprechend exakter Naht des Streckapparates gute funktionelle ErgebniBe. Im Falle einer vollsändigen Zertrümmerung der gesamten Kniescheibe, bei welcher eine exakte Rekonstruktion der Kniescheibengelenkfläche nicht möglich ist, entschlieBt man sich zur Frühpatellektomie. Die Frühpatellektomie ergibt beBere ErgebniBe als die Spätpatellektomie.
    Notes: Summary Fractures of the patella account for about 1 % of all fractures. They result from the impact of direct or indirect forces. Various fracture types are distinguished. Treatment of simple transverse or oblique fractures consists of anatomic reduction, with special attention to perfect reconstruction of the retropatellar gliding surface. Tension banding is used for fixation to achieve functional stability. Comminuted fractures in the pole region may require partial resection along with careful reconstruction of the extensor mechanism. In cases of complete communition of the patella, early patellectomy is recommended and gives better results than late patellectomy.

    Description / Table of Contents: Zusammenfassung Die vorwiegend konservative Behandlung kindlicher Frakturen hat die noch offenen Wachstumsfragen, die jeweils dem Alter entsprechend schnellere Knochenbruchheilung und eine mehr oder weniger starke Stimulierung des Längenwachstums zu berücksichtigen. Verbleibende Fragmentverschiebungen werden später um so vollständiger ausgeglichen, je jünger das verletzte Kind ist. Darstellung des konservativen Vorgehens bei Schaft-, metaphysären und Gelenkfrakturen. Sekundärschäden sind in der Regel nicht zu erwarten, daher erübrigen sich gezielte krankengymnastisch-physikalische Maßnahmen.
    Notes: Summary The preliminary conservative management of juvenile fractures by closed reduction must be performed with due regard for the pressure of epiphyseal plates, the more rapid bone healing and the increased stimulation of growth. Malunited fractures are realigned by the normal process of bone remodeling, and results are better in young children. The management of shaft, metaphyseal and joint fractures by closed reduction is outlined. Late sequelae such as a diminished range of motion of the affected limb are rare, so that intensive physical therapy is not required.

    Description / Table of Contents: Zusammenfassung Neben der technischen Erforschung sog. Biomaterialien wurden in den letzten Jahren vor allem Belastungsuntersuchungen mit dem Ziel einer Optimierung von Form, Werkstoff und Implantationstechnik durchgeführt. Für die konventionellen Prothesentypen ist die Verwendung von Knochenzement zur Verankerung nach wie vor erforderlich. Solange keine Langzeitergebnisse mit zementloser Implantation vorliegen, wird unter strenger Beachtung der Indikationsstellung auch weiterhin die Zementverankerung der Prothesenanteile notwendig sein. Vor- und Nachteile der sog. konventionellen Endoprothese sowie Verbesserungen von Form und Implantationstechnik werden dargestellt und diskutiert.
    Notes: Summary Advantages and disadvantages of the so-called conventional endoprostheses as well as improvements in respect of design and technic of implantation are described and discussed. As long as we have no long-term results with the cementless endoprosthesis types, it will be necessary to continue with the use of cemented endoprostheses under strict indications.

    Description / Table of Contents: ZusammenfaBung Bei 1043 im konventionellen Operationsraum durchgeführten arthroplastischen Eingriffen waren die Operationswunden in 41 % bakteriell kontaminiert, trotzdem kam es nur in 0,94% der Fälle zu einer Infektion. Durch Einrichtung einer Laminar-Flow-Operationskabine konme der Prozentsatz positiver Wundabstriche auf 18%, die Infektionsrate auf 0,2% gesenkt werden. Bei 100 Patienten wurde versucht, Dutch biochemische Typisierung nachgewiesener Staphylokokken deren Infektionsweg aufzuklären. In 4 von 6 Fällen konnten auf der Haut and in der Operationswunde Staph. epi. des gleichen Biotyps nachgewiesen werden.
    Notes: Summary Of 1043 alloarthroplasties performed in regular operating rooms without laminar flow, surgical wounds were contaminated with bacteria in 41 % of the cases. Woundinfection developed in only 0.95 %. After installation of a vertical laminar flow system, wound swabs showed bacterial growth in 18%, the rate of wound infection fell to 0.2%. In 100 patients we attempted to uncover the path of infection by biochemical typing of staphylococci that had been isolated from the wounds. In 4 out of 6 cases the type of Staphylococcus epidermidis isolated from the skin was identical to the type isolated from the wound.
