Zusammenfassung Bei 107 Patienten mit verschiedenen neurologischen Erkrankungen wurden einschließlich von Verlaufskontrollen 156 Untersuchungen des Säure-Basen-Status und der Atemgase (pH,pCO2, HCO3 −,pO2) sowie 79 Bestimmungen des Lactats und Pyruvats in Blut und Liquor cerebrosponalis durchgeführt. Unser Material wurde in 3 Gruppen aufgeschlüsselt und zwar in 1. Untersuchungen von Normalfällen, 2. von Kranken mit freiem Sensorium und 3. von bewußtseinsgetrübten Patienten. Eine dekompensierte metabolische (Lactat-)Acidose im Liquor cerebrospinalis war bei den bewußtseinsgetrübten Patienten hierbei am deutlichsten. Bei 30 Verlaufskontrollen von 19 Patienten mit Virus-Meningoencephalitis korrelierten verschiedene Stufen der Bewußtseinslage signifikant mit den Lactat-, pH- und HCO3 −-Werten des Liquor cerebrospinalis. Die kritischen Werte der gefundenen Stoffwechselgrößen im Liquor cerebrospinalis werden angeführt, da sie eine prognostische Aussage ermöglichen und als wichtige Indicatoren der vitalen Bedrohung gelten müssen.

Summary A total of 156 tests of acid-base balance (pH) and respiratory gases and a total of 79 estimations of lactate and pyruvate in blood and CSF were performed in 107 patients with a variety of neurological diseases. Our subjects fell into 3 groups; 1. normal subjects, 2. sick patients with normal perception, 3. patients with impaired consiousness. Uncompensated metabolic (lactate) acidosis was found to be most pronounced in the patients in group 3. Checks in 19 patients with viral meningoencephalitis revealed a significant correlation between the level of consciousness and the pH and the lactate and HCO3 −-levels in the CSF. The values we found to be critical for the metabolic parameters are cited as they allow an assessment of the prognosis and must be recognized as important indicators of a potentially fatal condition.
