Résumé A l'aide d'une caméra à scintillations reliée à un double convertisseur analogique digital, à 1600 ou 4096 mémoires et à un système digital magnétique, on a étudié chez des malades souffrant d'ictus cérébraux la valeur de trois techniques isotopiques, à savoir la scintiphotographie statique, séquentielle, la détermination du débit sanguin d'une région cérébrale. Dans la plupart des cas la scintiphotographie séquentielle montrait l'existence de gros vaisseaux cervicaux et intracraniens et une chute du débit sanguin de l'hémisphère correspondant. Un paramètre obtenu par l'analyse temporelle de l'activité, se trouvait en bonne corrélation avec les valeurs de débit hémisphérique, mais ces données n'autorisaient q'une estimation vague de la circulation cérébrale. La détermination du débit sanguin régional est plus efficace pour reconnaître des lésions focales que la scintiphotographie statique, en particulier lorsqu'il s'agit de déficit neurologique transitoire. La comparaison statistique de valeur de débit dans des cas de foyer vascularisé montrait une nette différence selon qu'il y a ou non fixation du radioélément. Ce paramètre paraît important pour apprécier la conservation des structures anatomique. L'application combinée des trois méthodes permet l'étude des rapports hémodynamiques entre les troncs artériels et le niveau tissulaire. L'obstruction des gros vaisseaux est reconnue dans la majorité des cas, le débit régional est quantifié et les lésions tissulaires sont démontrées.

Zusammenfassung Mittels Szintillationskamera, Dual Analog digital-Converter, 1600 oder 4096 Wortspeicher und digitalem Magnetbandsystem werden 3 Isotopenmethoden — statische Szintigraphie, Sequenzszintigraphie und Messung der regionalen Hirndurchblutung — in ihrer Aussagekraft untersucht. Durch die Sequenzszintigraphie konnten die meisten Strömungshindernisse großer Hals- und Gehirngefäße entdeckt werden; entsprechend der Schwere der Gefäßerkrankung war die Hemisphären-durchblutung bei diesen Patienten vermindert. Ein Parameter aus der Analyse der Zeit-Aktivitätskurven der Sequenzszintigraphie korrelierte signifikant zur Hemisphärendurchblutung, der Wert erlaubt aber nur eine unsichere Abschätzung der cerebralen Zirkulation, — In der regionalen Hirndurchblutungsmessung konnten fokale vasculäre Läsionen viel öfter entdeckt werden, als dies mit der statischen Szintigraphie möglich war; das war besonders der Fall bei Patienten mit kurzdauernden und leichten neurologischen Ausfällen. Ein statistischer Vergleich der regionalen Hirndurchblutungswerte der Fälle mit und ohne vermehrte Isotopenanreicherung zeigte einen signifikanten Unterschied. Dies ergibt einen wichtigen Hinweis für die Größe der Durchblutung, die für die Aufrechterhaltung der anatomischen Strukturen nötig ist. — Die gemeinsame Anwendung aller drei Methoden ermöglicht es, Veränderungen an großen Gefäßen zu erkennen, die Durchblutung in umschriebenen Hirnarealen quantitativ zu erfassen und ausgedehnte Gewebsläsionen aufzuzeigen.

Summary Using a scintillation camera connected to a dual analog to digital converter, a 1,600 or 4,096 word memory and a digital magnetic tape system, the value of 3 different isotope techniques — static and sequential scintiphotography and measurement of regional cerebral blood flow — was investigated in patients suffering from cerebrovascular attacks. In most cases sequential scintiphotography revealed abnormalities in large neck or intraeranial vessels, and hemispheric rCBF was decreased according to the vascular disease. One parameter obtained by analyzing the time activity histograms from the sequential scintigraphy correlated significantly with the hemispheric CBF values, but these data permitted only a vague estimation of cerebral circulation. — rCBF measurement disclosed focal vascular lesions more often than static scintiphotography, especially in cases with short-lasting and slight neurological deficits. A statistical comparison of the rCBF values in pathologically perfused foci in cases showing scintigrams with or without increased isotope uptake revealed a significant difference. This result may be interpreted as an important parameter for the blood supply necessary for the survival of anatomical structures. — The combined application of the three methods permits the assessment of cerebral hemodynamics on the level of the large arteries and at tissue level: obstructions of large vessels are detected in most cases, the flow in circumscript areas is quantified and tissue lesions are shown.
