    Notes: In this study the effects of immunomodulators on the ecto-5′-nucleotidase (ecto-5′-NT) activity on blood mononuclear cells (BMC) were examined in vitro. Interleukin-4 (IL-4) and interferon-γ (IFN-γ) decreased the level of ecto-5′-NT activity on BMC whereas prostaglandin E2 (PGE2) increased the ecto-5′-NT level. All three immunomodulators influenced the ecto-5′-NT activity of isolated monocytes whereas only IL-4 and PGE2 had an effect on the enzyme level on isolated lymphocytes. The effect was dependent upon protein synthesis. The effect was dose dependent: IL-4 was effective at concentrations down to 0.5 U/ml, IFN-γ down to 40 U/ml and PGE2 at nanomolar concentrations. These data indicate that immunomodulators may also take part in the regulation of ecto-5′-NT activity on BMC in vivo.BMC from 7 patients with different immunodeficiency syndromes showed decreased ecto-5-NT activity on freshly isolated cells. However, following culture ecto-5′-NT activity was increased above the level found on freshly isolated BMC from healthy persons. On BMC from 3 patients with hypogammaglobulinaemia, the effect of IL-4 on the level of ecto-5′-NT activity was identical to that found on BMC from healthy donors, whereas PGE2 increased ecto-5′-NT activity on BMC from only 1 of the 3 patients investigated. The decreased ecto-5′-NT activity of BMC from patients with immunodeficiency may thus be due to a defective regulation of ecto-5′-NT activity

    Notes: The binding of monoclonal antibodies against CD4 was specifically inhibited by treatment of human CD4+ cells with either alkaline protease (AP) or elastase (Ela), purified from Pseudomonas aeruginosa. Binding of antibodies against CD3 (pan T), CD5 (pan T), CD8 (T suppressor/cytotoxic). HLA-ABC, HLA-DR, HLA-DQ, HLA-DP/DR, and β2 microglobulin was not inhibited by AP or Ela. Heat-inactivation of the proteases at 65°C for 20 min or treatment with the metal chelator EDTA abolished the inhibitory activity of both proteases. These findings may serve to develop novel immunological methods for the isolation and study of the lymphocyte CD4 structure, which plays an important part in the immune response.

    Notes: A girl with severe combined immunodeficiency and pronounced malnutrition from chronic diarrhoea is presented. Immunological reconstitution was attempted by transplantation of bone marrow cells from the HL-A hemiallogeneic father. An initial transplant failed to induce a permanent take of the graft, whereas a second transplant with an increased cell dose ensured a take, which was followed by reconstitution of cell-mediated immune functions. Fractionation of the transplanted bone marrow cells apparently led to a delay in development of graft-versus-host symptoms. Germ-free isolation and extensive bacterial decontamination markedly reduced the microbial flora and was highly protective against contaminating microorganisms but failed to eradicate completely one strain of Escherichia coli that had invaded the child before institution of this regimen. During a moderate, delayed graft-versus-host reaction this strain caused widespread severe infection, to which the child succumbed 10 weeks after the second transplantation. This child presented some additional features, the most conspicuous being a deficiency of erythrocyte adenosine deaminase.

    Notes: The present study was designed to examine the effect of physical exercise on human natural killer (NK) cells. Six healthy volunteers underwent two different acute physical exercise tests with an interval of at least 1 week: (1) 60min bicycle exercise at 80% of maximal oxygen uptake (VO2max) and (2) 60 min back-muscle training at up to 29% of VO2max; blood samples were collected before and during the last few minutes of exercise, as well as 2 h and 24 h afterwards. The NK cell activity (lysis/fixed number of mononuclear cells) increased during bicycle exercise, dropped to a minimum 2 h later and returned to pre-exercise levels within 24 h. Back-muscle exercise did not significantly influence NK cell activity. Plasma levels of adrenaline, noradrenaline, and cortisol were elevated during bicycling, but not during back-muscle exercise, indicating that exercise intensity is a determinant of NK cell activity. During bicycle exercise the NK cell subset (CD 16+ cells) of mononuclear cells increased significantly. Furthermore an improved interleukin 2 (IL-2) boosting of the NK cell activity was found during work as compared to IFN-α and indomethacin-enhanced NK cell activity. These results indicate that NK cells with a high IL-2 response capacity are recruited to the peripheral blood during exercise. The decreased NK cell activity demonstrated 2 h after work was probably not due to fluctuations in size of the NK cell pool, since the proportion of CD16+ cells was normal. The finding that indomethacin fully restored the suppressed NK cell activity in vitro and the demonstration of a twofold increase in monocyte (CD20+ cells) proportions 2 h after work, strongly indicate that prostaglandins released by monocytes during the heavy physical exercise are responsible for the down-regulation of the NK cells.

    Notes: The human B-lymphocyte response to proiein-conjugated polysaccharide antigens has not previously been studied at the cellular level. In order to do so, we developed and evaluated haemolytic plaque-forming cell assays detecting Haemophilus influenzae type b (Hib) capsular polysaccharide-specific antibody-secreting cells (AbSC) of the isotypes IgM, IgG, and IgA. The appearance of AbSC in the blood after vaccimation of adults with diphtheria toxoid-conjugated Hib polysaccharide was investigated. AbSC were detected from post-vaccination day 5 to day 14.IgA was the predominant isotype among these cells. IgM AbSC peaked slightly earlier (median day 7) than IgG and IgA AbSC (both day 8). On post-vaccination day 8 ihe numbers of AbSC were: IgA. 1217/106 mononuclear cells (median); IgG, 211; and IgM, 30 (n= 11). Similar isotype distribution has earlier been found after vaccination with pure capsular polysaccharides from Hib and pneumococci. The predominance of IgA AbSC in response to both conjugate and pure polysaccharide vaccines is probably due to reactivation of the same clones of IgA-committed memory B cells originally primed at the mucosa by natural exposure to the polysaccharide or cross-reacting antigens.

    Notes: CD73 is a bifunctional glycosyl phosphatidylinositol anchored leucocyte differentiation antigen which has specific ecto-5′-nucleotidase (ecto-5′-NT) activity and is an accessory T-lymphocyte activation molecule. The aim of the present study was to investigate the CD73 expression on blood mononuclear cells (BMC) from a group of patients with primary immunoglobulin deficiency (IGD). This group of patients had both significantly decreased levels of ecto-5′-NT on BMC (P = 0.002) and decreased numbers of CD73 molecules per CD73+ lymphocyte (P = 0.01). Five of the 10 patients had a decreased percentage of CD73+ lymphocytes. Among B-lymphocytes the patients had normal percentages of CD73+ cells but four of the 10 patients had numbers of CD73 molecules per CD73+ B-lymphocyte below the normal range. Among CD4-lymphocytes three out of 10 patients had percentages of CD73+ below the normal range and four out of 10 patients had decreased percentages of CD73+ CD8-lymphocytes. Significant correlations were found between in vitro proliferative responses to mitogens and the number of CD73 molecules per CD73+ lymphocyte (rs = 0.60, P 〈 0.01) and per CD73+ CD8-lymphocyte (rs = 0.64, P 〈 0.02). In addition, a positive correlation was found between ability to proliferate and level of ecto-5′-NT on BMC (rs = 0.53, P 〈 0.05). Furthermore the ability of BMC to synthesize ecto-5′-NT was studied. During 2 days' culture ecto-5′-NT activity increased markedly on BMC from both patients and healthy donors. The level of activity on BMC from all patients attained levels higher than on freshly isolated BMC from healthy donors. This shows that the decreased levels of ecto-5′-NT found on freshly isolated BMC from patients with IGD is due to defective regulation of the enzyme activity in vivo

    Notes: The early phase of antigen-dependent triggering of rabbit blood lymphocytes (PBL) is described. Specific activation of PBL by Streptococcal vaccines requires primed lymphocytes from high responder rabbits. B-cell memory is conferred by Ig receptors of both μ and γ chains. Antigen-induced incorporation of leucine and thymidine reach peak values al day 5, with stimulation indices of 10 to 〉 100, closely followed by increases in the number of living cells. A second and less intensive phase of cell proliferation is seen on days 8–11. A large fraction of cells (between 3 and 33%) participates in proliferation. A rough calculation of the average doubling lime of cells during the exponential growth phase gives values of 8–16 h.

    Notes: The recent findings that bicycle exercise training may reduce the number of swollen joints in patients with rheumatoid arthritis (RA) stimulated us to examine the possibility that this was mediated through exercise-induced immunomodulation. The effect of a single bout of physical exercise on blood mononuclear subsets, proliferative responses and natural killer (NK) cell activity was determined. Six patients with moderately active RA for 20 years exercised for 27 min on a bicycle, the work intensity being estimated at 68% of max V̇O2max. Blood samples were collected before and during the last minutes of exercise, as well as 2 h afterwards. During bicycle exercise the proportion of T cells (DC3+ cells) declined, mainly because of a fall in T helper cells (DC4+ cells). The proportion of NK cells (CD16+ cells) increased during work, but reverted afterwards. The monocytes (CD14+ cells) did not change; B cells (CD20+ cells) declined slightly during exercise and reverted later. No change in PHA-, PPD- and Unstimulated BMNC proliferation occurred during exercise. Two hours after exercise, PPD- and IL-2-induced proliferation increased significantly, except for the PHA-stimulated response. The NK cell activity increased only significantly during exercise when the cells were preincubated with indomethacin, and returned to normal 2 h afterwards. This shows that brief, moderate exercise by patients with RA alters the composition of blood mononuclear cell subsets and cell functions. The clinical significance of this immunomodulation remains to be elucidated.
