Summary The authors describe firstly the topographical anatomical basis of cranial retrograde phlebography, and the radiological anatomy in a normal phlebogram is explained. The technical and methodological problems are briefly mentioned, and a new procedure (percutaneous catheterization of the vena frontalis) for the demonstration of the orbital veins and the cavernous sinus is mentioned. The chief differences from the normal in pathological processes are listed and examples of the corresponding phlebograms are illustrated. Cases previously published in the world literature are summarized and are elucidated from the pathological anatomical point of view. Retrograde phlebography is indicated: a) in unilateral exophthalmos and bilateral exophthalmos which does not have an endocrine basis, b) in every example of unilateral papilloedema or of optic atrophy after optic neuritis has been excluded and c) when there are clinical, or even only radiological, suspicions of a pituitary lesion.

Zusammenfassung Es wird zunächst über die topographisch-anatomischen Voraussetzungen der kranialen retrograden Phlebographie berichtet und die Röntgenanatomie an einem normalen Phlebogramm erläutert. Die technischen und methodischen Probleme werden kurz erwähnt und ein neues Verfahren (perkutane Katheterisierung der V. frontalis) zur Darstellung der Orbitalen Venen und des Sinus cavernosus genannt. Die wesentlichen Abweichungen vom Normalbild bei pathologischen Prozessen werden hervorgehoben und an entsprechenden Phlebogrammen exemplarisch demonstriert. Die bisher in der Weltliteratur mitgeteilten Fälle wurden zusammengestellt und unter pathologisch-anatomischen Gesichtspunkten aufgeschlüsselt. Die retrograde Phlebographie ist indiziert: a) beim einseitigen Exophthalmus und beim beidseitigen, nicht endokrin bedingten Exophthalmus, b) bei jeder einseitigen Stauungspapille und Optikusatrophie nach Ausschluß einer Neuritis und c) beim klinischen oder auch nur röntgenologischen Verdacht auf einen sellären Prozeß.
