    Description / Table of Contents: Zusammenfassung Man kann Mutationen in (numerische und strukturelle) Chromosomenaberrationen, sowie in Punktmutationen unterteilen. Beim Menschen treten sie häufig ohne erkennbare Ursachen auf. (Spontane Mutationen). Viele Arzneimittel und andere Stoffe unserer Umwelt erhöhen die spontane Mutations-rate bei einem oder mehreren der in der experimentellen Forschung verwendeten niederen Organismen. Um zu entscheiden, ob sie auch für den Menschen als gefährlich angesehen werden müssen, benötigt man Untersuchungen an Säugetier-Testsystemen. Diejenigen Testsysteme, die für in vivo-Untersuchungen an der Maus verfügbar sind, werden beschrieben und am Beispiel des Triaziquon (Trenimon) werden mutagene Wirkungen in verschiedenen Entwicklungsstadien der Keimzellen diskutiert. Man findet nicht nur quantitative, sondern auch qualitative Unterschiede in der mutagenen Wirkung zwischen verschiedenen chemischen Mutagenen wie zwischen chemischen Verbindungen und ionisierenden Strahlen.
    Notes: Abstract Mutations can be subdivided into numerical and structural chromosome aberrations and point mutations. In human beings, they occur frequently without any discernible external reason (spontaneous mutations). Many drugs and other environmental compounds are known to enhance the spontaneous mutation rate in lower organisms. In order to decide whether they must be considered dangerous for humans, too, investigations in mammalian test systems are required. The in-vivo test systems available in the mouse are described and, using Triaziquone (Trenimon) as an example, mutagenic effects at different germ cell stages are discussed. There are not only quantitative, but also qualitative differences in the mutagenic action between different chemical mutagens as well as between chemical compounds and ionizing radiation.

    Description / Table of Contents: Zusammenfassung Anhand eines Falles von Taubenzüchterkrankheit mit Lungenfibrose und Cor pulmonale konnte gezeigt werden, wie wichtig die frühzeitige Erkennung des spezifischen Antigens bei der exogen allergischen Alveolitis ist. Auch längere Zeit nach Beendigung der Antigenexposition können im Patientenserum Antikörper gegen Taubenserum und Taubenkot nachgewiesen werden. Es ist anzunehmen, daß als auslösendes Agens das Serumalbumin und ein Enzym der Taube im Kot, möglicherweise Subtilisin, eine Rolle spielen, so daß verschiedene antigene Determinanten, vielleicht sogar verschiedene Antigene bei der Taubenzüchterkrankheit pathogenetisch von Bedeutung sind.
    Notes: Abstract A case of pigeon breeder's lung with fibroid lung and cor pulmonale is described to illustrate the importance of early discovery of the specific antigen of the exogenous allergic alveolitis. It is possible to prove antibodies to pigeon serum and pigeon excreta in the serum of patients even quite a long time after they have ceased to be exposed to the antigen. It must be supposed that the serum albumin and an enzyme of the pigeon in the excreta, possibly subtilisin, are involved, so that several antigenous determinants and perhaps even several antigens are important in the pathogenesis of pigeon breeder's lung.

    Description / Table of Contents: Zusammenfassung Zahlreiche Taubenzüchter wurden in Vorträgen und schriftlichen Beiträgen auf das Krankheitsbild der Taubenzüchterlunge aufmerksam gemacht. Daraufhin gaben 42 Züchter die typischen Symptome der Taubenzüchterlunge an. Bei 24 Patienten konnte die Diagnose durch klinische, labortechnische, radiologische, lungenfunktionsanalytische Untersuchungen und Inhalations-Provokations-Teste gesichert werden. Bei allein in der Bundesrepublik über 100 000 Taubenzüchtern und einer darüberhinaus noch großen Anzahl von allergen-exponierten Personen ist das Krankheitsbild offensichtlich keine Rarität. Deshalb und im Hinblick auf die ernste Prognose kommt der Diagnostik der Taubenzüchterlunge eine nicht zu unterschätzende Bedeutung zu. Bereits weit fortgeschrittene Lungenfibrosen mit Cor pulmonale infolge über Jahre aktiver allergischer Alveolitis auf Taubenantigene konnten erkannt werden. Mit der Aufdeckung des Krankheitsbildes konnte aber bei noch mehr Patienten, die zu einer Expositionsprophylaxe und anderen Therapiemaßnahmen bereit waren, die Lungenfibrose verhütet bzw. deren Fortschreiten gebremst werden.
    Notes: Abstract Pigeon breeders have been informed about pigeon breeder's disease in meetings and by communications in Germany. As a result 42 breeders presented the typical symptoms of the disease. In 24 patients diagnosis could be evaluated by clinical examination, laboratory tests, pulmonary function tests, x-ray examination, and by inhalation of antigens. As there are more than 100,000 pigeon breeders in West Germany and a large number of people are exposed to the corresponding allergens, pigeon breeder's disease has probably a higher incidence of occurrence than has yet been established. Cases of advanced lung fibrosis with cor pulmonale had been detected, mediated by allergic alveolitis versus pigeon antigens. But in more patients, who consented to avoid further exposure, fibrosis of the lung could be prevented or progress of the disease could be stopped.

    Notes: Summary Purpose A two-arm randomized clinical trial was performed to determine the efficacy of PCNU and AZQ in the treatment ofde novo or recurrent primary brain tumors. An additional objective was to gather information on the administration and toxicity of these compounds, supplementing that obtained previously in phase I/II studies. Methods During 1982 and 1983 the Brain Tumor Study Group randomized 152 adult patients with primary brain tumors to receive PCNU 75–100 mg/m2 intravenously (IV) every 8 weeks or AZQ 15 mg/m2 IV once a week for 4 weeks, every 6–8 weeks. All patients who had not received ‘full dose’ radiotherapy before randomization received it concurrently with the first course of protocol chemotherapy. The data were analyzed for the total randomized population (RP), and for 130 patients in the valid study group (VSG) formed by excluding 22 patients for whom the histologic diagnosis was not documented by central review. Results Median survival times were 11.0 months for the PCNU group and 8.4 months for the AZQ group. The difference in survival curves was statistically significant for the RP (p=0.01) and the VSG (p=0.02). Lifetable analysis of the VSG showed estimated 2-year survivals of 34% for PCNU and 11% for AZQ. The advantage of PCNU remained significant (p=0.006) after adjustment for histopathologic category, age, initial performance status, and interval from initial reported surgery. Myelosuppression was the principal toxicity in both groups.
