    Notes: Abstract The raw extracts of a series of microorganisms were screened for the presence of acetyl-coenzyme A: arylamine N-acetyltransferase (AAAT) using a radioactive assay with 3H-acetyl-coenzyme A and aniline as substrates. Enzyme activities were primarily detected in the soluble fractions of Bacillus and Nocardia species, and in some further soil organisms. Only strains of Bacillus cereus were able to acetylate 4-nitroaniline and 3,5-dimethyl-4-nitroaniline. The fermentation conditions for the production of the enzyme were optimized. The AAAT from one strain of Bacillus cereus was purified 24-fold and characterized.

    Description / Table of Contents: Zusammenfassung Im Wechsel zwischen realistischen Aufnahmen am Unfallort und grafischen Darstellungen statistischer und medizinischer Erhebungen wird aus Anlaß des 25jährigen Bestehens des Kölner Notarztdien stes anhand von etwa 80000 Einsätzen die Organisation, Indikation und Effektivität des Notartzeinsatzes dargestellt und über häufige Maßnahmen bei der Versorgung des chirurgischen Notfallpatienten berichtet. 97 aller Einsatzorte ließen sich innerhalb von 10 min erreichen. Die Qualität der ersten Diagnose am Unfallort läßt sich ebenso belegen wie die größere Überlebenschance von Schwerverletzten, die am Unfallort ärztlich versorgt werden.
    Notes: Summary Presented in a lively sequence of realistic pictures of emergency situations and graphic tables about statistic and medical facts, a review is given of 80000 emergency cases within the 25 years of existence of the Cologne Emergency-Doctor Service. The organization, indication and effectiveness of this special task force are pointed out, and common measures of medical help for adequate care of injured patients are shown. Of all locations with emergency cases, 97 % were reached within 10 min. The quality of primary diagnosis at the place of the accident is shown clearly as is the higher chance for survival of polytraumatized patients in the hands of emergency doctors at the place of accident.

    Description / Table of Contents: Zusammenfassung Ca. 100 Jahre nach der ersten elektiven Appendektomie bestehen weiterhin oft Unsicherheiten in der Diagnose der Appendicitis, erkennbar an zu häufigen fehlinduzierten Laparotomien. Mittels einer prospektiven Untersuchung bei 75 Patienten wurden die üblichen diagnostischen Tests und anamnestischen Angaben untersucht (Sensivität, Spezifität, pos. Vorhersagewahrscheinlichkeit). DieKombination von 〉 10,000 Leukocyten/μm und Übelkeit zeigte eine hohe Treffsicherheit: Sensitivität 83%. Spezifität 87%, positiv 77%. Von nur 6 falsch positiven Fällen bestand bei 5 hingegen dennoch eine anders begründete Operationspflicht.
    Notes: Summary About 100 years after Morton's first elective appendectomy, it is still desirable to achieve greater accuracy in the diagnosis of appendicitis and thus avoid unnecessary laparotomies. A prospective study of 75 patients evaluated the common diagnostic tests and variables (e.g. sensitivity, specificity, and predictive values) in patients' histories. Thecombined presence of 〉 10,000 leukocytes/gmm and nausea was found to be most valuable: (sensitivity, 83% ; specificity, 87% ; positive predictive value, 77%). Of only six incorrectly diagnosed cases, five still required operative therapy for other conditions.
