    Notes: Summary We retrospectively reviewed the findings in 655 consecutive young patients who underwent contrastenhanced MR examinations (1.5T) of the head or spine. Their ages ranged from 4 months to 20 years (mean 10 years). There was a 1.7% incidence of minor adverse reactions to gadolinium (Gd)-DTPA, none of which required treatment; no serious adverse reactions were encountered. Based on the radiologic diagnosis the patients were divided into three groups: (1) normal, (2) CNS neoplasm, (3) abnormal but not neoplasm. There were 178 patients thought to have CNS neoplasms and of these 156 (88%) enhanced. Of 124 histologically confirmed neoplasms 115 (93%) showed enhancement after Gd-DTPA. Eight children had histologically confirmed spinal neoplasms; 5 of 6 neurofibromas and 2 ependymomas enhanced. In the 216 patients with abnormalities thought not to be neoplastic, the enhancement rate was 11%; most of the enhancing lesions were vascular malformations. There were very few examples of inflammatory disease, acute trauma or stroke among our patients.

    Notes: Summary We studied the signal and enhancement characteristics of the central skull base prospectively in 40 children aged 13 days to 8 years, on a 1.5 T MRI system. Identical standard short TR/TE spin echo sequences in the sagittal plane were performed before and after intravenous gadolinium-DTPA. The sequences used for comparison were filmed at identical window and level settings. Three independent observers assessed (1) the intensity of contrast enhancement of the basisphenoid, basiocciput and presphenoid, (2) the signal intensity of the spheno-occipital synchondrosis, (3) the degree of pneumatization of the sphenoid sinus and (4) the uniformity of signal intensity reflecting fatty replacement of the marrow of the basisphenoid, basiocciput and presphenoid. In 16% and 28% respectively of cases there was no enhancement of the basisphenoid and basiocciput after gadolinium administration; in 42% and 44% there was mild enhancement, and moderate or intense enhancement was observed in 42% and 28% of cases. Even when there was irregular fatty replacement, residual hemopoietic clements could enhance intensely. When skull base neoplasms are being investigated, the normal signal irregularity and enhancement of the central skull base in children must not be confused with pathologic invasion of the marrow.

    Notes: Abstract Acute disseminated encephalomyelitis (ADEM) is a white matter inflammatory disease which usually follows a viral infection or pharmaceutical intervention. We describe a case of presumed ADEM in a heart/lung transplant patient, the etiology of which cannot be elucidated. The fascinating aspect is the mode of clinical presentation and the rapid resolution of radiologic abnormalities. Histologic examination of the brain is provided in an attempt to elucidate the radiographic abnormalities.

    Notes: Glycosidic oxocarbenium ions A1+ were formed by isobutane chemical ionization from methyl 2,3,4,6-tetra-O-methyl-β-D-mannopyranoside, methyl 2,3,4,6-tetra-0-methyl-β-D-galactopyranoside and methyl 2,3,4,6-tetra-O-methyl-β-D-glucopyranoside (the ring - O-being converted into — O = ), and then- reaction with ammonia was studied by Fourier transform ion cyclotron resonance Spectrometry. Very slow formation (reaction efficiency 0.6-1.4%) of the adduct ion [A1 + NH3]4 was observed as the main process for carefully thermalized ions A1+. Interestingly, the efficiency of the adduct ion formation depends on the sterochemistry of ions A1+.

    Notes: The positive ion fast atom bombardment (FAB) mass spectra of oligosaccharides related to D-xylan and D-glucurono-D-xylans were investigated. Although the mass spectra of the underivatized oligosaccharides are relatively weak, they still offer reliable information on the molecular weight of these compounds from their [M + H]+ and [M + H + glycerol]+ ions. Esterification, glycosidation and/or partial methylation improve the relative abundances of the molecular and fragment ions with respect to those of the glycerol matrix. The mass spectra of these derivatized compounds also allow the determination of the mass of the monosaccharide units by the occurrence of more abundant and characteristic fragment ions. The differently linked D-xylobioses of the (1 → 2), (1 → 3) and (1 → 4) type cannot be distinguished on the basis of their mass spectra alone. The collisional activation (CA) spectra of the [M + H + glycerol]+ adduct ions, however, revealed that an unequivocal determination of the type of linkage in these isomers is possible. The small values of the discrepancy factors in the FAB CA spectra prove that CA mass spectrometry is, also in combination with FAB ionization, the most precise of the current mass spectrometric techniques.

    Notes: Distinction between the linkage types 1 → 2, 1 → 3 and 1 → 4 of xylobioses can be achieved on the basis of the unimolecular decomposition spectra of the oxonium ions of the per-O-acetylated methyl glycosides. The spectra of the oxonium ions of various unbranched xylotri-, tetra- and pentaoses allow determination of the linkage position between the xylose residues. This indicates that in unbranched peracetylated xylo-oligosaccharides the linkage between the xylose residues at the non-reducing end can be determined.

    Notes: The protonated and sodium cationized pseudomolecular ions of a series of reducing and non-reducing oligosaccharides possessing aldopentopyranosyl, 6-deoxyaldohexopyranosyl, ketohexofuranosyl, 4-O-methyl glucuronopyranosyl methyl ester and N-acetyl 2-deoxyglucopyranosylamine units were studied using collision-induced dissociation (CID) fast atom bombardment (FAB) mass spectrometry. The CID spectra of the [M + H]+ ions provide information about the sequence of the monosaccharide units, while dissociation of the [M + Na]+ ions by two-bond ring cleavage processes gives rise to fragment ions which allow differentiation of 1 → 2 and 1 → 4 linkages. The influence of the structure of the constituent monosaccharides on the dissociation of the [M + H]+ and [M + Na]+ ions is discussed. An unusual type of fragmentation, which we suggest involves the elimination of internal monosaccharides residues, has not only been observed in the FAB mass spectra but also in the unimolecular decomposition and CID spectra of both [M + H]+ ions and oxonium ions from several oligosaccharides containing monosaccharide residues with different masses.

    Notes: Per-O-acetylated methyl glycosides of D-xylan-type di- and trisaccharides were studied by mass-analysed ion kinetic energy (MIKE) and collisionally induced dissociation (CID) mass Spectrometry using protonated ammonia and methylamine, respectively, as reaction gases in chemical ionization (CI). The oligosaccharides form abundant cluster ions, [M + NH4]+ or [M + CH3NH3]+, and the main fragmentation of these ions in the MIKE and CID spectra is the cleavage of interglycosidic linkages. Thus, CI (NH3) or CI (CH3NH2) spectra in combination with the MIKE or CID spectra allow the molecular masses, the masses of monosaccharide units and the branching point in oligosaccharides to be established. In the case of disaccharides, it is possible to distinguish the (1 → 2) linkage from the other types of linkages.

    Notes: The 13C NMR chemical shifts of a series of phosphorinanes and the corresponding 4-phosphorinanones have been evaluated using ‘double difference’ plots. Most of the compounds fit the same linear relationships for the carbons alpha and beta to the hetera group previously found for the analogous sulfur, oxygen and nitrogen six-membered heterocycles, suggesting that these phosphorus compounds possess similar chair conformations. For the carbons gamma to the hetera group, the plots suggest that there are transannular interactions between the trivalent phosphine groups and the carbonyl group, similar to that previously observed for the sulfide group. In these 1-hetera-4-cyclohexanones, transannular interactions appear whenever a lone pair of electrons on a second-row heteroatom is present in the proper conformation for interaction with the carbonyl group.
