    Description / Table of Contents: Zusammenfassung Im Plasma einer Patientin mit Mammacarcinom konnte ein Enzym nachgewiesen werden, welches RNA als Template für die DNA-Synthese heranziehen kann. Dieses Enzym steht in Analogie zur reversen Transcriptase onkogener RNA-Viren und dem aus der Muttermilch isolierten Enzym von Spiegelmanet al. Die mögliche diagnostische Anwendbarkeit dieses Befundes wird diskutiert.
    Notes: Summary In the plasma of a patient with breast cancer, an enzyme was shown that might use natural RNAs as templates for the DNA synthesis. This enzyme is paralleled to the reverse transcriptase of oncogenic RNA viruses and the enzyme of Spiegelmanet al. from particles of human milk. The possible diagnostic implications of these findings are discussed.

    Notes: Summary The clinical course of 50 breast cancer patients whose first metastases were found in the lungs was investigated. 18 months after the start of primary treatment 50% of the patients had developed pulmonary metastases (range: 0–81 months). In 23% of patients a solitary, and in 68% more than one, lung metastases were detected. After a median time of 4 months, in 56% of patients the disease had spread to further organs with bone (25%) and liver (17%) being the most frequent sites. First line management of lung metastases employed surgery, endocrine treatment, chemotherapy or a combination of these modalities. In 24% of patients a complete response was achieved, and in 11% a partial response, with an overall response rate of 35%. Median survival from detection of lung metastases was 13 months (range 4–123+). Patients with only a solitary lung metastasis survived for a median of 11.5 months as compared to 10.5 months for patients with more than one pulmonary metastases. Patients with a disease-free interval of more than 18 months survived significantly longer than patients with a disease-free survival of 18 months or less.

    Description / Table of Contents: Zusammenfassung Die Arbeitsgruppen um Spiegelman und unserer Laboratorien haben nachgewiesen, daß das Enzym reverse Transcriptase auch extrazellulär bei malignen Erkrankungen aufgefunden werden kann. Ein solches Enzym wurde von Spiegelman aus der Muttermilch, von uns aus dem Plasma von Patientinnen mit Mammacarcinom und leukämischen Erkrankungen isoliert. Das von uns isolierte Enzym wurde zur elektrophoretischen Homogenität angereichert und kann 1) synthetische Templates (Primers) mit der der viralen Transcriptase inhärenten Spezifität ablesen; 2) heteropolymere RNA-Anteile einer einsträngigen Phagen-RNA in eine homologe DNA-Sequenz übersetzen und 3) weist chromatographisch ähnliches Verhalten wie die reverse Transcriptase onkogener RNA-Tumorviren auf. Vorläufige Untersuchungen zum Nachweis eines analogen Enzyms im Plasma bei anderen malignen Erkrankungen haben bisher kein positives Ergebnis gebracht.
    Notes: Summary Work from Spiegelman and from our laboratories documented extracellular occurrence of the enzyme reverse transcriptase in malignant states. This enzyme was shown to be present in particles of human milk by Spiegelman and in patients with breast cancer or leukemias by us. The isolated enzyme was purified to electrophoretic homogenity and 1) Utilizes synthetic template—primers with a specificity like the viral reverse transcriptase. 2) Is capable of copying heteropolymeric regions of natural RNA-template-primer complexes. 3) Shows the chromatographic behaviour of the viral enzyme. These preliminary investigations documented the presence of the above mentioned enzyme only in these two forms of malignancies, while the other patients looked treated for the probably viralrelated enzyme in the plasma were negative.

    Notes: Summary Mucin-like cancer-associated antigen (MCA), a new tumor marker using the mouse monoclonal antibody b-12 is thought to be of value in the management of patients with breast cancer. In this study sera from 191 female patients with breast cancer (112 with progressive disease [PD] and 79 with no evidence of disease [NED]) were analyzed for MCA levels and compared with those of cancer antigen 15-3 (CA 15-3) in single determination and in combination with carcinoembryonic antigen (CEA) and tissue polypeptide antigen (TPA). A cut-off level of 14 U/ml for MCA seems to be more appropriate than the recommended 11 U/ml to distinguish between PD and NED in patients with breast cancer. Although there was a fairly good correlation of MCA to CA 15-3, MCA was inferior in sensitivity and specificity to CA 15-3. Patients with osseous metastases and those with more than one metastatic site showed higher MCA levels than patients with visceral or soft tissue metastases, a fact which was comparable to CA 15-3. Combining MCA and CA 15-3 resulted in a gain in specificity but marked loss of sensitivity. The combination of MCA and CEA results also in a loss of sensitivity whereas the combination of CA 15-3 and CEA showed an increased specificity and only a negligible loss of sensitivity. The combination of MCA with TPA is of little value in the follow-up of breast cancer, as is the combination of CA 15-3 with TPA. The combination of CA 15-3 with CEA can be still recommended for follow-up for early detection of metastases in breast cancer. Further investigations with MCA to determine the lead time of this new marker as well as its role as a possible monitor of the response to therapy are recommended.

    Notes: Summary Twenty-six cycles of high-dose ifosfamide+mesna (HD-IFO+M) were applied to seven female patients with advanced breast cancer refractory to prior treatment, using three different durations of continuous infusion (4, 24, and 48 h) every 3 weeks. To evaluate the most tolerable time schedule, the duration of the infusions was changed periodically in each patient. Toxicity was low in general, but continuous infusion of HD-IFO+M over 24 h appeared to be the best tolerated. On partial response lasting 27 weeks was achieved and four patients achieved stable disease lasting from 9 to 12 weeks.

Ein Vergleich der gefundenen Wirkungen mit bereits publizierten Ergebnissen zeigt dennoch eine, wenn auch nur partielle Blockierbarkeit der Oxyfedrineffekte durch Propranolol. Die anderen, z. T. gegenüber der alleinigen Gabe von Oxyfedrin sogar verstärkten Wirkungen wie die signifikante Abnahme des arteriellen Spiegels und der myokardialen Bilanz von Pyruvat werden durch gleichsinnige, additive Wirkungen von Propranolol und Oxyfedrin erklärt.
    Notes: Summary Following beta-adrenergic blockade with propranolol, infusions with oxyfedrin caused significant increases in heart rate, arterial free fatty acids and 3-hydroxy-butyrate levels, uptake of free fatty acids into the heart and liver, and a significant decrease in coronary vascular resistance. A comparison with data from previous experiments without beta-adrenergic blockade shows at least a partial blockade of the hemodynamic and metabolic effects caused by oxyfedrin. The other metabolic parameters, showing identical changes with oxyfedrin and propranolol, could not be taken for a comparison of the action of oxyfedrin before and after betaadrenergic blockade.

Während die Freien Fettsäuren und die Ketonkörper einen starken arteriellen Anstieg sowie auch eine vermehrte Extraktion durch das Myokard zeigten, kam es zu einem arteriellen Abfall und verminderter myokardialer Aufnahme von Lactat und Pyruvat. Die unterschiedliche Wirkung von Oxyfedrin und Katecholaminen auf den Myokardstoffwechsel wird diskutiert.
    Notes: Summary The influence of Oxyfedrin on the arterial levels and the myocardial extraction of glucose, lactate, pyruvate, free fatty acids, acetoacetate, 3-hydroxybutyrate, esterified fatty acids and glycerol has been studied by coronary sinus catheterization on dogs. The arterial levels and myocardial extraction of free fatty acids and ketone-bodies were markedly increased, the arterial concentration and myocardial uptake of pyruvate and lactate were found to be decreased. The different effect of Oxyfedrin and catecholamines on myocardial metabolism has been discussed.

Während der arterielle Mitteldruck nur gering sinkt, ist die Abnahme des coronaren Widerstandes ebenfalls statistisch gesichert. Die arteriellen Spiegel der Freien Fettsäuren sowie von Acetacetat und 3-Hydroxybutyrat zeigen ebenso wie die Aufnahme dieser Substrate in das Myokard eine Zunahme, die jedoch nur bei den Freien Fettsäuren signifikant ist. Auch die hepatale Aufnahme der Freien Fettsäuren nimmt unter Oxyfedrin zu, während die Ketonkörper vermehrt aus der Leber abgegeben werden. Umgekehrt ist die Wirkung der Substanz auf die Metaboliten des Kohlenhydratstoffwechsels. Während die arteriellen Spiegel sowie die myokardialen Bilanzen von Pyruvat und Lactat statistisch gesichert abnehmen, werden diese Substrate — gegenüber einer hepatalen Aufnahme in der Vorperiode — unter Oxyfedrin aus der Leber abgegeben. Die Glucose wird, mit Ausnahme einer signifikanten Abnahme der myokardialen Aufnahme, nur gering beeinflu\t. Auf Grund der erhobenen Befunde wird eine betamimetische Wirkung der Substanz angenommen.
    Notes: Summary Intravenous infusions of 10 ug/kg/min oxyfedrin exhibit significant increases in heart rate, coronary blood flow, dp/dt and myocardial oxygen consumption together with decreases in mean arterial blood pressure and coronary vascular resistance. Arterial level as well as the myocardial uptake of free fatty acids, acetoacetate and 3-hydroxy-butyrate rise distinctly, accompanied by an increasing hepatic extraction of free fatty acids and higher release of ketone-bodies by the liver. Glucose levels are only slightly effected by oxyfedrin infusions, but arterial levels, myocardial uptakes and hepatic extraction of pyruvate and lactate decline continuously, leading even to release of the two substrates from the liver. Based on these results oxyfedrin is discussed as aΒ-adrenergic sympatomimetic agent.

Keine Verbesserung brachte die Immunotherapie mit Azimexon.

Im Gegensatz zur Auswertung nach 30 min konnte kein signifikanter Unterschied bei den adjuvant behandelten Patientinnen nach 48 Monaten gefunden werden. Patientinnen mit receptor-negativen, Grad III Tumoren überlebten mit Chemotherapie signifikant besser.
