    Notes: Abstract The role of conventional arthrography versus computed tomography (CT) arthrography of the glenohumeral joint using iotrolan was evaluated in patients with different shoulder problems. In addition, a diagnostic combination of conventional and CT arthrography was compared with magnetic resonance (MR) arthrography of the glenohumeral joint. Two diagnostic protocols were used. Protocol 1: conventional followed after 30 min by CT arthrography of 37 joints using a double contrast technique with iotrolan 300. Protocol 2: conventional followed after 90–180 min by MR arthrography in 20 patients using a single-contrast technique with 10 ml iotrolan 300 and 1 ml gedopentetate dimeglumine 500 mM. Ten patients also underwent CT arthrography. Neither patient group experienced contrast-related complications. Image quality was good for all conventional arthrograms, excellent in 45/47 CT arthrograms and good in 20 MR arthrograms. CT and MR arthrography were diagnostically valuable in many patients. We conclude that glenohumeral joint evaluation should be perfomed first using conventional or CT arthrography. Iotrolan has proven to be highly reliable and safe in these applications. Iotrolan in combination with gadopentetate dimeglumine, permits MR arthrography following completion of the standard examinations if necessary.

    Description / Table of Contents: Zusammenfassung In einer klinischen Studie sollte geprüft werden, ob durch Kombination von direkter Vergrößerungsmammographie und Speicherfolientechnik die digitale Mammographie mit einer diagnostisch ausreichenden Bildqualität realisierbar ist und zu einer vergleichbaren oder verbesserten Mammadiagnostik führen kann. Mit einem Mammographiesystem für direktradiographische Vergrößerungsaufnahmen wurden digitale Übersichtsmammographien in 1,5- bzw. 1,7facher Vergrößerung und Ausschnittsvergrößerungen in 4facher Vergrößerung angefertigt. Es erfolgte ein intraindividueller Vergleich von 100 digitalen Übersichtsmammogrammen und 50 digitalen Zielaufnahmen mit den konventionellen Voraufnahmen derselben Patientinnen. Digitale Übersichtsmammographien in 1,5- bzw. 1,7facher Vergrößerungstechnik zeigten im Hinblick auf Herdbefunde, Mikroverkalkungen und klinische Konsequenzen keine signifikant abweichenden Ergebnisse im Vergleich zu konventionellen Übersichtsmammogrammen. Die Ausschnittsvergrößerungen erbrachten jedoch eine verbesserte Beurteilbarkeit von Mikroverkalkungen. In ∼ 15 % führte dies zu einer Änderung der diagnostischen Vorgehensweise. Die digitale Mammographie in Speicherfolientechnik kann durch Kombination mit der direkten Vergrößerungstechnik und der damit verbundenen Verbesserung der Ortsauflösung erstmals sinnvoll eingesetzt werden. Übersichtsmammogramme erbrachten eine vergleichbare diagnostische Aussagekraft, während Ausschnittsvergrößerungen eine verbesserte Beurteilung von Mikroverkalkungen ermöglichten.
    Notes: Summary Introduction: The combination of direct magnification mammography and computed radiography provides an improvement in spatial resolution of storage phosphor-based digital systems. A clinical study comparing conventional and digital direct magnification mammograms was performed. Methods: 100 survey mammograms in 1.5- or 1.7-fold magnification and 50 4-fold spot magnification views were obtained with a prototype direct magnification mammography system and a storage phosphor-based digital system. An intraindividual comparison of these with previous conventional radiograms of the same patients was carried out. Results: The diagnostic value of digital survey mammograms using the direct magnification technique is comparable to that of conventional radiograms of the breast, especially with regard to the identification of microcalcifications and lesions and the clinical consequences. Spot magnification views performed with this combination of techniques allowed improvement in the evaluation of microcalcifications. In 15 % of cases, diagnostic procedures were adjusted accordingly. Conclusion: The combination of the direct magnification technique with digital storage phosphor radiography systems allows the performance of digital mammography by improving the overall spatial resolution. The diagnostic value of digital direct magnification survey mammograms was comparable to that of conventional mammograms. Digital 4-fold spot magnification views improved visualisation of the morphologic aspects of microcalcifications.
