Zusammenfassung Eine akute hepatische Porphyrie vom Typ der hereditären Koproporphyrie wurde bei einer 28jährigen Frau mit Tetraplegie und Atemlähmung erkannt, bei der sechs Jahre vorher eine Epilepsie diagnostiziert wurde. Die Porphyriediagnose basiert auf einer hohen Ausscheidung von δ-Aminolävulinsäure, Porphobilinogen und Koproporphyrin III im Urin sowie von Koproporphyrin III in der Galle. Die Aktivität der Koproporphyrinogen-Oxidase war in der Leber und Niere vermindert, während die Aktivität der δ-Aminolävulinsäure-Synthase stark erhöht war. Die Patientin war über zwölf Jahre krank: Neurologisch-psychiatrische, gastroenterologische, gynäkologische und urologische Diagnosen wurden getroffen, denen entsprechende therapeutische Maßnahmen einschließlich von neun Operationen folgten.

Summary An acute hepatic porphyria of the hereditary type was found in a 28-year-old woman with tetraplegia and respiratory paralysis, who had had epilepsy diagnosed 6 years previously. The diagnosis of porphyria was established by high excretion of urinary δ-aminolevulinic acid, porphobilinogen and coproporphyrin III as well as of biliary coproporphyrin III. Coproporphyrinogen oxidase activity was decreased in liver and kidney, whereas δ-aminolevulinic acid synthase activity was much increased. The patient had been ill for the past 12 years: neurologic-psychiatric, gastroenterologic, gynaecologic, and urologic diagnoses were made and followed by corresponding therapeutic measures including nine operations.
