Zusammenfassung Von 1982–1985 wurden 224 abdominale, 41 transhiatale und 9 abdominothorakale Gastrektomien, sowie 7 proximale Teilresektionen wegen Magencarcinom mit dem zirkulären Klammergerät ILS Proximate (25 mm Kopf) durchgeführt. Intraoperative Probleme bei der Anastomosierung waren selten (5,5%), gehäuft bei transdiaphragmalem oder abdominothorakalem Vorgehen. Die Insuffizienzrate war bei transhiatalem gegenüber abdominalem Zugang erhöht (22,6/7,9%). Eine klinische (n = 110) und radiologische (n = 86) Nachuntersuchung ergab 3mal eine funktionelle Passagebehinderung; 16mal Reflux, jedoch keine Narbenstenose im Anastomosenbereich.

Summary From 1982 to 1985, 224 abdominal, 31 transhiatal, and 9 abdominothoracic gastrectomies were performed because of gastric cancer, in addition to 7 proximal gastric resections. The procedures were carried out using the circular stapler ILS Proximate. Anastomosis problems occurred intraoperatively only in 5.5%, but the rate increased in the transhiatal and abdominothoracic procedures. The rate of anastomotic leakage was 7.9% after abdominal and 22.6% after transhiatal operations. Clinical (n = 110) and radiological (n = 86) studies showed no scar stenosis in the anastomosis area.
