    Description / Table of Contents: Zusammenfassung. Zwischen 1990 und 1995 wurden wegen posttraumatischer Fehlstellungen 27 Korrekturarthrodesen des subtalaren Gelenks durchgeführt. Präoperativ litten 24 Patienten (88,89 %) unter mäßigen bis starken Schmerzen, 15 (55,56 %) hatten eine Gehstrecke bis 100 m, 7 (25,92 %) waren deswegen vollberentet (berufsunfähig). Die Kontrolle durchschnittlich 14 (13,85) Monate postoperativ zeigte eine signifikante Schmerzreduktion. Achtzehn Patienten (66,67 %) hatten keine oder nur leichte Schmerzen, die Gehstrecke war bei 19 Patienten (70,37 %) frei. In Anlehnung an die von Angus und Skuginna erstellten Bewertungskriterien nach Korrektureingriffen am Fuß wurde bei 81,48 % der Patienten ein gutes bis befriedigendes Ergebnis erzielt. Entsprechend der von Kitaoka et al. erstellten Kriterien lag die durchschnittlich postoperativ erreichte Punktzahl bei 75,8.
    Notes: Summary. Between 1990 and 1995, 27 patients with posttraumatic disorders of the subtalar joint were treated by correcting fusion. Preoperatively 24 patients (88.89 %) suffered from pain; 15 (55.56 %) had a walking range below 100 m. Seven patients were unable to work in their original profession. After an average period of 13.85 months postoperatively the patients were examined again. At this time 18 (66.67 %) had no or minimal pain; 19 (70.37 %) had no claudication. According to the score of Angus and Skuginna, in 81.48 % of patients the result of the correcting arthrodesis was successful; according to Kitaoka's clinical rating system for ankle-hindfoot, midfoot, hallux and lesser toes, the average postoperative score was 75.8 points.

    Description / Table of Contents: Summary As pacing impedance is inversely related to pacing current, the increase of pacing impedance additionally decreases pacing current. Whether the impedance measurement at implantation predicts the outcome during follow-up, was studied in 87 patients who received the VDD-single lead UniPass 425 connected to the pacemaker Unity (Sulzer Intermedics). The impedance changes between implantation and 6 months follow-up were assessed for each patient. Similar impedance values were defined, if the two measurements were within a range ≤ -100 to +100 Ω. Six-months impedance was lower or higher compared to implantation, if the difference exceeded 〉 -100 or 〉 +100 Ω. At implantation, impedance was 535 ± 98 Ω (range: 333–811 Ω) and significantly increased to 604 ± 160 Ω (range: 361–1150 Ω) after 6 months. Mean difference between the two measurements was 69 ± 162 Ω (range: -336 bis +560 Ω). Similar impedance had 43 (implantation: 527 ± 75 Ω, 6 months: 531 ± 87 Ω), lower values 11 (implantation: 660 ± 83 Ω, 6 months: 494 ± 73 Ω) and higher values 33 patients (implantation: 503 ± 99 Ω, 6 months: 735 ± 168 Ω). Compared to the patients with similar impedance patients with lower impedance had a significantly higher impedance values at implantation. Conclusions: Pacing impedance increased significantly within 6 months after implantation. Pacing impedance changed 〉 100 Ω in 51% of the patients. The long-term follow-up of pacing impedance can be predicted generally, but not for the individual patient.
    Notes: Zusammenfassung Da sich die Stimulationsimpedanz invers zum Stimulationsstrom verhält, senken hohe Impedanzwerte bei gleichbleibenden Stimulationsimpulsen zusätzlich den Stimulationsstrom. Inwieweit der intraoperativ gemessene Impedanzwert der Ventrikelstimulation repräsentativ für den Langzeitverlauf ist, wurde bei 87 Patienten untersucht, die die VDD-Einzelelektrode UniPass 425 und den Schrittmacher Unity (Sulzer Intermedics) erhalten hatten. Es wurden die Impedanzänderungen zwischen Implantations- und 6-Monats-Zeitpunkt ermittelt. Als gleiche Impedanzwerte wurde definiert, wenn beide Meßwerte ≤± 100 Ω voneinander abwichen. Kleinere bzw. größere Impedanzwerte lagen 6 Monate nach Implantation vor, wenn sie sich 〉 ± 100 Ω vom Implantationswert unterschieden. Intraoperativ betrug die Impedanz 535 ± 98 Ω (Spannweite: 333–811 Ω) und war nach 6 Monaten signifikant auf 604 ± 160 Ω (Spannweite: 361–1150 Ω) angestiegen. Der Unterschied zwischen den beiden Impedanzwerten betrug 69 ± 162 Ω (Spannweite: -336 bis +560 Ω). Gleiche Impedanzwerte lagen bei 43 (Implantation: 527 ± 75 Ω, 6 Monate: 531 ± 87 Ω), kleinere bei 11 (Implantation: 660 ± 83 Ω, 6 Monate: 494 ± 73 Ω) und größere bei 33 Patienten (Implantation: 503 ± 99 Ω, 6 Monate: 735 ± 168 Ω) vor. Im Vergleich zu den Patienten mit gleichbleibenden Impedanzwerten hatten Patienten mit niedrigeren Impedanzen bei Implantation signifikant höhere Ausgangswerte. Schlußfolgerungen: Die Stimulanzimpedanz nimmt im Laufe von 6 Monaten nach Implantation signifikant zu. Bei 51% der Patienten kommt es zu deutlichen Veränderungen der Stimulationsimpedanz. Diese lassen sich in der Tendenz, aber nicht im Einzelfall voraussagen.

    Notes: The Saccharomyces cerevisiae Lanosterol 14α-demethylase (14DM) gene was overxpressed in S. cerevisiae using Promoter sequences of the highly expressed S. cerevisiae glyceraldehyde-3-phosphate dehydrogenase TDH3 gene. To investigate factors affecting 14DM overproduction, the levels of 14DM-specific specific RNAs, apoprotein, and heme protein, repectively, were determined and the 14DM-specific RNA levels compared with the RNA levels originating from the enodogenous TDH gene(s). The quantitative measurements revealed that the 14DM steady-state RNA levels reached were some three-to five-fold below the theoretically expected values. With a View towards futrher improving expression of the 14DM gene, the specing between the TDH3 promoter and the AUG was adjusted precisely and to rule out possible toxic effects exerted by the 14DM protein, the TDH3 promoter was placed under galactose regulation by introducing an UASG segment. Furthermore, the effects of the gene copy number on 14DM overproduction were investigated. From the analysis of the improved expression constructs five conclusions could be reached: (1) experssion from the native 14DM gene is comparable to the expression driven by the TDH3 promoter-14DM fusion construct on single copy plasmid vectors; (2) expression from the TDH3 promoter-14DM construct on single-copy vectors is nearly as effcient as expression from the corresponding endogenous TDH3 gene; (3) the gene copy number has an effect on the relative expression levels of the TDH3 promoter-14DM constructs; (4) the steady-state amounts of protein produced are very nearly proportional to gene dosage; and (5) protein toxicity does not have a major impact on 14DM production. The maximum yield of 14DM was in the order of 7% of the total yeast protein and the maximum production of functional 14DM heme protein appears to be limited by the availability of heme.
