Summary 1. When classifying pain in the clinic one should distinguish: a) the prickling pain, b) the burning pain (causalgiform pain), c) the piercing pain, d) the bruising, pressing, and convulsive pain. 2. Spontaneous pain arises from pathologic irritation of the primarily sound substratum with full functioning capacity. 3. Hyperpathy, however, is an abnormal process of sensation to pain and temperature by normal irritation on the pathological, numerically decimated substratum. It is manifested, according to a general rule, by an injury to the pathways of pain in all layers. 4. Hyperalgesia is the excessive sensation of pain to normal irritation at a chemically (metabolically) lowered local source of pain. 5. Hyperpathy is characterized by an inadequate, very disagreeable excessive reaction to normal irritation, preferably affecting large areas or the deep substrata. The hyperpathic sensation arises at an elevated threshold of irritation. It is characterized by a period of latency and post-duration, undistinguished localization of the irritation area, and diffuse irradiation in the irritated area.

Résumé 1. En classifiant la douleur à la clinique, il faut qu'on distingue: a) la douleur picotante, b) la douleur ardente (douleur causalgieforme), c) la douleur pénétrante, d) la douleur contusionnante pressante et convulsive. 2. La douleur spontanée naît par l'irritation pathologique du substrat qui, en premier lieu, est sain et complètement capable de fonctionner. 3. L'hyperpathie, d'autre part, est un procès de sensation anormal à l'égard de la douleur et de la température par une irritation normale sur le substrat pathologique, numériquement décimé. Elle est manifestée, selon une règle générale, chez une lésion des voies de douleur dans toutes les couches. 4. L'hyperalgésie est la sensation de douleur excessive répondant à l'irritation normale chez une source de douleur locale abaissée chimiquement (métaboliquement). 5. L'hyperpathie est charactérisée par une réaction excessive inadéquate très désagréable répondant à l'irritation normale qui, préférablement, influence de vastes parties ou les substrats profonds. L'origine de la sensation hyperpathique est un seuil d'irritation élevé. Elle est charactérisée par une période de latence et de post-durée, par une localisation peu distincte du lieu d'irritation et par une irradiation diffuse dans la partie irritée.

Zusammenfassung 1. In der Klinik sollte man bei der Einteilung der Schmerzarten unterscheiden: a) den Kribbel-Prickel-Schmerz, b) den Glüh-Brennschmerz (kausalgiformen Schmerz), c) den schneidend-stechenden Schmerz, d) den quetschend-pressend-krampfenden Schmerz. 2. Der Spontanschmerz entsteht durch pathologische Reizung des zunächst gesunden, voll funktionsfähigen Substrates. 3. Hyperpathie hingegen ist ein abnormer Empfindungsvorgang für Schmerz und Temperatur durch normalen Reiz am pathologischen—zahlenmäßig dezimierten—Substrat. Sie manifestiert sich auf Grund eines allgemeinen Gesetzes bei einer Schädigung der Schmerzbahn in allen Schichten. 4. Hyperalgesie ist die übermäßige Schmerzempfindung auf normalen Reiz bei chemisch (metabolisch) gesenkter örtlicher Schmerzquelle. 5. Hyperpathie ist charakterisiert durch inadäquate, besonders unangenehme Überreaktion auf normalen, vorwiegend großflächigen oder auf die Tiefensubstrate wirkenden Reiz. Die hyperpathische Empfindung entsteht bei erhöhter Reizschwelle, ist charakterisiert durch Latenz und Nachdauer, schlechte Lokalisation des Reizortes und diffuse Irradiation im Reizgebiet.
