    Description / Table of Contents: Zusammenfassung Zur Korrektur extremer Schlingen- und Knickbildungen der A. carotis interna in 29 Fällen wurden 17 Verlagerungsoperationen, 8 Desobliterationen mit Venenpatch und Streckung und 4 Resektionen durchgeführt. Vorteil der Verlagerungsoperation, die bei Ausschluß von Arteriosklerose erfolgt, ist das minimale Operationsrisiko. Die Nachuntersuchung der Patienten (1/2 — 7 Jahre) zeigte nach allen 3 Operationsverfahren bis auf 2 Fälle Beschwerdenfreiheit der Patienten. Verlagerungsoperationen sind bei extremer Schlingenbildung der A. carotis ohne begleitende Arteriosklerose indiziert.
    Notes: Summary Surgical correction of kinked internal carotid artery in 29 patients was done by straightening the kink by transposing the digastric muscle under the artery (17), by endarterectomy and patch plasty (8) and by resection (4). Straightening of artery was done, if no arteriosclerotic lesion was found. The advance of this surgical treatment is a minimal risk. In a follow-up study from 1/2 - 7 years only two patients had recurrence of symptoms due to kink. Straightening procedures have their indication in kinked artery without arteriosclerosis.

    Description / Table of Contents: Summary In patients with an acute arterial occlusion of the right upper extremity a Doppler scanning examination should be done before balloon catheter embolectomy. If there is no arterial puls detectable, an angiography should be performed afterwards to localize the embolus. In case of a proximal arterial occlusion of the right arm we recommend this procedure to prevent an embolus dislocation by catheter embolectomy and following cerebral embolization. For direct surgical embolectomy we recommend a supraclavicular incision.
    Notes: Zusammenfassung Bei klinischem Verdacht auf arteriellen Verschluß der rechten oberen Extremität sollte vor retrograder Ballonkatheterembolektomie eine Duplex-Sonographie der A. axillaris durchgeführt werden. Findet sich in der Arterie kein Fluß, sollte eine Angiographie erfolgen, um die Lokalisation des Embolus aufzuzeigen. Bei sehr proximalem Verschluß der arteriellen Strombahn des rechten Armes empfehlen wir dieses Vorgehen, da eine retrograde Katheterembolektomie zu einer Dislokation des Embolus und nachfolgender cerebraler Embolisation führen kann. Für die operative Therapie eignet sich in diesem Fall ein supraclaviculärer Zugang.

    Notes: Abstract. High-speed trains could be used to solve two different accessibility problems. In the first case, where a point to point link is dominant, they are a potential substitute for air travelling. In the second case it links together many cities and, hence, creates a new type of region or corridor with a high interregional accessibility. One important hypothesis for the discussion in this paper is the degree to which cities that are linked together into a band of cities by means of a high-speed train connection are transformed to an extended functional region or in other words an integrated corridor economy. This paper particularly examines economic integration in a corridor economy in the short, medium and long term. In considering the short-term perspective we discuss not only the integration of goods and service markets but also the integration of labour markets and markets for shopping, private services and leisure activities. The discussion of the medium term perspective is concentrated on the relocation of households and firms within a high-speed train corridor. To study the long-term integration effects of a high-speed train corridor we maintain that the analysis must be conducted using a genuinely dynamic model for the specialisation of production and, hence, for trade with and transport of goods and services and consequently transport of people.

    Notes: Abstract A method is described for the dissociation of guinea pig lung and mesentery into their component cells. The method comprises incubation of the tissues with the enzyme pronase in Ca++- and Mg++-free saline followed by mechanical dispersion and yields on average 3–8% free mast cells. These cells are morphologically intact and viable. They retain an active sensitization or can be passively sensitized. The levels of cAMP and cGMP in the whole cell suspension respond to catecholamine, histamine or cholinergic stimulation in a way similar to that observed in the corresponding undispersed tissue. Although the separation of guinea pig mast cells from other cells proved to be more difficult than the purification of rat mast cells, an inrichment of greater than 50% has been achieved. The mast cells isolated by this method proliferate in vitro and have been cultured successfully for several months.

    Description / Table of Contents: Summary Renal artery stenosis (RAS) is the most common cause of secondary hypertension, with a prevalence of about 1% in the general population of people with hypertension. Severe arterial stenosis may also lead to impairment of excretory renal function. In experienced hands renal artery revascularization with or without stent implantation may be a safe and effective treatment in patients with sustained hypertension resistant to intensive antihypertensive treatment. Conventional balloon angioplasty of non-ostial RAS caused by fibromuscular dysplasia with a high technical and functional success rate may be the treatment of choice. However, there is continous discussion concerning the utility of ballon angioplasty and renal stenting, respectively, in patients with atherosclerotic disease. At the time being, there are 3 randomized European trials ongoing to analyze the benefit of medical treatment versus percutaneous intervention. Several prospective studies dealing with renal artery stenting in ostial RAS found that the implantation of endoprostheses leads to much better morphologic longterm results as compared to those of balloon angioplasty alone and may be a safe and effective alternative to surgery. In addition, the functional results suggest that stent implantation in patients with mild or severe renal dysfunction may slow progression of renal failure and, thus delay the need for renal replacement therapy. It is to note that renal artery stenting does not impede any further surgical intervention. However, prior to any interventional treatment the indication of an eventual catheter procedure in patients with RAS should be discussed between experienced nephrologists and interventionalists based on clinical, functional and duplexsonographic data.
    Notes: Zusammenfassung Die Nierenarterienstenose (NAST) ist die häufigste Ursache der sekundären Hypertonie mit einer Prävalenz von etwa 1% bei Hochdruckpatienten. Eine hämodynamisch signifikante NAST kann darüberhinaus in der Folge zu einer eingeschränkten Nierenfunktion führen. Die Ballonangioplastie der Nierenarterie mit oder ohne Implantation von Endoprothesen stellt potentiell eine kasuale Therapie der NAST bei Patienten mit therapierefraktärer Hypertonie dar. Die Intervention ist in erfahrenen Händen einfach, komplikationsarm und in aller Regel mit sehr gutem morphologischem Erfolg assoziiert. Die günstigen technischen und funktionellen Ergebnisse machen dieses Verfahren zur Therapie der ersten Wahl insbesondere bei fibromuskulärer Dysplasie. Ob alle Patienten mit arteriosklerotischer NAST einer angioplastischen Revaskularisation zugeführt werde sollen, ist derzeit Gegenstand von randomisierten Studien, die die Katheterangioplastie mit der medikamentösen Therapie vergleichend analysieren. Die Ergebnisse der Stentimplantation sind für die ostiale NAST im Vergleich zur Ballonangioplastie hinsichtlich des morphologischen Ergebnisses signifikant besser. Darüberhinaus zeigen unsere eigenen Ergebnisse und die der Literatur, daß die Katheterrevaskularisation mit Implantation von Endoprothesen die Nierenfunktion stabilisieren bzw. verbessern, und damit potentiell organerhaltend sein kann. Es ist anzumerken, daß die endoluminale Therapie mit Gefäßprothesen eine sekundäre chirurgische Intervention nicht ausschließt. Von herausragender Bedeutung ist die enge Zusammenarbeit des interventionell tätigen Radiologen mit erfahrenen Nephrologen mit dem Ziel einer „adäquaten” Indikationsstellung unter Einbeziehung klinischer, funktioneller und duplexsonographischer Daten.
