Summary By increasing the administered anti-Rh0(D) dosis from 240 µg to 360 µg the time required for the elimination of foetal Rh-positive red cells contained in a transplacental haemorrhage of 3 ml blood can be reduced by one day. However as soon as all receptors of the erythrocytes are blocked by the administered antibodies (anti-D), no further increase of the anti-D dosis can have any effect on the time of elimination. This fact has been confirmed by our experimental finding the time required for the elimination of Rh-positive red cells in 0,1 ml infused blood remained unchanged after administering 120 µg and 240 µg anti-D respectively. Also by administering the anti-Rh0(D) serum intravenously the time of elimination of foetal Rh-positive red cells transpassed to the mother can be further shortened. Thus 212 µg anti-D given intravenously eliminated the erythrocytes in a transplacental haemorrhage of 0,3 ml blood one day shorter than a 240 µg dosis administered intramuscularly. The intramuscular route of administration neverless has the advantage of providing a prophylaxis against serumhepatitis in cause of the gammaglobulincontain of the intramuscular serum. Until the full significance of the time of elimination of foetal Rh-positive red cells in the prevention of Rh-immuniziation is clear, we adwise, that the shortest possible elimination time with the corresponding anti-D dosis should be aimed at.

Zusammenfassung Durch Erhöhung der applizierten Antikörpermenge von 240 auf 360 µg Anti-D konnten bei Einschwemmungen über 3 ml um 1 Tag kürzere Abbauzeiten erzielt werden. Sind allerdings alle Rh-Receptoren der Erythrocyten von verabfolgten Antikörpern besetzt, läßt sich durch eine Steigerung der Dosierung keine schnellere Elimination erreichen. 2 ml Serum mit einem Anti-D-Gehalt von 120 µg eliminierten die fetalen Rh-positiven bei einer Einschwemmung im Bereich von 0,1 ml ebenso schnell wie 240 µg. Bei Modelluntersuchungen an Freiwilligen wurden ähnliche Ergebnisse erzielt. Durch die Anwendung von i. v. Seren kann die Abbauzeit verkürzt werden. 212 µg Anti-D eines intravenös applizerten Präparates eliminierten die fetalen Erythrocyten bei einer Einschwemmung um 0,3 ml um 1 Tag schneller als 240 µg Anti-D eines intramuskulär injizierten Serums. Die i.m. Präparate gewährleisten aber durch ihren Gammaglobulingehalt einen Schutz gegen die Serumhepatitis. Da bisher nicht auszuschließen ist, ob die Eliminationsgeschwindigkeit der fetalen Rhpositiven Erythrocyten bei der Prophylaxe der Rh-Sensibilisierung eine Rolle spielt, sollten möglichst kurze Abbauzeiten durch entsprechende Dosierung der Anti-Rh0(D)-Immunglobuline angestrebt werden.
