Summary The clinical and radiological differentiation of the space-occupying intracerebral lesions is even to-day occasionally very difficult. By means of early angiography, serial angiography, and encephalographic investigations such lesions are mostly identified in their early stages. In consequence very great diagnostic difficulties are developing. One is not always successful in separating lesions which certainly require operation (e. g. Tumour, Abscess, Granuloma, etc.) from those in which an operation is not necessary. Among the latter group one is chiefly concerned with vascular lesions with surrounding oedema, such as softenings or haematomata. In addition, cerebral thrombophlebitis must be taken into consideration as a possible diagnosis. Embolie-mycotic aneurysms and blood dyscrasias can in rare cases mimic the clinical picture of a brain tumour; the same is true for circumscribed inflammatory lesions and for radiation necrosis. Based on personal observations the problem of differentiating the various lesions is discussed. In many cases the duration of the clinical course provides a valuable aid to diagnosis. However, with a rapidly progressive clinical course an exploration cannot always be avoided.

Résumé La différenciation clinique et radiologique des lésions occupant un espace intracrânien est même aujourd'hui parfois très difficile. Par des moyens d'angiographie précoce, d'angiographies en série et d'investigations encéphalographiques, de telles lésions sont la plupart du temps identifiées dans leurs stades primitifs. En conséquence de très grandes difficultés de diagnostic se développent. On ne réussit pas toujours à distinguer les lésions qui nécessitent sûrement une opération (e. g. tumeur, abcès, granuloma, etc.) de celles où l'opération n'est pas nécessaire. Parmi ce dernier groupe, un est surtout l'objet de lésions vasculaires avec oedème environnant, telles que ramollissements ou hématomes. De plus, une thrombophlébite cérébrale doit être considérée comme une diagnostic possible. Des anévrismes emboliques, mycotiques et des «dyscrasie» du sang peuvent dans de rares cas imiter l'image clinique d'une tumeur du cerveau. Ceci est vrai aussi pour les lésions inflammatoires circonscrites et pour les nécroses de radiation. Basé sur des observations personnelles le problème de la différenciation des lésions diverses est discuté. Dans de nombreux cas, l'évolution clinique fournit une aide valable au diagnostic. Cependant, avec une évolution clinique à progression rapide, une exploration ne peut pas toujours être évitée.

Zusammenfassung Die klinische und röntgenologische Differentialdiagnose raumfordernder intracerebraler Prozesse ist auch heute noch gelegentlich recht schwierig. Durch frühe arteriographische, serienangiographische und encephalographische Untersuchungen werden solche Prozesse häufig schon in ihren Anfangsstadien erfaßt. Dadurch können große diagnostische Schwierigkeiten entstehen. Es gelingt heute nicht in jedem Falle, einen Prozeß mit unbedingter Operationsindikation (Tumor, Abszeß, Granulom u. ä.) von solchen abzugrenzen, die eine Operation nicht erforderlich machen. Am häufigsten handelt es sich bei letzteren um Gefäßprozesse mit Begleitödem, wie z. B. Erweichungen und Massenblutungen. Auch die Hirnvenenthrombose muß in die differentialdiagnostischen Überlegungen einbezogen werden. Embolisch-mykotische Aneurysmen und Bluterkrankungen können in seltenen Fällen ebenfalls das klinische Bild eines Hirntumors nachahmen, das gleiche gilt für umschriebene entzündliche Prozesse und den Röntgenspätschäden. An Hand eigener Beobachtungen wird die differentialdiagnostische Problematik der einzelnen Erkrankungen erörtert. In vielen Fällen erlaubt nur die klinische Längsschnittbeobachtung eine eindeutige Stellung der Diagnose. Bei rasch progredienten klinischen Verlauf wird aber eine Trepanation nicht immer zu vermeiden sein.
