    Notizen: Abstract: Paraduodenal hernia is a rare condition in which the small bowel loops are herniated into an unusual fossa in the periduodenal area. We treated a patient with paraduodenal hernia diagnosed pre-operatively. A 28-year-old woman was admitted to our hospital because of intermittent abdominal pain. Abdominal ultrasonography revealed a large tumor adjacent to the pancreas. Provisional diagnosis made according to computed tomography (CT) findings was tumor of the pancreas tail. However, on a CT scan performed after the administration of diatrizoate meglumine/diatrizoate sodium (Gastrografin, Schering, Berlin, Germany) the mass was shown as a jejunum loop located between the stomach and the pancreas body. Subsequent laparotomy revealed that the jejunum loop was herniated into an unusually large mesocolic fossa and that the hernial orifice was covered by the adhesion between the transverse and descending colons. It seemed that the small intestine within the mesocolic fossa was strangulated by this adhesion. The patient's abdominal pain resolved postoperatively. These observations suggest that paraduodenal hernia should be suspected in patients with chronic, atypical abdominal pain, regardless of the findings for small bowel obstruction.

    Notizen: Summary Our multimodal treatment of hepatocellular carcinoma (HCC) has brought about a significant improvement of the survival rate. It consists of a combination of hepatectomy and transcatheter arterial embolization using lipiodol (L-TAE). In order to facilitate L-TAE, we have developed a special catheter with notches. A group of patients with HCC (124 cases), excluding cases with absolutely non-curative resections and operative deaths, were treated between December 1980 and November 1986. Each case was treated for more than 1 year after hepatectomy. The patients were divided into two groups: A, patients with a single tumor not larger than 5 cm, and B, cases with larger tumors or more than one lesion. Some patients in each group were treated with L-TAE after hepatectomy. In group A, there was no significant difference in survival between treated and non-treated cases. In group B, L-TAE gave a significantly better survival than no postoperative treatment.

    Notizen: Abstract  In the debate on euthanasia and physician-assisted suicide, we have to exclude terminally ill patients in whom the desire for death is caused by major depression. However, it is still not clear to what degree major depression can be treated by psychiatric intervention in this setting. We evaluated the effect of antidepressant treatment in terminally ill cancer patients. Six cancer patients with suicidal ideas thought to be due to major depression were treated with tricyclic antidepressants. Three had requested terminal sedation to relieve them from their suffering. The median survival of five of these patients was 4 weeks after diagnosis; one was lost to follow-up. The efficacy of the antidepressant treatment was assessed using the Hamilton Rating Scale for Depression (HRSD). One week after the start of treatment with antidepressants, five of the six patients showed a marked improvement in their mood and showed no further suicidal thoughts or requests for terminal sedation. The average reduction in the HRSD score was 23.4 points (14–38; SD = 9.9). Antidepressant treatment can be effective in alleviating the desire for death due to major depression, even in terminally ill cancer patients.

    Notizen: Abstract A system for measurement of leaf gas exchange while regulating leaf to air vapour pressure difference has been developed; it comprises an assimilation chamber, leaf temperature controller, mass flow controller, dew point controller and personal computer. A relative humidity sensor and air and leaf temperature sensors, which are all used for regulating the vapour pressure difference, are mounted into the chamber. During the experiments, the computer continuously monitored the photosynthetic parameters and measurement conditions, so that accurate and intenstive measurements could be made. When measuring the light-response curve of CO2 assimilation for single leaves, in order to regulate the vapour pressure difference, the leaf temperature and relative humidity in the chamber were separately and simultaneously controlled by changing the air temperature around the leaf and varying the air flow rate through the chamber, respectively. When the vapour pressure difference was regulated, net CO2 assimilation, transpiration and leaf conductance for leaves of rice plant increased at high quantum flux density as compared with those values obtained when it was not regulated. When measuring the temperature-response curve of CO2 assimilation, the regulation of vapour pressure difference was manipulated by the feed-forward control of the dew point temperature in the inlet air stream. As the vapour pressure difference was regulated at 12 mbar, the maximum rate of and the optimum temperature for CO2 assimilation in rice leaves increased 5 μmolCO2 m−2 s−1 and 5°C, respectively, as compared with those values obtained when the vapour pressure difference took its own course. This was reasoned to be due to the increase in leaf conductance and the decrease in transpiration rate. In addition, these results confirmed that stomatal conductance essentially increases with increasing leaf temperature under constant vapour pressure difference conditions, in other words, when the influence of the vapour pressure difference is removed. This system may be used successfully to measure inter- and intra-specific differences and characteristics of leaf gas exchange in plants with a high degree of accuracy.

    Notizen: Abstract This study evaluates the hypothesis thatcerulein relaxes the sphincter of Oddi (SO) via nitricoxide (NO). The spontaneous motility and the response tocerulein on the canine SO were recorded using aconstant-perfusion technique.NG-L-arginine-methyl-ester (L-NAME) increasedthe spontaneous motility and dose-dependently reducedthe cerulein-induced inhibitory response of the SO.After treatment with L-NAME at higher doses, cerulein induced an excitatory response.This effect was reversed by treatment with excessL-arginine. Similar results were obtained usingcholecystokinin octapeptide in place of cerulein. Inseparate studies, cerulein generated increases inintracellular cAMP and cGMP levels in the SO. Thisindicates that the intracellular mechanism mediatingcerulein-induced relaxation involves the production ofcAMP and cGMP. On the other hand, treatment withL-NAME absorbed the increase in cAMP and cGMP levels bycerulein. These studies demonstrate that ceruleinrelaxes the canine SO mainly via NO, increasingintracellular cAMP and cGMP levels.
