    Notes: Helicobacter pullorum and Campylobacter lari are rarely isolated from humans with acute enteritis. Hitherto the two species could only be identified by genotypic techniques. Gas liquid chromatography of whole cell fatty acid extracts is described as the first phenotypic method for discrimination of the two species. Cholesteryl glucoside, a characteristic feature of the genus Helicobacter, but seldom found in other bacteria, could not be detected in Helicobacter pullorum. Therefore, rapid determination of this glycolipid may serve as a discrimination marker for Helicobacter pullorum from most other Helicobacter species.

    Notes: Abstract Fluorescent oligonucleotide probes complementary to variable regions of Porphyromonas gingivalis and Bacteroides forsythus 16S ribosomal RNA were used to identify these organisms in smears of formaldehyde-fixed subgingival plaque samples from patients suffering from periodontitis. Fluorescence in situ hybridization represents a useful method for assessing the microbial ecology of the periodontal flora.

    Notes: Summary ¶ In order to identify overall and site-specific nosocomial infection (NI) rates in patients receiving neurosurgical intensive care therapy, a prospective study was started in February 1997 in the eight-bed neurosurgical ICU of the University Hospital of Freiburg, Germany. Case records were reviewed twice a week, all microbiology reports were reviewed and ward staff was consulted. NI were defined according to the CDC-criteria and were categorised into specific infection sites. Within 20 months, 545 patients with a total of 5,117 patient days were investigated (mean length of stay: 9.4 days). 113 NI were identified in 90 patients (72 pts. with one, 13 with two and 5 with three infections, respectively). A moderate to high overall incidence (20.7/100 pts.) and a moderate incidence density (22.1/1,000 patient days) of NI in the neurosurgical ICU could be documented; these figures are well within the range of published data. Site specific incidence rates and incidence densities were: 1 bloodstream infection per 100 patients (0.9 central line-associated BSIs per 1,000 central line-days), 9 pneumonias per 100 patients (15.1 ventilator-associated pneumonias per 1,000 ventilator-days), 7.3 urinary tract infections per 100 patients (8.5 urinary catheter-associated UTIs per 1,000 urinary catheter-days). Additionally, 1.1 cases of meningitis, 0.7 brain abscesses/ventriculitis, and 1.7 other infections (surgical site infection, bronchitis, catheter related local infection, diarrhoea) were documented per 100 patients, respectively. 14.6% of isolated pathogens were E. coli, 10.2% enterococci, 9.6% S. aureus, 6.4% CNS, 6.4% Klebsiella spp., 5% Enterobacter spp. and 5% Pseudomonas spp.. In 11 cases of NI no pathogen could be isolated.

    Description / Table of Contents: Summary The maximum elimination of leucocytes and thrombocytes is a presupposition for the performance of metabolic studies on red cells. Depending on the experimental purpose an elimination rate up to 1–0,1 leucocytes resp. 〈50 thrombocytes per 104 red cells is necessary. Centrifugation-and washing-methods, mostly used nowadays for separation of erythrocytes, do not allow such a far-reaching isolation. The usage of cotton wool, however, introduced byFleming in 1926, can be done quickly and easily and solves this experimental problem.
    Notes: Zusammenfassung Für Stoffwechseluntersuchungen an Erythrocyten ist die weitestgehende Elimination der Leuko-und Thrombocyten Voraussetzung, und zwar in Abhängigkeit vom Versuchsziel bis auf 1–0,1 Leuko- bzw. 50 Thrombocyten/104 Erythrocyten. Eine derartige Isolierung ist mittels der gebräuchlichen Zentrifugations- und „Wasch“-Verfahren der roten Zellen ohne Reticulocytenverlust unmöglich. Sie gelingt hingegen unter Verwendung von Baumwolle (inauguriert vonFleming, 1926) rasch und mühelos.

    Description / Table of Contents: Summary The efficacy and tolerance of norfloxacin in the treatment of complicated urinary tract infections and in low dose long-term prophylaxis over six months for recurrent UTI were investigated in 60 and 27 patients, respectively. In the prophylaxis group, compliance with medication was controlled weekly using Mecur®BT. Five to nine days after treatment, the urine was sterile in 50 out of the 60 patients treated (83.3%); relapse occurred in six patients and was associated with development of resistance in two. Four patients suffered re-infection within the study period. Failure of treatment was closely related to persisting complicating factors in the urinary tract. The overall tolerance of norfloxacin was satisfactory. However, there was one severe allergic reaction. During the total 391 weeks of low dose long-term prophylaxis given to 27 patients, only one break-through infection occurred. Yet compliance studies proved that antimicrobial activity was present in the urines of less than 50% of the patients. During prophylaxis, the rate of infection was reduced from three to four to 0.13 to 0.05 infections/patient/year. There was no development of resistance in the isolates from these patients. Only two adverse reactions were observed.
    Notes: Zusammenfassung Es wurde die Effektivität und Verträglichkeit von Norfloxacin in der Therapie komplizierter Harnwegsinfektionen bei 60 urologischen Patienten und in der Langzeit-Rezidivprophylaxe mit Niedrigdosen Über sechs Monate bei 27 Patienten untersucht. Fünf bis neun Tage nach Therapie bestand Keimfreiheit bei 50 der 60 Patienten (83,3%); bei sechs Patienten kam es zum Rezidiv, in zwei Fällen trat dabei eine Resistenzentwicklung auf. Bei vier Patienten wurde eine Reinfektion nachgewiesen. Therapieversager waren mit der Persistenz komplizierender Faktoren korreliert. Die Verträglichkeit war mit Ausnahme einer schweren allergischen Reaktion gut. In insgesamt 391 Wochen von Langzeitprophylaxe trat bei einem von 27 Patienten eine Durchbruchinfektion auf. Die einmal wöchentlich durch Micur®-BT kontrollierte Einnahmesicherheit hatte dabei unter 50% gelegen, so daß sich durch die Effektivität der Norfloxacin-Langzeitprophylaxe die Zahl der Durchbruchinfektionen von drei bis vier pro Patient pro Jahr auf 0,13 bis 0,05 reduzierte, dies bei zwei allergischen Reaktionen und ohne daß Resistenzentwicklung auftrat.

    Description / Table of Contents: Zusammenfassung Bei 18 urologischen Patienten wurden intraoperativ Gewebekonzentrationen von Cefazolin (CEZ) bestimmt. Proben von Skelettmuskulatur und Blut wurden gleichzeitig in vorbestimmten Intervallen entnommen. Die Konzentration von CEZ im Serum und Muskelhomogenat wurde biologisch mit dem Agar-Diffusionstest bestimmt. Ein Vergleich der Standardkurven mit PBS und Überstand von antibiotikafreiem Muskelhomogenat zeigte, daß für CEZ keine wesentliche Bindung an das Muskelhomogenat nachzuweisen ist. Da die verschiedenen Gewebekompartimente in einer einzelnen Gewebeprobe noch nicht genau erfaßt werden können, wurde auf die Korrektur der gemessenen Gewebekonzentrationen verzichtet. 2 Stunden nach Beginn einer Kurzinfusion (25 Min.) von 2 g CEZ wurde in der Muskulatur ein Spitzenspiegel von 20µg/g erreicht. Eine Gewebekonzentration über 10µg/g wurde für 3 Stunden aufrechterhalten. Diese Konzentration ist hoch genug, um fast alle Stämme von E. coli, Klebsiella, Salmonella, Shigella und einen großen Teil von Proteus mirabilis therapeutisch zu erfassen, während grampositive Bakterien schon bei niedrigeren Konzentrationen erreicht werden.
    Notes: Summary Tissue concentrations of cefazolin were measured in 18 patients undergoing urological operations. Specimens of skeletal muscle and blood were taken simultaneously at predetermined intervals. The concentrations in serum and homogenized skeletal muscle were determined by means of the agar well diffusion method. A comparison of standard curves obtained with phosphate buffer solution and the supernatant of muscle homogenate revealed no substantial binding of cefazolin to the supernatant of muscle homogenate. Because of methodological difficulties in determining the different compartments of a tissue specimen concentrations in tissue were not corrected. Within two hours after a short infusion (25 min.) of 2 g of Cefazolin in muscle tissue a peak concentration of 20µg/g was reached. A tissue level above 10µg/g was maintained for three hours. This concentration is high enough to inhibit nearly all strains of E. coli, Klebsiella, Salmonella, Shigella and a large part of Proteus mirabilis, whereas nearly all gram-positive bacteria are inhibited at a lower concentration.
