    Notes: Abstract Diffusely infiltrating low-grade astrocytomas (WHO grade II) have an intrinsic tendency for progression to anaplastic astrocytoma (WHO grade III) and glioblastoma (WHO grade IV). This change is due to the sequential acquisition of genetic alterations, several of which have recently been identified. In low-grade astrocytomas, p53 mutations with or without loss of heterozygosity on chromosome 17p are the principal detectable change. Anaplastic astrocytomas contain p53 mutations at an overall incidence of 34% and, in addition, loss of heterozygosity on chromosome 19q and frequent homozygous deletion of the p16 tumor suppressor (MTS-1) gene. The most malignant astrocytic neoplasms, the glioblastoma, further shows loss of chromosome 10 and amplification of the epidermal growth factor receptor (EGF-R) gene at overall incidences of 66% and 34%, respectively. The type and distribution of p53 mutations in astrocytic brain tumours are not suggestive of specific environmental carcinogens operative in their aetiology. Analysis of 91 families with p53 germline mutations reported to date show that tumours of the nervous system account to 12% of all neoplasms. Of a total of 57 brain tumours reported, 30 were classified histologically and of these, 73% were of astrocytic origin. The observation that somatic p53 mutations in sporadic brain tumours are largely restricted to those of astrocytic origin and that astrocytomas also prevail among CNS neoplasms associated with p53 germline mutation strongly suggests, that p53 mutations are capable of initiating neoplastic transformation in astrocytes of the human nervous system.

    Notes: Summary Clinical manifestations, findings, management and outcome of a series of 177 cases with tumours of the limbic and paralimbic systems are presented. There was no operative mortality. Postoperatively 95% of them had no or only minor neurological deficits. Most of them were able to resume work. Pre-operatively 77% of the patients had epilepsy, but 84% became seizure-free after tumour removal. All 77 cases with malignant tumours died within 1–5 years. In the past many neurosurgeons were reluctant to attempt complete tumour removal in these areas. This series demonstrates the efficacy of highly skilled microneurosurgery.

    Notes: Summary One hundred-ninety-six patients with spinal cord or nerve root compression were operated on by anterior microsurgical discectomy without graft between 1978 and 1987. The patients presented either with spinal cord compression (33%) or radicular symptoms with a duration of 12 days to more than then years (66%). Investigations to confirm these clinical syndromes employed myelography, CT or MRI. Two-hundred-eighty-eight levels were explored; 124 one level, 54 two level, 17 three level and 2 four level operations were done. The follow-up (mean 6.7 years) showed 66% of the patients with excellent or good results after two years. The low risks of anterior discectomy were further diminished with skilful microsurgical technique without intercorporeal fusion. A comparison of our results at follow-up with published series, in which the anterior approach was used but with intervertebral autologous bone graft after disc removal, shows no significant differences. We therefore recommend the anterior microsurgical discectomy without fusion as the treatment of choice.

    Notes: Summary The present study determined whether the cortical potential from the human index finger changed with chronic nerve stimulation. Cerebral potentials were repeatedly recorded to stimulation of the ulnar nerve and the digital nerves of thumb, index and middle fingers, before and during a 7-day period in which the index was electrically stimulated (80 Hz) for 8–10 h daily. Cerebral potentials were recorded at three scalp sites over the contralateral “hand” area. Chronic stimulation produced no significant changes in the amplitudes or distribution of the cerebral potentials from the individual digits or the ulnar nerve. However, for the stimulated index finger there was a significant, progressive increase in latency of N20 and P25 without a detectable change in conduction velocity of distal peripheral axons. Timing in human central somatosensory pathways may be altered by the previous pattern of peripheral nerve inputs.

    Notes: Summary Iron and transferrin are required for DNA synthesis and cell division. Cellular iron uptake is mediated by transferrin receptors. In order to investigate whether iron uptake in brain tumors is associated with their histological grade, we studied 24 patients (5 astrocytoma, 11 glioblastoma, 8 meningioma) using positron emission tomography and 12 Fe-citrate. Tracer uptake from blood into brain and tumor tissue was assessed 1. using multiple time graphical analysis yielding a measure for unidirectional net tracer uptake (Ki and 2.) testing a one- and two-tissue kinetic compartment model, where K1 denotes tracer uptake from blood into tissue, k2 efflux from tissue into plasma, and 0 specific tracer binding. In the plasma, 52Fe was bound to a 80 kD protein (transferrin). Ki (in units of 10−5/min) was higher in glioblastomas (Ki mean ± SD 13.6 ± 6.1) compared with astrocytomas (4.8 ± 3.5, Mann Whitney p = 0.015) and contralateral brain (2.2 ± 0.9, Mann Whitney p = 0.009). Highest values were found in meningiomas (no blood-brain barrier (BBB); Ki 33.4 ± 16.5, Mann Whitney p = 0.008 compared with glioblastomas). Among the compartment models, fitting with Kl1 and regional plasma volume explained the data best (one-tissue model), data fits were not significantly improved by addition of a k2 or k3 parameter. K1 and Ki values were significantly correlated (Spearman Rank, p = 0.0006). We conclude that 52Fe accumulation in tumors is governed by tracer uptake at the BBB, and does not reflect number of transferrin receptors at the level of tumor cells.

Der Abfall der Clearancegrö\en in weiteren 3 Fällen wird auf Blutungen in das Operationsfeld (Blasenfreilegung) zurückgeführt, ihre Ursache ist neben der Heparinisierung möglicherweise unmittelbare Folge der Röntgenbestrahlung.

    Description / Table of Contents: Miscibility gaps are calculated for ternary systems composed of one solvent and two rigid-chain polymers or one rigid-chain and one flexible polymer applying the Flory model of rigid rods. The athermal Flory model is extended by an isotropic interaction contribution (χ-function), and it is demonstrated how repulsive and attractive interaction energies influence on the phase behaviour. Nevertheless, the properties of the ternary systems are mainly formed by the entropic effect of the rod-like molecules.
    Notes: Die Mischungslücken in ternären Systemen aus einem Lösungsmittel und zwei steifkettigen Polymeren oder je einem steifkettigen und einem flexiblen Polymer werden auf der Basis des Flory-Modells starrer Stäbchen berechnet. Das athermische Flory-Modell wird durch einen isotropen Wechselwirkungsbeitrag (χ-Funktion) erweitert, und der Einfluß abstoßender und anziehender Wechselwirkungskräfte auf das Phasenverhalten wird demonstriert. Die Eigenschaften der ternären Systeme werden im wesentlichen durch den entropischen Effekt der stäbchenförmigen Moleküle geprägt.
