    Description / Table of Contents: Summary Background: Gait characteristic and muscle power are often described in the clinical literature but with little empirical support. We assessed some parameters of gait and muscle power using simple and standardized methods within a framework based principally on classic clinical observations of depression. Method: 20 depressed inpatients were examined with standard tests of isometric muscle power and gait parameters, which measure step length, average walking speed, maximal walking speed, and three-minute walk distance. The results were compared with those of 20 matched healthy controls. The examination procedure was repeated after a three week period of inpatient treatment and significant symptomatic improvement. Results: As predicted by our hypothesis depressed patients’ muscle power and gait characteristics differed clearly from normal controls. Significant improvements after 3 weeks of inpatient treatment were found above all in extension of the knee and extension of the elbow tests as well as in all measures of our gait analysis. However, all gait parameters still differed significantly from those of healthy controls. Conclusions: Using simple but reliable tests which were used for the first time in psychiatry we examined certain relationships between depression and motor function in a moderately to severely depressed inpatient sample.
    Notes: Zusammenfassung Hintergrund: Gangcharakteristika und verminderte Muskelkraft depressiver Personen wurden in der klinischen Literatur oft beschrieben, aber bisher kaum empirisch untersucht. Wir erfaßten verschiedene Gang- und Kraftparameter unter Verwendung einfacher und standardisierter Verfahren innerhalb eines theoretischen Rahmens, welcher im wesentlichen auf klassischen klinischen Beobachtungen der Depression beruht. Methode: 20 hospitalisierte, depressive Personen wurden mittels standardisierter isometrischer Krafttests und Ganganalysen, welche Schrittlänge, durchschnittliches Gehtempo, maximales Gehtempo und 3-min-Gehstrecke erfassen, untersucht und mit 20 gesunden Versuchspersonen verglichen. Der Untersuchungsgang wurde nach 3 Wochen stationärer Behandlung und signifikanter Besserung der Symptomatik wiederholt. Resultate: Die Annahme, daß sich isometrische Muskelkraft und Gangparameter von depressiven und gesunden Personen klar unterscheiden, wurde bestätigt. Signifikante Verbesserungen am Ende der dritten Therapiewoche ließen sich v.a. bei der Knie- und Ellbogenextension und zudem in allen untersuchten Gangparametern nachweisen. Sämtliche Resultate der Ganganalyse depressiver Patienten unterschieden sich allerdings noch signifikant von denen gesunder Probanden. Schlußfolgerung: Unter Verwendung eines einfachen, jedoch reliablen Untersuchungsverfahrens untersuchten wir spezifische Beziehungen zwischen Depression und motorischen Funktionen in einer Gruppe mittel- bis schwergradig depressiver Versuchspersonen. Die erstmals für psychiatrische Fragestellungen im Rahmen der vorliegenden Pilotstudie angewandten Verfahren eignen sich zur reliablen und wenig belastenden Untersuchung von Beziehungen zwischen Depression und Motorik und können somit in breiter angelegten Studien verwendet werden.

    Notes: Summary Subaquatic density currents and turbidity currents flowing down an incline are analyzed. By noting that at least depositing turbidity currents on a constant slope depend on the source conditions, it is proposed to consider them as self-preserving or as piecewise selfpreserving in analogy to other shear flows. The requirements for self-preservation are examined and the limiting conditions for realizable flows are derived. Depth-averaged models are set up for gradually varying, steady flows. The proposed models differ from previous ones with respect to the definition of the width and other scales of the dense layer, and to the statement on the variation of the buoyancy flux. Available experimental data are shown which tend to support the concept of approximate self-preservation. An approach by Bagnold [1] is then modified to relate erosion or deposition rates to the flow dynamics, and suggestions for future research are given.

    Notes: Summary Theoretical considerations, experimental data and field evidence suggest that the motion of turbidity currents (suspension clouds) can be characterized by dimensionless products (numbers) which are primarily a function of the depositional geometric boundary conditions, especially of the dip of the depositional bottom slope. The influence of the slope on these numbers must be particularly strong for flows at (geologically important) low dips of the bottom slope. The authors propose a “thickness number” which contains easily measurable entities of the turbidite, such as the overall thickness, the settling velocity of the largest grain, and the densimetric gravitational acceleration of the deposit. The thickness number, which varies several orders of magnitude for ancient turbidites, is expected to reflect sensitively the paleoslope and also specific geometrical boundary conditions of the flow.

    Description / Table of Contents: Zusammenfassung Von 806 Patienten, die in der Zeit von 1968–1983 an der Chirurgischen Universitdtsklinik Würzburg splenektomiert wurden waren, wurden 111 hinsichtlich ihrer Langzeitbeschwerden untersucht. Verwandt hierzu wurden der Depressionsfragebogen nach Beck und der aus der psychosomatischen Medizin stammende GieBener Beschwerdebogen. Bei letzterem finden sich signifikant gehduft Störungen im Allgemeinbefinden der ,,Milzlosen“ im Vergleich zur Normalbevölkerung der BRD. Die Beschwerden sind jedoch uncharakteristisch, so daß ein eigener, spezieller Splenektomiefrage bogen entworfen wurde. Mit diesem wurde auch eine Kontrollgruppe (statistische Zwillinge) mit einem der Splenektomie vergleichbaren Operationstrauma befragt. Dominierend bei den Splenektomierten war die gehäufte Infektrate (p〈0.0000001). Signifikant erhöht waren auch die vermehrte Erschbpfbarkeit und die verminderte körperliche und geistige Leistungsfdhigkeit. Für die anderen Symptome, z. B. Alkoholunverträglichkeit oder vegetative Dystonie, die früher als typische Beschwerden angesehen wurden, ließ sich keine statistische Absicherung erbringen. Da vermehrte Erschöpfbarkeit und verminderte Leistungsfdhigkeit hoch mit der Infektanfällig-keit korreferten, der Unterschied zur Kontrollgruppe aber bei weitem nicht so grog war, scheinen these Befmdlichkeitsstörungen eher Ausdruck der gehduften Infektionen als direkte Folgen des Milzverlustes zu sein.
    Notes: Summary 111 of 806 former patients splenectomized at the Würzburg university hospital during the years 1968–1983 were interviewed for their complaints since their operation. Investigations included the use of Beck's inventory for measuring depression and the Gießen questionnaire for the evaluation of general complaints used in psychosomatic medicine. There was a significant increase of symptoms after splenectomy as compared to the normal population. A special list of 18 questions to investigate typical postsplenectomy complaints was answered by 95 of these 111 persons and by a control group of “statistical twins” with similar upper abdominal surgery without splenectomy. The leading difference was the highly significant increase of the susceptibility to infections after splenic loss followed by accelerated exhaustion and increased physical and mental weakness. The distribution of other complaints like for instance the intolerance to alcohol hitherto judged as typical sequelae of splenectomy was not statistically different in both groups. Since the symptoms listed above as significantly increased in the splenectomized patient were closely correlated with the susceptibility to infection they seem to be rather the expression of the decreased resistance than direct consequences of the loss of the spleen.
