    Notes: Summary The effects of Nimodipine on the global and regional cerebral blood flow were studied in 42 patients with cerebrovascular disorders. In 25 patients with focal deficits such as transitory ischemic attack (TIA), prolonged reversible ischemic neurological deficit (PRIND), and minor stroke due to arteriosclerosis, and in eleven patients with cerebral vasospasm after subarachnoid hemorrhage, the cerebral blood flow was measured by133Xenon inhalation technique 60 min after oral administration of 40, 60, or 80 mg Nimodipine. In 6 patients with vasospasm the effects of Nimodipine i.v. were examined. The result in twelve patients with minor stroke who were only given placebo (lactose; “test-retest”) was identical regional (rCBF) and global (CBF) cerebral blood flow before and 60 min after; placebo, blood pressure, and arterial pCO2 remained constant as well. After Nimodipine, however, the CBF increases, the increase after vasospasm being significant when taking the pCO2 in the Wilcoxon test into account. The rCBF increases much more in the regions with low perfusion rates than in well-perfused areas. This is also observed in the patients with TIA, PRIND, or minor stroke, most clearly after oral administration of 60 mg, whereas regions with normal perfusion rates show little reaction. The blood pressure was lowered, depending on the initial pressure. There was no evidence of a steal phenomenon.

    Notes: Abstract Diagnosis of Lyme arthritis (LA) in children and adolescents may be difficult due to non-specific clinical manifestations and unreliable serological tests for antibodies to Borrelia burgdorferi. In a national prospective study, 186 children with arthritis were examined in whom the attending physicians had considered the diagnosis of LA. Ultimately, LA was confirmed in 87 patients and these were compared with the remaining 99 children in whom arthritis was attributable to other causes. In comparison to patients with other causes of arthritis, patients with LA had a higher frequency of episodic arthritis and initial knee joint arthritis, reported tick bites more frequently, were older, had a lower frequency of initial arthralgias, and there were fewer large joints involved. A score was developed in a group of these patients and tested in a second group. It enabled patients with LA to be distinguished from those with other causes of arthritis: within a range from 12 to −7 points, a score of 2.5 or less excluded LA whereas 6 or more points were highly indicative of LA. If only those children with a score result between 2.5 and 6 had been tested for antibodies to B. burgdorferi, the number of tests would have been reduced by 63%. Conclusion Careful analysis of clinical presentation and use of a clinical score may help in distinguishing LA from other causes of arthritis and thus reduce unnecessary and expensive testing and uninterpretable test results.

    Notes: Summary We tested the ability of a non-invasive regional cerebral blood flow (rCBF) measurement to show vasospasm. The correlations between the flow values, the clinical condition, the CT scan and angiography were also studied. During none of the measurements were there any negative side effects on the patients. A bad clinical condition was usually accompanied by low flow data during the rCBF investigation. In accordance with local clinical signs (e.g., hemiparesis) we observed corresponding local hypoperfusion. These results were also confirmed by several follow-up studies in which good correlations between the rCBF findings, the angiography and the CT scan were found. As the rCBF investigation can be repeated within a very short time, we used this method for the testing of vasoactive drugs. Nimodipine® (Bayer) showed promising results which suggest further clinical investigations. The main problem connected with the rCBF measurement is the limited ability of the patient to cooperate. For the detection of vasospasm the low resolution both in localisation and depth of this method is of only minor importance. We partly overcame this disadvantage by developing a special respirator enabling us to investigate artificially ventilated patients. We think that the measurement of rCBF is an important aid for the timing of the operation.

    Description / Table of Contents: Abstract As a contribution to computer drawing especially of pieces of surfaces of higher order three versions of an ALGOL-program are presented. They permit to draw to net of parameter curves or a pair of picture for stereoscopic viewing or of a picture of the surface with hidden lines suppressed; the input information requested is kept at a minimum. For the hidden line problem a method is developed. The program is intended for use in pure mathematics particularly in differential geometry.
    Notes: Zusammenfassung Als Beitrag zum Computer-gesteuerten Zeichnen speziell von Flächenstücken höherer Ordnung in Parameterdarstellung werden drei Versionen eines ALGOL-Programms vorgestellt. Sie ermöglichen es — bei minimalem Aufwand an Eingabe-Information — das Parameterlinien-Netz, ein Bildpaar zur Stereo-Betrachtung oder eine Ansicht mit “hidden-line”-Unterdrückung darzustellen. Dafür wurde ein neues Verfahren entwickelt. Das Programm ist zur Anwendung in der reinen Mathematik, in erster Linie in der Differentialgeometrie, gedacht.

    Notes: Abstract. In order to study the disappearance of human chorionic gonadotropin (hCG) in serum after surgery for ectopic pregnancy and to facilitate the postoperative follow-up of hCG decline serial hCG serum concentrations were analysed in 189 women with an uneventful postoperative course and in 4 patients with persistent trophoblast. A double logarithmic transformation of hCG concentrations resulted in a linear relationship of the individual hormonal decline versus the postoperative time (r2=0.990). The decrease of hCG levels was independent from the type of surgery or gestational age. The calculated half life of hCG steadily increased with rising time interval after surgery. A nomogram of hCG decrease was constructed using the regression lines of the individual sequential hormone concentrations. The nomogram enabled the detection of active residual trophoblast within one week after surgery before serious complications occur. Our scheme requires only three hCG tests to be performed before surgery, and on day 2 and 7.

Die Ergebnisse lassen erkennen, daß die Myoglobinkonzentration im Serum einen nützlichen Parameter bei der Beurteilung von Muskelzellschäden darstellt.
    Notes: Summary Serum myoglobin was measured by a sensitive radioimmunoassay in healthy controls and patients with primary and secondary skeletal muscle disorders. The results indicate that serum myoglobin is a useful parameter in the assessment of muscle damage.

The lowest values of activity were determined in those cells located in close proximity to the vessels, which emphasizes their exceptional morphological and functional position.

    Description / Table of Contents: Zusammenfassung Von 806 Patienten, die in der Zeit von 1968–1983 an der Chirurgischen Universitdtsklinik Würzburg splenektomiert wurden waren, wurden 111 hinsichtlich ihrer Langzeitbeschwerden untersucht. Verwandt hierzu wurden der Depressionsfragebogen nach Beck und der aus der psychosomatischen Medizin stammende GieBener Beschwerdebogen. Bei letzterem finden sich signifikant gehduft Störungen im Allgemeinbefinden der ,,Milzlosen“ im Vergleich zur Normalbevölkerung der BRD. Die Beschwerden sind jedoch uncharakteristisch, so daß ein eigener, spezieller Splenektomiefrage bogen entworfen wurde. Mit diesem wurde auch eine Kontrollgruppe (statistische Zwillinge) mit einem der Splenektomie vergleichbaren Operationstrauma befragt. Dominierend bei den Splenektomierten war die gehäufte Infektrate (p〈0.0000001). Signifikant erhöht waren auch die vermehrte Erschbpfbarkeit und die verminderte körperliche und geistige Leistungsfdhigkeit. Für die anderen Symptome, z. B. Alkoholunverträglichkeit oder vegetative Dystonie, die früher als typische Beschwerden angesehen wurden, ließ sich keine statistische Absicherung erbringen. Da vermehrte Erschöpfbarkeit und verminderte Leistungsfdhigkeit hoch mit der Infektanfällig-keit korreferten, der Unterschied zur Kontrollgruppe aber bei weitem nicht so grog war, scheinen these Befmdlichkeitsstörungen eher Ausdruck der gehduften Infektionen als direkte Folgen des Milzverlustes zu sein.
    Notes: Summary 111 of 806 former patients splenectomized at the Würzburg university hospital during the years 1968–1983 were interviewed for their complaints since their operation. Investigations included the use of Beck's inventory for measuring depression and the Gießen questionnaire for the evaluation of general complaints used in psychosomatic medicine. There was a significant increase of symptoms after splenectomy as compared to the normal population. A special list of 18 questions to investigate typical postsplenectomy complaints was answered by 95 of these 111 persons and by a control group of “statistical twins” with similar upper abdominal surgery without splenectomy. The leading difference was the highly significant increase of the susceptibility to infections after splenic loss followed by accelerated exhaustion and increased physical and mental weakness. The distribution of other complaints like for instance the intolerance to alcohol hitherto judged as typical sequelae of splenectomy was not statistically different in both groups. Since the symptoms listed above as significantly increased in the splenectomized patient were closely correlated with the susceptibility to infection they seem to be rather the expression of the decreased resistance than direct consequences of the loss of the spleen.
