    Notes: Recent American and Swedish studies have shown an increase in “false” negative results when analysing dilute urine specimens for drugs of abuse. In the light of these studies, it was decided to perform a pilot study to determine the extent of possible specimen adulteration and dilution in a random batch of 50 urine specimens presented to this laboratory, using creatinine, osmolality, pH and relative density. It was found that 20% of the specimens were outside the pH range associated with the optimum working of Syva EMIT Drugs of Abuse in Urine (DAU) immunoassay screening techniques, and that if the National Institute on Drug Abuse (NIDA) recognized dilution cut-off of 1.8 mmol/L for urine creatinine concentration is applied, 84% of the specimens surveyed here would need to be repeated to ensure accurate results. Because of these findings, it is recommended that routine creatinine and pH estimations should be performed on all specimens submitted for urinary drugs of abuse screening, or at least when unexpectedly negative results are obtained.

    Notes: There are now several rapid drugs of abuse testing kits that have been designed for near-patient testing and on-site clinic screening for drug abuse. These kits are sold on the basis of their cost and rapid generation of accurate results. Five such kits have been evaluated by comparison with recognized and established methodologies. All of the kits evaluated were found to lack both sensitivity and specificity. An unacceptable proportion of false negative and false positive results were observed for most kits. This raises the question of their usefulness in the near-patient and clinic situation.

Sie wurden sodann behandelt mit Häraoperfusion unter Benutzung eines Röhrchens, das mit Holzkohle gefüllt und mit einem Acryl-Hydrogel überzogen war. Bei den meisten Drogen war die Klärung bedeutend wirksamer als nach Berichten bei Behandlung mit Hämodialyse. Minimale Klärung von gewöhnlichen biochemischen Substanzen wurde beobachtet, und obgleich die Zahl von Leukocyten und Blutplättchen verringert war, zeigten sich keine darauf zurückzuführenden nachteiligen Effekte. Sorgfältige histologische Untersuchung von Geweben, die von in dieser Weise behandelten Hunden herrührten, zeigten keinen Befund von Kohleembolie.
    Notes: Abstract Dogs were given large doses of barbiturates, glutethimide, ethanol, methaqualone, ethchlorvynol, meprobamate, chloral hydrate, paracetamol and aspirin. These were treated by haemoperfusion using a column packed with charcoal coated with an acrylic hydrogel. Clearances for most drugs were significantly higher than those reported for haemodialysis. Minimal clearances of common biochemical entities were observed and although leucocyte and platelet counts were diminished, no deleterious effects attributable to this were encountered. Careful histological examination of tissues derived from perfused dogs revealed no evidence of charcoal emboli.

    Notes: Summary Sixteen depressed elderly patients in hospital (mean age 81 years) received a single oral dose of nortriptyline prior to commencing treatment with this drug. Plasma nortriptyline measurements after the single dose were used to calculate the plasma drug clearance and to predict the daily dose required for each patient to achieve a steady-state concentration within the suggested therapeutic range of 50–150 µg·l−1. Using these dosage regimes, the mean observed steady-state concentration showed a significant correlation with the predicted values (r=0.71, p〈0.002). All patients had steady-state concentrations within or very close to this suggested range (mean 106, range 38–157 µg·l−1). Use of the prediction test can prevent the development of toxic plasma concentrations and enhance the possibility of therapeutic success. Our findings suggest that a safe starting dose of nortriptyline for the elderly is 30 mg per day.

Die Wirkung von Hämoperfusion oder Hämodialyse wurde mit der Behandlung durch herkömmliche Methoden verglichen. Die Hämodialyse entzog bedeutende Mengen von Phenobarbiton, Amylobarbiton und Pentobarbiton, aber die Hämoperfusion erwies sich sowohl für diese Substanzen als auch für Quinalbarbiton quantitativ überlegen. Dies zeigte sich auch im klinischen Zustand aller drei Gruppen. Wenige oder keine Störungen in der Blutchemie wurden beobachtet, obwohl bei der Hämoperfusion die Anzahl von Thrombocyten und Leukocyten absank.
    Notes: Abstract Dogs were given large doses of barbiturates. The effect of either haemoperfusion or haemodialysis was assessed and compared to treatment with supportive measures alone. Haemodialysis removed significant amounts of phenobarbitone, amylobarbitone and pentobarbitone, but haemoperfusion proved to be quantitatively superior both for these drugs and also for quinalbarbitone. This was also reflected by the clinical status of all three groups. Little or no disturbances in blood chemistry were observed, although platelet and leucocyte counts were diminished during haemoperfusion.

    Notes: Summary There is a lack of suitable commercially available reference materials for controlling the accuracy of determinations of trace elements in body fluids. This is a particular problem with blood lead measurements where results on individual patients or findings of population surveys may have major clinical, environmental or economic implications. Bovine blood haemolysate surrogate ‘reference’ materials for internal accuracy control of blood lead measurements have been used systematically over the past decade by the SAS Trace Elements Sub-group in Britain. They have also been used by participating laboratories in the European Community Screening Programme for blood lead in an effort to harmonise the data. The seven SAS laboratories have collaborated in assigning values for blood lead concentration to three specially prepared batches of bovine ‘reference materials’ between 1984 and 1987. The results show that it is possible for a small group of specialist laboratories to accurately and reproducibly assign values for reference materials in this way, so as to be suitable for use in internal accuracy control. Moreover, it has been possible to demonstrate the stability of the materials over a two-year period. This procedure offers a useful model for improving the accuracy of other assays of clinical interest.
