    Notes: The application of the elastic recoil detection technique utilizing heavy ions for the analysis of semiconductor samples is demonstrated. With this technique the depth profiles of the primary constituents as well as profiles of all impurities can be measured in one spectrum. Depending on the target material, a depth resolution down to 20 nm can be achieved. All elements except hydrogen can be detected with almost the same sensitivity, namely ∼1×1015 at/cm2 with 136 MeV I in a 30° recoil geometry. For hydrogen, the sensitivity is about four times better.

    Notes: We present the first results using a new technique that combines femtosecond pump–probe methods with energy- and angle-resolved photoelectron–photoion coincidence imaging. The dominant dissociative multiphoton ionization (DMI) pathway for NO2 at 375.3 nm is identified as three-photon excitation to a repulsive potential surface correlating to NO(C 2Π)+O(3P) followed by one-photon ionization to NO+(X 1Σ+). Dissociation along this surface is followed on a femtosecond timescale. © 1999 American Institute of Physics.

    Notes: Summary. At Queen Charlotte's Maternity Hospital from 1980 to 1987, 195 women had a trial of scar in their second ongoing pregnancy, having been delivered previously by elective caesarean section. Overall 154 (79%) achieved a vaginal delivery. Patients who went into spontaneous labour had a significantly better chance of being delivered vaginally than those who were induced. A past history of early pregnancy loss had no influence on the outcome, nor was outcome compromised in the group of patients whose elective caesarean section had taken place before 32 weeks gestation.

Although progestogens protect the endometrium against excessive oestrogen-induced stimulation, they can cause adverse symptomatic and psychological effects and may have undesirable metabolic consequences. Thus, the minimum progestogen dose which results in consistent endometrial transformation should be prescribed. To define this dose for norethisterone and dl-norgestrel, 197 endometrial samples obtained from postmenopausal women receiving conjugated equine oestrogens (0·625 mg or 1·25 mg daily) with one of six doses of norethisterone (or the acetate), or one of three doses of dl-norgestrel added for the first 12 days of each calendar month were examined with the light microscope; 109 samples were also assessed by transmission electron microscopy. There was an inverse relation between the percentage of samples showing proliferative features and the progestogen dose. However, proliferative endometrium was observed in 6% of samples with the highest dose of dl-norgestrel (500 μg) and in 3% of samples with 2·5 mg norethisterone. Conversely, complete secretory transformation was observed in 25% of samples with the lowest dose of norethisterone (0·1 mg) and in 40% of samples with 75 μg dl-norgestrel. Mild atypical hyperplasia was diagnosed in four samples. There was a wide inter-patient variation in response and none of the nine progestogen dose regimens induced secretory change in every patient.

    Notes: Summary. A radiolabelled monoclonal antibody (NDOG,) directed against placental alkaline phosphatase (PLAP) was used in the radio-immunodetection of ovarian carcinoma. Tumour deposits were successfully visualized in 11 of 15 patients and the abnormalities demonstrated were classified as focal or diffuse. Of the 11 patients, eight showed focal abnormalities alone and three had a diffuse abnormality, of which two also showed a focal abnormality. False-positive results may occur not only due to uptake of 123I by gut mucosa and an inadequately blocked thyroid gland but also from activity in an incompletely emptied bladder. A false-negative result occurred due to high background activity in the liver masking a known, discrete tumour deposit.

    Notes: A case of placenta membranacea is presented in which bleeding between 7 and 20 weeks of gestation was persistent and marked, necessitating blood transfusion to a total of 10 litres. Spontaneous abortion occurred at 20 weeks. Examination of the placenta and membranes showed the presence of a distinct placental disc 2 cm. thick; elsewhere the sac was completely covered by villi 0.2 to 0.3 cm. thick. Comparisons are made with other case reports and the use of ultrasound in the diagnosis of this condition is discussed.

    Notes: Summary. The cervical transformation zone (TZ) was not fully visible in 25 women referred to the colposcopy clinic with cytological suspicion of cervical intraepithelial neoplasia (CIN). Each was given a 5-day course of oral ethinyl oestradiol during the follicular phase of the menstrual cycle, and colposcopy was then repeated. The transformation zone was visible at this second examination in 16 of the 25 patients. Of these patients, 11 were treated with a local destructive technique, nine by laser, two by cryocautery; one patient had a cone biopsy, and four patients did not require treatment. Although cone biopsy was necessary in 8 of the remaining 9 patients it is encouraging that the use of a simple medical regimen allowed us to avoid conisation and its morbidity in almost two thirds of these patients.

    Notes: Five patients with fetal complications associated with the use of an intrauterine pressure catheter in labour are described. In four, a fetal vessel was punctured either by the catheter or its introducing sheath. In the remaining patient, cord compression resulted from entanglement with the catheter. These problems may by minimized by a careful catheter introduction technique.

    Notes: A synthetic and a natural oestrogen were administered alternately for three months to nine women with primary amenorrhoea using a randomized cross-over schedule. Measurements of haemostatic function were performed before and at the end of each treatment period. No significant change in haemostatic function was observed after treatment with the‘natural’ oestrogen, oestriol succinate. In contrast, treatment with a synthetic oestrogen, ethinyloestradiol, caused shortening of the prothrombin time and an increase in plasma concentration of factor VII and plasminogen. These data support other observations in suggesting that natural oestrogens may have fewer potentially adverse effects on haemostatic function than synthetic oestrogen.We thank the Special Trustees of Leeds General Infirmary for financial support towards the technical costs of this project.
