Description / Table of Contents: Summary A case of late onset metachromatic leukodystrophy with a clinical picture of paranoid hallucinatory psychosis and severe dyskinesia is described. The problem of diagnostic recognition is discussed. In the case, diagnostic procedures were initiated after atypical clinical course, and established on the basis of MRI and specific biochemical tests.

Notes: Summary Ten patients with tumors of the pineal region underwent CT and MRI investigations. There were 3 germinomas, 3 teratomas and 1 of each of the following: pineocytoma, PNET, ependymoma and meningioma. Not only were tumor size and growth compared to CT, but an attempt was made to obtain knowledge of the histology of the tumor by special T2 calculations. The investigations did not lead to an improvement in type specific diagnosis.

Description / Table of Contents: A 37-year-old male heroin addict who also performed inhalation of the white smoke obtained by heating heroin on tinfoil (chinesing or chinese blowing), presented with disturbance of consciousness, behavioural changes and headache. Tetraparesis developed rapidly with flexory hypertonia of the upper and extensor hypertonia of the lower limbs and within 17 days the patient died of respiratory failure. The neuropathological examination of the brain showed a severe cerebral edema, signs of hypoxic encephalopathy and a spongiform leucoencephalopathy with axonal degeneration only in the most severely affected areas. The pathogenesis of this neurological complication in heroin addicts performing chinese blowing is unknown and toxic elements could not be detected yet.
