    Notizen: Summary Ten patients with tumors of the pineal region underwent CT and MRI investigations. There were 3 germinomas, 3 teratomas and 1 of each of the following: pineocytoma, PNET, ependymoma and meningioma. Not only were tumor size and growth compared to CT, but an attempt was made to obtain knowledge of the histology of the tumor by special T2 calculations. The investigations did not lead to an improvement in type specific diagnosis.

    Notizen: Abstract We performed MRI, including diffusion-weighted imaging, in 15 patients with recurrent strokes with acute ischaemia and at least one old lesion according to the clinical history and/or CT. Routine MRI showed similar signal intensity changes in both situations. Diffusion-weighted images, however, were positive in all acute or subacute infarcts. The high signal of acutely disturbed diffusion due to intracellular oedema could also be identified in small brain stem lesions. Spatial resolution was increased by applying separate gradients in each axis instead of creating anisotropy-independent trace images.

    Notizen: Summary The use of a 20 MHz pulsed Doppler technique during stereotactic brain tumour biopsies is described. By means of a miniaturized ultrasonic probe the tissue planned for biopsy was investigated for vessel signals. Out of 41 patients, with this technique a flow-pulse curve in front of the biopsy cannula was registered in 14 cases. In 9 cases one pulse curve was determined and in 5 cases two or more flow curves were found, indicating the presence of arterial vessels. The depth of the vesses signal could be localized and this area avoided for biopsy sampling. In a series of 41 Stereotactic biopsies carried out in this way there was no intra-operative haemorrhage or postoperative haematoma.

    Notizen: Summary  In 72 patients with acute subarachnoid haemorrhage (SAH) the relationship between the amount of subarachnoid blood clots detected by initial cranial computed tomography (CCT) up to 48 hours after bleeding and the later development of vasospasm, established by blood flow velocity measurement with transcranial Doppler ultrasound (TCD) was investigated. The serial Doppler examinations started within the first 72 hours after SAH and were carried out every second day up to three weeks. Each Doppler recording was accompanied by a neurological examination. Patients classified as Hunt and Hess grade V were excluded from the study. All patients with remarkable brain oedema in CCT or with intracranial pressure above 25 mmHg were also excluded. Because of the well known age-dependence of vasospasm after SAH, two age groups were formed.  A statistically significant correlation (p〉0.05) between blood flow velocities and blood load after SAH was not found. The mean age of the investigated 72 individuals was 48.9 years (14 up to 76 years). 47 patients were younger than 56 years. Linear regression analysis indicated a correlation with a quite low significance level (r=0.350, p〈0.025) between TCD blood flow velocities and blood load in CCT in these younger subjects. No significant correlation (p〉0.05) between these two variables could be established in the 25 patients older than 55 years. In a second step an intra-individual comparison of side-to-side differences in TCD and CCT was made. There were no significant differences in blood flow velocities between subjects with or without side-to-side differences in cisternal blood load.  It is concluded that the amount of blood visible on initial CCT after SAH is not a powerful predictor of cerebral blood flow velocities measured by TCD.

    Notizen: Abstract We present a 2-year-old boy and a 6-year-old girl with mild Canavan disease (CD). Aspartoacylase activity in skin fibroblasts was deficient. Magnetic resonance imaging (MRI) of the brain did not show the prominent leucodystrophy previously reported in CD, but there was a hyperintense signal from the lentiform nuclei and the heads of the caudate nuclei on the T2-weighted MR images. This suggests a specific vulnerability of the corpus striatum in these patients. In the older patient, the white matter became affected at the age of 6 years. Proton magnetic resonance spectroscopy (1H-MRS) of white matter revealed a normal concentration of N-acetyl-l-aspartate (NAA) and a markedly decreased concentration of choline containing compounds (Cho) in the boy but a normal ratio of NAA to Cho in the girl. We conclude that deficient NAA catabolism affects myelin metabolism. This may present as changes in the striatum and/or as a low concentration of Cho before leucodystrophy appears on MRI.

    Notizen: Abstract  The accuracy of coordinate transformation from the computed tomographic (CT) space to the stereotactic frame space was analysed for frame-based stereotactic systems which use a localisation frame and coordinate transformation based on matrix calculation. The coordinate transformation was divided into three consecutive steps: (1) transforming the localisation frame into the CT image built up from pixels with distinct attenuation values, (2) determining the rod centres of the localisation frame in the CT image, and (3) coordinate transformation from the image to the frame space using the centres of the rods in the image space and algebraic, matrix-based calculation. The error contribution at each step was evaluated separately and its effect on the subsequent mathematical operations was analysed. The first step dealt with the influences of the mathematical and physical properties of the CT on the image of the localisation frame. Noise, slice thickness, convolution filter, dimension of the pixel matrix, and image processing had an influence on the attenuation values in each pixel. Above all, the slice thickness had an effect on the shape of the oblique rods in the CT image. At the second step, the main error contribution was due to the method by which the centre of the rods was calculated. The most accurate method was to determine the centre of gravity using the attenuation values as single mass points (with accuracy in the range of ±1/10 pixel, or ±0.125 mm), followed by rounding off the centre of gravity and the highest pixel value in the square matrix R2(N) within 1 pixel. Pointing with a cursor under visual control was accurate to 1 pixel and the pixel with the highest attenuation value showed deviations of up to 2 pixels in the x and y axes. Thus, the methods differed by a factor of 20. The influence of the CT mathematics and physics on the determination of the centre of the fiducials was negligible in comparison to the method of calculation used. There was no systemic error due to the filtred back projection algorithm. Data input errors due to noise were in the range of 1/10 pixel. The effects of the remaining physical influences were all in the range of the error due to noise. In particular these results speak in favour of no influence of slice thickness on coordinate transformation.

    Notizen: Abstract Three cases of symptomatic neuralgia of the trigeminal nerve due to an amyloidoma in the gasserian ganglion are described. The correct diagnosis was not made prior to histological examination of the surgical biopsy specimens. Medical history and clinical observation led to the diagnosis of a malignant process of the nasal cavities in the first patient; of an inflammatory dental focus in the second patient; and of multiple sclerosis in the third patient. CT findings were normal in cases 1 and 2; in case 3, a schwannoma was suspected from the CT appearances. In case 1, MRI had not been performed; in cases 2 and 3, MRI revealed a tumour mass which was also considered to be a schwannoma. Histologically, the tumours consisted of masses of amyloid deposits which had largely replaced the pre-existing ganglionic cells and satellite cells. Electron microscopy confirmed the fibrillar structure of the deposits. Immunohistochemistry and immunocytochemistry revealed the amyloid to belong to the AL-lambda subtype.

    Notizen: Summary The severity of Friedreich's ataxia was graded in ten patients by clinical examination and in five by use of posturography. These data were compared with neuroradiology findings. CT-confirmed infratentorial atrophy occured only in advanced cases of Friedreich's ataxia; the correlation with the clinical score was poor. On mid-sagittal MRI planes the diameters of fourth ventricle, brain stem at the level of the inferior olive and spinal cord at the levels of the foramen magnum and C3 were measured. Patients with Friedreich's ataxia had significant MRI-confirmed atrophy of the cranial spinal cord as compared with a normal, age-matched control group. This was also observed in patients with Friedreich's ataxia in the early stages. A reliable correlation between atrophy of the cranial spinal cord and the clinical score, however, could again not be found. MRI exploration of the cranial spinal cord may be recommended as an additional diagnostic marker in Friedreich's ataxia.
