    Description / Table of Contents: Zusammenfassung Es wird über ein ungewöhnliches, durch einen Cysticercus racemosus hervorgerufenes Krankheitsbild berichtet. Die klinischen Erscheinungen entsprachen zunächst ganz dem eines cerebralen Gefäßprozesses mit plötzlich auftretender Aphasie und Hemiparese rechts. Auch arteriographisch wurde ein Mediateilverschluß nachgewiesen. Nach vorübergehender Besserung kam es noch mehrfach zu weiteren Schüben, die jeweils wieder einem linkshirnigen Gefäßprozeß entsprachen. Erst mehr als 3 Jahre nach Beginn der Krankheitserscheinungen boten sich Hinweise auf einen raumfordernden Prozeß, der sich dann auch arteriographisch und ventrikulographisch bestätigen ließ. Die Diagnose der Zystizerkose konnte erst während der Operation gestellt werden. Nach anfänglicher postoperativer Besserung starb der Patient plötzlich, ohne daß die Todesursache hätte hinreichend geklärt werden können. Die Obduktion ergab eine schwere Zystizerkenarteriitis mit Teilverschluß der Arteria cerebri media entsprechend dem arteriographischen Befund. Die Differentialdiagnose des eigenartigen Krankheitsbildes wird ausführlich dargelegt. Ebenso werden die sich aus pathologisch-anatomischer Sicht ergebenden Probleme besprochen.
    Notes: Summary The history of an unusual case of cysticercus racemosus is reported. The clinical picture first suggested a cerebro-vascular accident with sudden aphasia and right hemiplegia. The arteriogram revealed a partial obstruction of the arteria cerebri media. A temporary improvement was followed by several relapses consistently compatible with a vascular process in the left hemisphere. At last, 3 years after onset of the initial symptoms, a space occupying lesion was suspected and verified by arterio- and ventriculography. The final diagnosis of cysticercosis was established during surgery. The patient, first making a good postoperative recovery, suddenly died. The exact cause of death could not be determined. Autopsy revealed a very severe arteritis due to cysticercosis associated with a partial obstruction of the arteria cerebri media which had been previously visualized on the angiogram. The differential diagnosis of this unusual case and its pathological-anatomical aspects are discussed in detail.

    Notes: Summary Frequency of Far Lateral Lumbar Disc Herniation The analysis of pre-operative computer-assisted tomograms and myelograms in a series of 694 operated lumbar disc herniations showed that a far lateral disc prolapse occured in 7% of the cases. Within the group of those far laterally herniated discs 3% of the herniations were predominantly located in the intervertebral foramen, whereas 4% of the protruded discs were mainly situated extraforaminally compressing the spinal nerve in its paravertebral course. Surgical Management of Extraforaminal Far Lateral Lumbar Disc Herniation By March 1988 40 patients had been operated on for an extraforaminal disc protrusion making use of an external microsurgical exposure (in two cases by a transmuscular approach and in 38 cases via an enlarged midline approach). A medium-term follow-up of these 40 patients revealed a substantial clinical relief of pain in 34 cases (85%). Based on these gratifying results we regard the external exposure of the extraforaminally protruded disc as the treatment of choice.

    Notes: Summary Fourty-six cats were made hydrocephalic and hydromyelic by means of an intracisternal kaolin injection. In 17 other cats hydrocephalus and syringohydromyelia were achieved by operative occlusion of the foramina Luschkae of the fourth ventricle. In both the kaolin treated animals and the animals whose outlets of the fourth ventricles were operatively obstructed a progressive dilatation of the ventricles and central canal occurred, which could be demonstrated and followed in 30 animals by ventriculography, myelography and/or contrast filling of the hydromyelic central canal. Coinciding with the dilatation of the central canal the clinical picture of a raised intracranial pressure due to obstructive hydrocephalus improved. The presented results suggest that the dilated central canal acts as a kind of natural by-pass between the ventricles and the spinal subarachnoid space. In order to determine the role of spinal kaolin arachnoiditis on spinal cyst formation and central canal dilatation in 13 animals, kaolin was locally applied in the lower thoracic region. The local spinal kaolin arachnoiditis had no influence on central canal dilatation or cyst formation.

    Notes: Summary Our intention was to find out, if there is a difference between the cardiovascular responses to fast and slow increases in intracranial pressure. The investigations were carried out on dwarf pigs and included recordings of intracranial pressure, systemic blood pressure, pulmonary venous pressure, right and left atrial pressures, cardiac output, and heart rate. The results seem to indicate that a rapid rise in intracranial pressure provokes mainly peripheral vasoconstriction, whereas slow intracranial compression leads to marked augmentation of cardiac efficiency and strokework, the peripheral resistance being changed little or not at all.
