    Notes: Summary Tangential missile injuries are recognized as a cause of cerebral damage by indriven bone fragments, parenchymal lacerations and intracerebral haematomas. Severe blood loss from this type of injury was not reported. Four patients are described in whom high kinetic energy missiles caused severe tangential craniocerebral injuries. Their condition was aggravated by life threatening haemorrhage. The bleeding vessels were cerebral arteries or major veins entrapped in the fracture line and bleeding extracranially. This blood loss cannot be controlled by dressing and salvage could be achieved only by immediate resuscitation, vigorous fluid replacement and very rapid evacuation to a neurosurgical center.

    Notes: Summary A patient with fracture of the optic canal who demonstrated spontaneous recovery of useful vision following acute visual loss is reported. Acute stretching of the involved optic nerve was demonstrated by CT. The return of the optic nerve (ON) to its normal position coincided with the recovery of vision. The visual evoced response (VER) was the first parameter to herald the good visual outcome.

    Notes: Summary Since the advent of computed tomography (CT) the recognition of the occurrence of multiple intracranial meningiomas (MIM) in the same individual has been on the increase. In our material the incidence of MIM at first assessment of CT films was 20%, with distant multiplicity prevailing over the regional one. This incidence will probably change in the course of time as MIM develop not only concurrently but also consecutively. On the other hand our surgical macroscopic incidence of regional multiplicity alone was 49%. The discrepancy between the CT and surgical findings prompted us to reevaluate the CT studies of 100 consecutive patients. This reevaluation demonstrated: 1. in two cases, small meningiomas were overlooked at first assessment; 2. nineteen cases of solitary globoid meningiomas seemed to be the consequence of the coalescence of adjacent smaller masses. Thus, the CT incidence of MIM increased to 40%, with regional multiplicity prevailing over the distant one. The authors think that the aforesaid findings question the very existence of solitary meningiomas as a pathological entity. They would be the end product of a coalescence of multiple adjacent smaller growths. Accordingly, a more aggressive surgical approach is suggested to include the resection of a generous fringe of dura mater around the main tumour. As this is not always possible, or too risky, a comprehensive complement to surgery like radiotherapy could be given a reasonable randomized trial.

    Notes: Summary This study attempts to find out whether the motor evoked potential (MEP) elicited by single pulse and slow-rate (1 Hz) repetitive transcranial magnetic stimulation (TMS) can disclose concealed subclinical impairments in the cerebral motor system of patients with minor head injury. The motor response to single pulse TMS (STMS) of the patient groups was characterized by significantly higher threshold compared with that of the control group. The central motor conduction time, as well as the peripheral conduction time were normal in all patients pointing to cortical impairment. Two main patterns of MEP changes in response to repetitive TMS (RTMS) were observed in the patient group. A. – progressive decrease of the MEP amplitude throughout the stimulation session to a near complete abolition. B. – irregularity of the amplitude and the waveform of the MEP in a chaotic form. The MEP latency remained stable during the whole stimulation session. The MEP abnormalities recovered gradually over the period of a few months. The higher threshold of the motor response to STMS and the abnormal patterns of the MEP to RTMS seem to reflect transient impairment of cortical excitability or “cortical fatigue” in patients who sustained minor head injures. Further study is needed to evaluated the extent and the pathophysiological mechanisms of the central nervous system fatigue phenomenon following head injury.

    Notes: Summary The TSER of six patients with facial anaesthesia dolorosa showed shorter latencies and higher amplitudes of the late waves, which are believed to represent the processing of somatosensory input in the higher subcortical and cortical centres. Shorter latencies and higher amplitudes may reflect abnormal facilitation or decreased inhibition by these centres. The TSER also supplied an objective means of pain measurement, as the stimulating impulse at the affected side had to be reduced from 20 mA (the usual intensity used in patients without evoking pain or an unpleasant sensation) down to 9–12 mA, to avoid unbearable pain.

    Notes: Abstract We have examined the electroretinographic responses, the psychophysically determined course of dark adaptation and/or the scotopic and photopic (static) perimetric profile of the uninjured eyes of 11 patients with unilateral intraocular foreign bodies. Most of the patients showed subnormal ERG amplitudes over a range of light intensities, and subnormal light sensitivity in isolated retinal areas. The data suggest that eyes not directly injured by a unilateral traumatic ocular episode may show visual defects.
