    Description / Table of Contents: Zusammenfassung In der Behandlung des therapieresistenten Ascites haben sich peritoneovenöse Shunts bewährt. Als häufigste Komplikation wird ein Verstopfen des Katheters beschrieben. Der modifizierte Shunt Denver Storz ist mit einer flexiblen Pumpkammer mit Ventil ausgestattet. Bei positivem Druck öffnet sich ein Ventil auf der Zuflußseite. Der Standard Shunt hat eine Strömungsrate von 42 ml/min, ein Reflux wird durch Schluß des Ventils bei entgegengesetztem Druckgefälle verhindert. Die Probleme des Verstopfens werden vermieden. Die typische Operation wird beschrieben.
    Notes: Summary A peritoneovenous shunt (PV) is useful in the treatment of therapy-resistant ascites. PV shunting is often complicated by clogging problems. The Storz Denver PV shunt consists of a flexible pump chamber and valve. The valve opens at a positive pressure. The standard shunt provides a flow rate of 42 ml of water per minute. When the pressure gradient is reversed, no detectable reflux occurs. Clogging problems are avoided. The typical operation is described.

    Description / Table of Contents: Zusammenfassung Die Diskontinuitätsresektion ist an der Tübinger Chirurgische Universitäts-Klinik zwischen 1970–1985 notfallmäßig bei den verschiedensten Indikationen durchgeführt worden. Neben den entzündlichen Dünn- und Dickdarmerkrankungen handelt es sich um Patienten mit Ileus, perforierter Appendicitis, perforiertem Colon-Ca und Mesenterialinfarkt. Die Diskontinuitätsresektion beseitigt den krankmachenden Herd, vermeidet die primäre Anastomose mit den Gefahren der Insuffizienz bei Peritonitis, ist ohne größere Mobilisation möglich und läßt die Chance eines Wiederanschlusses.
    Notes: Summary Emergency discontinuity resection was performed in the surgical department of the University of Tübingen between 1970 and 1985 for various indications: inflammatory bowel diseases, intestinal obstruction due to various causes, perforated appendicitis, perforated colonic carcinoma, and mesenteric with all the associated dangers of leakage, requires little mobilization of the colon and allows delayed reanastomosis with further resection to be performed if necessary.

    Description / Table of Contents: Zusammenfassung Die operative Behandlung der menschlichen Echinokokkose stellt nach wie vor die Therapie der Wahl dar. Dennoch konnten durch die Chemotherapie mit Vermox erhebliche Fortschritte erzielt werden. Die Chemotherapie ist für Patienten reserviert, bei denen technische Inoperabilität vorliegt, die nicht radikal operiert werden können bzw. die Radikalität des Eingriffes fraglich erscheint, vor allem beim Echinococcus multilocularis der Leber. Ergebnisse und Erfahrungen der medikamentösen Behandlung bei der Behandlung des Echinococcus multilocularis und granulosus werden mitgeteilt.
    Notes: Summary Operative treatment of human echinococcosis is still the therapy of choice. Nevertheless, important progress has been made using chemotherapy with Vermox. Chemotherapy is reserved for patients with local inoperability, patients who cannot undergo a radical operation, especially in cases of Echinococcus multilocularis of the liver. Results and experience of drug therapy of Echinococcus multilocularis are presented.

    Description / Table of Contents: Zusammenfassung Ziel der operativen Behandlung des Morbus Crohn ist die Beseitigung von Komplikationen um dadurch einen günstigeren Verlauf der Erkrankung einzuleiten. Eine chirurgische Heilung ist nicht möglich. Angestrebt wird eine Resektion im makroskopisch gesunden Gewebe. Die Größe des Sicherheitsabstandes ist umstritten. Umgehungsanastomosen werden nicht mehr angelegt (Ausnahme DuodenalCrohn). Perioperative Antibioticaprophylaxe und gute präoperative Vorbereitung der Patienten ist obligat.
    Notes: Summary The aim of the operative treatment of Morbus Crohn is to remove complications and to introduce a better course of the disease. A complete recovery by operation is impossible. The aspiration is to resect into the macroscopically non-inflamed tissue. The extent of the safety margin to the intact tissue is controversial. Bypass anastomoses should not be performed any more (except in cases of duodenal Morbus Crohn). Perioperative antibiotic prophylaxis and a good preoperative preparation of the patients is mandatory.

    Description / Table of Contents: Zusammenfassung Von 1974–84 wurden an der Chirurgischen Universitäts-Klinik Tübingen 163 Patienten wegen eines Traumas splenektomiert. Bei der Nachuntersuchung fanden wir keine erhöhte Infektanfälligkeit und kein OPSI-Syndrom. Nahezu 40% unserer Patienten hatten noch funktionsfähiges Milzrestgewebe im Szintigram auf Grund spontaner Splenosis peritonei, Autotransplantation oder wegen einer Nebenmilz. Die Bestimmung der Serumimmunglobuline IgM, A, G, des Antikörpertiterverlaufs nach Tetanusimpfung, der Lymphocytenstimulation durch ConA, PHA, PWM und der Granulocytenphagocytose zeigte keinen Unterschied zwischen splenektomierten Patienten mit oder ohne Milzrestgewebe und einer Kontrollgruppe.
    Notes: Summary From 1974 to 1984, 163 patients underwent splenectomy for traumatic rupture of the spleen. On follow-up, no cases of OPSI syndrome and no increase in the rate of infections occurred. Nearly 40% of the patients displayed partial splenic function on99mTc sulfur scintigraphy because of peritoneal splenosis, accessory spleens or autotransplantation. Measurement of the levels of immunoglobulins M, A and G, the immune response to tetanus vaccine, the stimulation of lymphocytes by ConA, PHA and PWM and the phagocytic capacity of granulocytes showed no difference between patients with no splenic function, those with partial splenic function or a group of normal individuals used as a control.

    Notes: Abstract Investigations on the absorption of shortchain fatty acids across ruminal epithelium of sheep were performed both in vitro (Ussing chamber technique, using propionic acid representatively for shortchain fatty acids) and in vivo (washed, isolated reticulorumen). A pH-induced, nearly tenfold increase in the concentration of undissociated propionate led to an only twofold increase in mucosal-to-serosal flux of propionate (in vitro). Neither amiloride (1 mmol·l-1, in vitro) nor theophylline (10 mmol·l-1, in vivo), inhibitors of the ruminal Na+/H+ exchanger, exerted any significant influence on propionate fluxes or short-chain fatty acids absorption, respectively. Total replacement of luminal Na+ (by choline) did not alter short-chain fatty acids absorption (in vivo). Mucosal 4,4′-diisothiocyanatostilbene-2,2′-disulfonic acid (0.1 mmol·l-1) or mucosal nitrate (40 mmol·l-1) markedly reduced propionate net flux (in vitro). Increasing mucosal Cl- concentration brought about a significant drop in mucosal-to-serosal flux of propionate (in vitro) and in short-chain fatty acids net absorption (in vivo), respectively. The results obtained suggest that short-chain fatty acids are absorbed both as anions and as undissociated acids across ruminal epithelium of sheep. It is concluded that short-chain fatty acids anions either compete with Cl- for binding sites at a common anion-exchange mechanism or that they are absorbed by an short-chain fatty acids anion/HCO 3 - exchanger indirectly coupled to a Cl-/HCO 3 - exchanger via intracellular bicarbonate.

    Notes: Abstract This study sought to investigate effects of short-chain fatty acids and CO2 on intracellular pH (pHi) and mechanisms that mediate pHi recovery from intracellular acidification in cultured ruminal epithelial cells of sheep. pHi was studied by spectrofluorometry using the pH-sensitive fluorescent indicator 2′,7′-bis (carboxyethyl)-5(6′)-carboxyfluorescein acetoxymethyl ester (BCECF/AM). The resting pHi in N-2-hydroxyethylpiperazine-N′-2-ethanesulfonic acid (HEPES)-buffered solution was 7.37 ± 0.03. In HEPES-buffered solution, a NH4 +/NH3-prepulse (20 mM) or addition of butyrate (20 mM) led to a rapid intracellular acidification (P 〈 0.05). Addition of 5-(N-ethyl-N-isopropyl)-amiloride (EIPA; 10 μM) or HOE-694 (200 μM) inhibited pHi recovery from an NH4 +/NH3-induced acid load by 58% and 70%, respectively. pHi recovery from acidification by butyrate was reduced by 62% and 69% in the presence of EIPA (10 μM) and HOE-694 (200 μM), respectively. Changing from HEPES- (20 mM) to CO2/HCO3 −-buffered (5%/20 mM) solution caused a rapid decrease of pHi (P 〈 0.01), followed by an effective counter-regulation. 4,4′-diisothiocyanatostilbene-2,2′-disulfonic acid (DIDS; 100 μM) blocked the pHi recovery by 88%. The results indicate that intracellular acidification by butyrate and CO2 is effectively counter-regulated by an Na+/H+ exchanger and by DIDS-sensitive, HCO3 −-dependent mechanism(s). Considering the large amount of intraruminal weak acids in vivo, both mechanisms are of major importance for maintaining the pHi homeostasis of ruminal epithelial cells.
