    Notes: Microbeam radiation therapy of the intracerebral 9L gliosarcoma in rats, an experimental surrogate for human malignant gliomas, using mainly 30–130 keV wiggler-generated x rays, extended the residual lifespans of some rats ten or more times over those of untreated, similar gliosarcoma-bearing rats. The rats were exposed 300 or 600 times to an upright, 25-μm-wide, 4-mm-high x-ray beam. A multislit collimator has been designed to shorten the time required for the therapy. © 1995 American Institute of Physics.

    Notes: We report a female patient with adult onset of cow's milk protein allergy. For 1 year, she experienced repeated gastrointestinal symptoms and had a single exercise-induced anaphylactic reaction. In addition to positive skin tests and specific IgE to milk proteins and a positive challenge test, immunohistochemistry of the small intestine showed a marked increase of IgE-positive mast cells. This finding is highly specific and could provide an additional tool for diagnosing cow's milk protein allergy and possibly also other food allergies.

    Description / Table of Contents: Zusammenfassung. Bronchuscarcinome infiltrieren oft Venen, wobei Tumorausläufer bis in den linken Vorhof entstehen können. Die intravasalen Tumorzapfen können während einer Operation abgelöst werden und zu Tumorembolien führen. Deshalb sollte, wie bereits 1951 von Aylwin empfohlen, bei der Lungenresektion früh und herznah die entsprechende Vene ligiert werden, vor allem dann, wenn die intravasale Tumorprogression präoperativ computertomo- oder echokardiographisch nachgewiesen wurde. Wir berichten über den Infarkt einer Großhirnhemisphäre infolge eines Tumorembolus in der A. cerebri media, der während einer Oberlappenresektion wegen eines Bronchuscarcinoms auftrat. In einer Literaturübersicht werden 38 Patienten mit peripherer Tumorembolie bei primärem Lungencarcinom geschildert.

    Notes: Summary. Carcinomas of the bronchi often infiltrate veins, particularly in advanced stages. Tumor propagation in pulmonary veins with intra-atrial extension has been reported. Systemic arterial tumor embolism is a potentially fatal hazard that is caused by surgery or it can be the initial manifestation of a bronchogenic carcinoma. The importance of early pulmonary venous ligation in lung cancer surgery, first stressed by Aylwin in 1951, is evident, particularly when intravenous tumor progression has been diagnosed preoperatively by echocardiography or CT scan. We report a case of lethal hemispheric brain infarction following tumor embolism during lobectomy for bronchus carcinoma. A review of the literature on this subject is presented; there were 38 cases of arterial tumor embolism because of primary lung neoplasms.

    Description / Table of Contents: Zusammenfassung Eine bisher wenig beobachtete Nebenwirkung der BLM-Perfusionstherapie von oralen Plattenepithelcarcinomen sind umschriebene Nekrosezonen an den Akren. Als Ursache ließ sich ein Gefäßschaden mit Thrombenbildung in den Arteriolen nachweisen. Die elektronenmikroskopischen Beobachtungen an 5 mit BLM perfundierten Tumorarealen und Gefäßen wiesen einen Endothelschaden mit Schwellung und vakuoliger Degeneration der Endothelzellen nach. An den Arteriolen kam es zur Ablösung von Endothelien, Schwellung der glatten Muskelfasern und Zerstörung der elastischen Lamellen. Dies bildet den Boden für eine Mikrothrombosierung. Ein negativer Einfluß des Gefäßschadens auf die Wirkung der Cytostatikatherapie wird diskutiert. Gefäßschäden im Bereich der Lungenstrombahn werden als eine Teilursache der multifaktoriell bedingten Bleomycinlunge angesehen.
    Notes: Summary During intra-arterial perfusion therapy of oral squamous cell carcinomas with bleomycin, circumscribed necrotic areas in the perfused region are observed in a few cases. These are usually located in the acral regions. As the cause, a lesion of the blood vessels with formation of microthrombosis could be demonstrated. Electron-microscopic observations of the blood vessels in five bleomycinperfused tumor areas demonstrated lesions of the endothelium characterized by swelling of the cells, formation of intracytoplasmatic vacuoles and villous projections into the lumen of the vessel. In arterioles, separation of the endothelium from the underlying tissue, swollen smooth muscle cells and destruction of elastic lamellae were found. These were the pacemakers for the formation of thrombosis. A negative influence of the vascular lesions on the cytostatic effect on the tumor is likely. Vascular lesions also constitute one of the initial factors for the development of the bleomycin-induced lung lesion (bleomycin lung).

    Notes: Summary A previously healthy woman in middle age, vacationing in Spain, is treated with a massive dose of insulin for minimal hyperglycemia following an apparent gastrointestinal disease. This results in rapid coma and, 20 days later, in death. At autopsy, the main finding consists in a remarkable and uncommon ribbon-like juxtacortical necrosis of the white matter in both hemispheres of the telencephalon. There is also a microscopic focal necrosis in the pons cerebri. The grey matter of cortex and basal nuclei, and the subcortical arcuate fibers are spared. The detailed autopsy fails to reveal other essential changes. We have not been able to find reports on an identical case. Hypoglycemic coma usually causes cerebral lesions different from those seen in the present case. A brief analysis of the differential diagnosis is made. In the absence of unequivocal signs of infection, vascular disease or degenerative marks, the findings are tentatively related to complex interactions between fluid loss, hypoglycemic coma, hypoxia and other metabolic disturbances.

    Description / Table of Contents: Summary In the rare event of cancer-to-cancer metastasis, one malignant neoplasm spreads to a second carcinoma. To date, 42 such cases have been published in the English- and German-language literature. We report two additional patients, each with metastatic bronchial cancer present in a renal carcinoma. In addition to immunohistochemical examinations, we performed in situ DNA image cytometry of histologic preparations as a novel analytical technique to differentiate metastatic tissue from the host tumor. In both cases DNA histograms of metastatic tumor resembled those of the primary malignancy but were clearly dissimilar to the host tumor. In 70 % of previously reported cases renal carcinoma was the host tumor while in 50 %, cancer of the lung was the most frequent donor tumor.
    Notes: Zusammenfassung Bei der seltenen Karzinom-in-Karzinom-Metastasierung siedelt in einem malignen Neoplasma die Metastase eines Zweitneoplasmas. 42 dieser Fälle sind bisher in der deutsch- und englischsprachigen Literatur beschrieben worden. Wir berichten über 2 Patienten jeweils mit Metastasen eines Bronchuskarzinoms in einem Nierenkarzinom. Neben immunhistochemischen Untersuchungen wurde die diagnostische Möglichkeit der In-situ-DNS-Bildzytophotometrie an histologischen Präparaten, als neue Methode der analytischen Pathologie, zur Unterscheidung der Metastase vom Wirtstumor genutzt. Bei beiden Patienten konnten wir mittels DNS-Histogrammen die Metastasen von den Wirtstumoren unterscheiden und sie auch eindeutig ihren Primärneoplasmen zuordnen. Die Literaturübersicht zeigt Nierenkarzinome als häufigste Wirtstumoren (70 %), während die Metastasen meist von Bronchuskarzinomen stammen (50 %).
