    Notes: Microbeam radiation therapy of the intracerebral 9L gliosarcoma in rats, an experimental surrogate for human malignant gliomas, using mainly 30–130 keV wiggler-generated x rays, extended the residual lifespans of some rats ten or more times over those of untreated, similar gliosarcoma-bearing rats. The rats were exposed 300 or 600 times to an upright, 25-μm-wide, 4-mm-high x-ray beam. A multislit collimator has been designed to shorten the time required for the therapy. © 1995 American Institute of Physics.

    Notes: We report a female patient with adult onset of cow's milk protein allergy. For 1 year, she experienced repeated gastrointestinal symptoms and had a single exercise-induced anaphylactic reaction. In addition to positive skin tests and specific IgE to milk proteins and a positive challenge test, immunohistochemistry of the small intestine showed a marked increase of IgE-positive mast cells. This finding is highly specific and could provide an additional tool for diagnosing cow's milk protein allergy and possibly also other food allergies.

    Description / Table of Contents: Zusammenfassung Eine bisher wenig beobachtete Nebenwirkung der BLM-Perfusionstherapie von oralen Plattenepithelcarcinomen sind umschriebene Nekrosezonen an den Akren. Als Ursache ließ sich ein Gefäßschaden mit Thrombenbildung in den Arteriolen nachweisen. Die elektronenmikroskopischen Beobachtungen an 5 mit BLM perfundierten Tumorarealen und Gefäßen wiesen einen Endothelschaden mit Schwellung und vakuoliger Degeneration der Endothelzellen nach. An den Arteriolen kam es zur Ablösung von Endothelien, Schwellung der glatten Muskelfasern und Zerstörung der elastischen Lamellen. Dies bildet den Boden für eine Mikrothrombosierung. Ein negativer Einfluß des Gefäßschadens auf die Wirkung der Cytostatikatherapie wird diskutiert. Gefäßschäden im Bereich der Lungenstrombahn werden als eine Teilursache der multifaktoriell bedingten Bleomycinlunge angesehen.
    Notes: Summary During intra-arterial perfusion therapy of oral squamous cell carcinomas with bleomycin, circumscribed necrotic areas in the perfused region are observed in a few cases. These are usually located in the acral regions. As the cause, a lesion of the blood vessels with formation of microthrombosis could be demonstrated. Electron-microscopic observations of the blood vessels in five bleomycinperfused tumor areas demonstrated lesions of the endothelium characterized by swelling of the cells, formation of intracytoplasmatic vacuoles and villous projections into the lumen of the vessel. In arterioles, separation of the endothelium from the underlying tissue, swollen smooth muscle cells and destruction of elastic lamellae were found. These were the pacemakers for the formation of thrombosis. A negative influence of the vascular lesions on the cytostatic effect on the tumor is likely. Vascular lesions also constitute one of the initial factors for the development of the bleomycin-induced lung lesion (bleomycin lung).

    Notes: Summary 21 patients (6 females and 15 males) with Dermatitis herpetiformis have been studied with special considerations of the small intestinal biopsyfindings. During this investigation the patient's mean age was 45 years (20–68) and the mean age at onset was 38 years (17–64). In 5 patients (23.8%) total villous atrophy (group I) and in 4 patients (19.0%) a severe partial or subtotal villous trophy (group II) was found on small intestinal biopsy. The sprue-like changes were patchy lesions. Histocompatibility-antigens of type HLA-A1 were found in 38.1% and of type HLA-B8 in 47.6%. After 20 years of the disease one patient died of a malignant lymphoma of the intestine (immunoblastic sarcoma). In all patients the number of interepithelial lymphocytes in the small bowel mucose was significantly increased, as it was in those patients with a normal villous pattern (group III). In a quantitative analysis of specifically labeled (peroxidase-anti-peroxidase complex) IgG-, IgA- and IgM-cells in the intestinal mucosa it was found that the number of all three plasma cell classes are increased significantly (P〈0.01).

    Description / Table of Contents: Zusammenfassung 10 FÄlle von Bleomycin-behandelten oralen Plattenepithelcarcinomen wurden hinsichtlich der Stromareaktion elektronenmikroskopisch untersucht. Hierbei wurde besonders die Genese und phagocytÄre Funktion von mehrkernigen Riesenzellen (vom Fremdkörpertyp) beobachtet. Diese phagocytieren vor allem devitalisierte, keratinisierte Tumorzellen. Die Genese aus monocytogenen Makrophagen und die Endocytose der gro\en Keratinlamellen werden detailliert beschrieben. Beide PhÄnomene sind untrennbar miteinander verknüpft (funktionelle Genese) und führen zu einer funktionellen Spezialisierung. Die Initialstadien der intracellulÄren Digestion finden nicht in membranbegrenzten Vakuolen statt, sondern in spezialisierten Cytoplasmaarealen. Diese enthalten hohe Konzentrationen an Hydrolasen und werden durch eine organellenfreie Zone von der übrigen Zelle abgeschirmt. Diese einzigartige Form der Phagocytose und Digestion („Gigantophagocytose”) ist nur in den hochspezialisierten Riesenzellen möglich und erklÄrt ihre biologische Bedeutung. Erst in den folgenden Schritten der Digestion kommt es zur Bildung von sekundÄren Lysosomen. Diese Riesenzellen besitzen eine hohe funktioneile AktivitÄt und Spezialisierung. Der Widerspruch dieser Beobachtung zu den experimentellen Befunden anderer Autoren wird diskutiert.
    Notes: Summary Ten cases of oral squamous cell carcinoma, treated by bleomycin, were studied by electron microscopy with particular regard to the stromal reaction. The genesis and phagocytic function of multinucleated giant cells of foreign body type were observed. These cells phagocytize devitalized, keratinized tumor cells in particular. Their genesis from monocytic macrophages and endocytosis of large keratinized tumor cells are described in detail. Both phenomena are connected and the mode of formation of the cells results in functional specialization. The initial stages of intracellular digestion do not seem to take place within membrane limited vacuoles but in specialized cytoplasmatic areas which are formed around the ingested material. These contain high concentrations of hydrolases, sealed off from the rest of the cell by a clear zone of organell-free cytoplasm. This unique form of phagocytosis and digestion (“gigantophagocytosis”) is only possible in these highly specialized giant cells and explains their biological significance. It is likely that secondary lysosomes are formed in subsequent stages of digestion. The differences between our results and the experimental observations of other authors are discussed.

    Notes: Summary The histology and immunohistology of twenty-seven malignant non-Hodgkin's lymphomas of the intestinal tract were studied. Nine of these cases were in the small intestine, ten in the ileocaecal region, two in the appendix and four in the large intestine. In one case, several locations in the gastrointestinal tract were involved. The so-called Kiel-Classification was applied. We have found thirteen lymphomas with low grade (lymphocytic, lymphoplasmacytic, centrocytic, centroblastic/centrocytic) and fourteen with high grade of malignancy (centroblastic, lymphoblastic, immunoblastic). For most of the lymphoplasmacytic and immunoblastic lymphomata a monoclonal pattern of intracellular immunoglobulin (IgM/kappa) was identified by the immunoperoxidase method. Tumour cells of lymphocytic, centrocytic, centroblastic/centrocytic, centroblastic and lymphoblastic lymphomas were always Ig-negative. The immunoperoxidase technique helped considerably in distinguishing between (monoclonal) malignant lymphomas and (polyclonal) lympho- or immunoproliferative processes. Six out of twenty-seven malignant lymphomas had developed from immuno-inflammatory diseases of the gut. Four of these were complications of coeliac disease. One had developed from a “malabsorptive” dermatitis herpetiformis Duhring, and one from a complication of a long-standing ulcerative colitis. In two patients with coeliac sprue and “malabsorptive” dermatitis herpetiformis Duhring respectively the ulcerating small intestinal lymphomas were initially misinterpreted as “benigne ulcerative non-granulomatous jejunitis”. The evidence from the literature summarized suggests strongly that the benign non-granulomatous jejunoileitis, lymphomatous ulcer, intestinal “pseudolymphoma” and malignant lymphoma, when associated with villous atrophy of adjacent mucosa and malabsorption symptoms, are all one condition, namely, malignant lymphoma.
