    Notes: Abstract The Italian CTS Study Group performed a wide multicentric and multiparametric study to quantify the increase of electrodiagnostic sensitivity using an extended neurophysiological protocol and particularly segmental and comparative tests. The study also evaluated the clinical features of carpal tunnel syndrome (CTS) diagnosed by these tests, using validated physician- and patient-oriented measurements. The study group was composed of 740 patients for a total of 1123 idiopathic CTS hands. Overall, the sensitivity of standard tests (median digit-wrist sensory conduction velocity and wrist-thenar distal motor latency) was 83.5%. Comparative/segmental tests disclosed abnormal findings in a further 11.4% of cases, providing CTS electrodiagnosis in about 7 of 10 “standard negative” cases. The overall sensitivity of the electrodiagnostic protocol was 94.9%. The CTS population diagnosed by segmental/comparative tests had a clinical picture characterized by a high percentage of pain, and severe discomfort but no limitation in functional daily activity. Our data confirm the usefulness of a complete neurophysiological assessment by using segmental/comparative tests when standard tests yield normal results.

    Notes: Abstract Volcanic catastrophes constitute a majorproblem in many developing and developed countries. Inrecent years population growth and the expansion ofsettlements and basic supply lines (e.g., water, gas,etc.) have greatly increased the impact of volcanicdisasters. Correct land-use planning is fundamental inminimising both loss of life and damage to property.In this contribution Geographical Information Systems(GIS), linked with remote sensing technology andtelecommunications/warning systems, have emerged asone of the most promising tools to support thedecision-making process. Some GIS are presented fortwo volcanic areas in Italy, Mt. Etna and Vesuvius.GIS role in risk management is then discussed, keepingin mind the different volcanic scenarios of effusiveand explosive phenomena. Mt. Etna system covers alarge area (more than 1,000 km2) potentiallyaffected by effusive phenomena (lava flows) whichcause damage to both houses and properties in general.No risk to life is expected. The time-scales of lavaflows allow, at least in principle, modification ofthe lava path by the building of artificial barriers.Vesuvius shows typically an explosive behaviour. Inthe case of a medium size explosive eruption, 600,000people would potentially have to be evacuated from anarea of about 200 km2 around the Volcano, sincethey are exposed to ruinous, very fast phenomena likepyroclastic surges and flows, lahars, ash fallout,etc. Ash fallout and floods/lahars are also expectedin distal areas, between Vesuvius and Avellino,downwind of the volcano. GIS include digital elevationmodels, satellite images, volcanic hazard maps andvector data on natural and artificial features (energysupply lines, strategic buildings, roads, railways,etc.). The nature and the level of detail in the twodata bases are different, on the basis of thedifferent expected volcanic phenomena. The GIS havebeen planned: (a) for volcanic risk mitigation (hazard,value, vulnerability and risk map assessing), (b) toprovide suitable tools during an impending crisis, (c)to provide a basis for emergency plans.

    Notes: Summary In the carpal tunnel syndrome (CTS) sensory nerve conduction is more sensitive than motor conduction. However, 8%–25% of the sensory distal latencies in symptomatic hands may still be normal. A systematic study was made of the median, ulnar and radial orthodromic nerve conduction velocities (SNCV) stimulating each of the fingers separately. Four SNCVs from the median nerve, two SNCVs from the ulnar nerve and one from the radial nerve were obtained, and the ratio of the median to radial SNCV and the ratios of the median and ulnar SNCVs were estimated. The significance of these parameters in the diagnosis of the CTS was studied, and a rapid technique for the screening of nerve entrapment in the initial stages of the disease is proposed. Three hundred and seventy-five symptomatic hands were examined. Seventy-five hands showed normal distal latency, in which cases, however, the SNCV of the ring finger was always outside the normal range, while the SNCVs of the thumb, index and middle fingers were abnormal in 64%, 80% and 92% of cases respectively. The amplitudes of the sensory responses were the least sensitive of the parameters studied. Our results suggest that a study of the median nerve digital branch to the ring finger may be of value in providing an easily performed and rapid technique for screening an early median nerve entrapment at the wrist.

    Description / Table of Contents: Sommario Gli autori riportano 2 casi di nistagmo alternante non periodico, uno di origine vascolare (ischemia nel territorio dell'arteria vertebro-basilare) ed il secondo di origine traumatica (colpo di frusta) con segni otoneurologici tipici di lesione della fossa cranica posteriore ed in particolare del vestibulum-cerebellum e del tronco (presenza di gaze paretic nystagmus, nistagmo di rimbalzo, dismetria saccadica, iperreflessia vestibolare ed alterazione del test di soppressione visuale. In un caso è stato possibile somministrare baclofene che ha determinato un miglioramento della sintomatologia clinica e strumentale. La temporanea sospensione del farmaco è risultata in un peggioramento sia clinico che strumentale. È discusso il meccanismo di azione di tale farmaco.
    Notes: Abstract We report two cases of non periodic alternating nystagmus, one of vascular origin (ischemia of the vertebrobasilar territory) and a second of traumatic origin (whiplash injury) with otoneurological signs, typical of lesions in posterior cranial fossa and in particular of vestibulum-cerebellum and brainstem: gaze paretic nystagmus, rebound nystagmus, saccadic dysmetria, vestibular hyperreflexia and impaired visual suppression test. In one case it was possible to give baclofen therapy, which yielded positive results. Suspension of drug administration resulted in the worsening of clinical signs. The mechanism of action of the drug will be discussed.

    Description / Table of Contents: Sommario Sono stati studiati numerosi casi di polineuropatia dovuta ad esposizione professionale a solventi industriali in diversi calzaturifici della provincia di Siena. Lo screening di 654 lavoratori ha permesso di identificare 98 casi di polineurite, di cui 16 gravi, 45 di media gravità e 37 senza sintomi neurologici obiettivi ma con le caratteristiche alterazioni elettrodiagnostiche. La studio longitudinale di 53 pazienti ha dimostrato che la sintomatologia neurologica sia soggettiva che obiettiva cosi come le alterazioni elettrodiagnostiche continuano per anni in numerosi pazienti. Inoltre, dopo alcuni anni di osservazione, alcuni pazienti mostrano segni di interessamento del SNC come spasticità ed iperreflessia. In tutti i campioni di colle e di solventi utilizzati sia dai lavoratori a domicilio sia nelle fabbriche dove si sono verificati casi di polineurite, è stata trovata un'alta percentuale di esano commerciale.
    Notes: Abstract Cases of polyneuropathy due to exposure to industrial solvents have been studied at several shoe factories in the province of Siena. After the screening of 654 employees 98 verified cases were detected. Of these, 16 were rated as moderate to severe, 45 as mild, and 37 were minimally involved but with characteristic electrodiagnostic abnormalities. Follow-up study in 53 patients showed that neurological signs and symptoms as well as electrodiagnostic abnormalities continued for years in several patients. In addition, after a year's observation, some patients showed signs of central nervous system dysfunction such as spasticity of the lower limbs and increased deep tendon reflexes. High percentages of commercial n-hexane were found in all the samples of glues and solvents collected from home-workers and from factories where cases of polyneuropathy occurred.

    Description / Table of Contents: Sommario Si riporta lo studio clinico, neurofisiologico e liquorale in un caso di M.F. con interessamento del Sistema Nervoso. Nel liquor della paziente è stata identificata una proteina anomala del p.m. di 22.000, assente nel siero della paziente e nel liquor di soggetti di controllo, scomparsa dopo terapia immunosoppressiva endorachide. La natura di questa proteina è discussa alla luce delle ipotesi patogenetiche della malattia.
    Notes: Abstract We report the clinical, neurophysiological and CSF study in a case of mycosis fungoides with nervous system involvement. The CSF contained an abnormal protein of molecular weight 22000 that was not in the patient's serum or in the CSF of control subjects and that disappeared after intrathecal immunosuppressive therapy. The nature of this protein is discussed in the light of hypotheses regarding the pathogenesis of the disease.

    Description / Table of Contents: Sommario Viene riportato il caso di un uomo di 41 anni, con severo deficit bilaterale della dorsiflessione del piede, insorto due ore dopo un prolungato esercizio fisico, con assenza di dolore spontaneo e segni obiettivi di alterazioni delle sensibilità. Lo studio EMG dimostrava assenza di attività elettrica volontaria od evocata nei muscoli tibiale anteriore ed estensore breve della dita, mentre era presente una normale attività nei muscoli peroneo lungo e gastrocnemio bilateralmente. La velocità di conduzione motoria del nervo tibiale e sensitiva del nervo peroneo superficiale risultavano normali. Dopo una fasciotomia bilaterale eseguita a distanza di quattro ore dal ricovero, il paziente dimostrava un progressivo recupero dell'attività dei muscoli tibiali anteriori che risultava pressoché completo a distanza di quattordici ore dall'intervento. Sulla base dei dati elettrofisiologici noi discutiamo il possibile ruolo patogenetico di un blocco neuroaprassico del nervo peroneo profondo nel tratto in cui attraversa il setto fibulare anteriore. Infine viene formita una possibile interpretazione dell'assenza di dolore.
    Notes: Abstract We report the case of a 41 year old man who complained of a severe bilateral deficit of the anterior tibial compartment two hours after prolonged exercise. On admission there was no spontaneous or evoked pain, no objective sensory deficit but total loss of dorsiflexion of feet and toes. Electrophysiological investigation showed no voluntary or evoked electrical activity in tibial and extensor digitorum muscles; while peroneus longus and gastrocnemius muscles showed a nearnormal pattern. On the other hand a weak motor response was obtained by direct stimulation of the anterior tibial muscle belly. An early bilateral fasciotomy was followed by almost complete recovery of the spontaneous and evoked motor activities of the tibial anterior muscles. On the basis of these findings we discuss the possible pathogenetic role of a neuroapraxic block of the deep peroneal nerve where it crosses the anterior fibular septum and supply a possible interpretation of the absence of pain.

    Notes: The resonant mode field expansion technique presented in previous papers to analyse one- or two-port microstrip structures of different geometrical shapes is now applied to multiport radial lines. In particular, a radial-line n-way power divider is considered. This device allows, under certain hypotheses, an equiphase and equiamplitude splitting of an input signal. The Z matrix formulation has been derived and utilized to obtain the scattering parameters for the circuit. The numerical results are in good agreement with both experimental and theoretical data presented in the literature. The low number of resonant modes required to obtain satisfactory numerical convergence allows a substantial reduction of the computing time with respect to other more complicated approaches.
