    Notes: Summary In the oral exocrine pancreatic function test using fluorescein dilaurate, this synthetic substrate attaches primarily to the triglyceride surfaces of the neutral lipids administered as part of the breakfast: these fluorescein dilaurate molecules cannot be attacked by cholesterol esterase. In the course of triglyceride saponification by lipase and colipase, however, the fluorescein dilaurate is liberated and hydrolyzed by cholesterol esterase. The pancreatic function test, therefore, measures the lipolytic activities not merely of cholesterol esterase, but indirectly of lipase, as well.

    Notes: Summary Both hypercholesterolemia and hypertension are risk factors for atherosclerotic vascular disease, and elevated cholesterol levels occur more frequently than expected in patients with hypertension. Elevated levels of intermediate-density lipoproteins (IDL) and low-density lipoproteins (LDL) were shown to be atherogenic, and LDL, comprising the major cholesterol-carrying fraction in human plasma, are structurally related to lipoprotein (a) [Lp(a)], a further risk factor for atherosclerosis. In the present study we investigated 200 male employees (mean age 26±7 years) to determine whether the relationship of IDL and Lp(a) to systemic blood pressure is similar to the reported correlations between total and LDL cholesterol and systemic blood pressure. To this end blood pressure was measured several times in each individual, and lipids, lipoprotein-cholesterol, apolipoprotein B (apo B), and Lp(a) were determined in fasting serum. IDL cholesterol and apo B, the main protein component of IDL and LDL correlated with blood pressure. However, levels of Lp(a) correlated neither with systolic or diastolic blood pressure nor with lipoprotein cholesterol, body weight, or age. Although IDL and Lp(a) are considered lipoprotein risk factors for atherosclerosis, levels of Lp(a), unlike IDL, are not related to blood pressure, body weight, or age. Our data suggest different metabolic and pathophysiological mechanisms of the risk factors, IDL, LDL, and Lp(a).

    Notes: Summary The hormonal status of men with cirrhosis of the liver has been investigated in numerous studies. Little, however, is known about changes of sexual hormones in women afflicted by this disorder. In a study of 31 postmenopausal women (mean age 63±8 years) suffering from cirrhosis of various etiology (alcoholic,n=8; posthepatitic B,n=1; PBC,n=5; cryptogenetic,n=17) the blood levels of estradiol (E2), estrone (E1), androstenedione (A), testosterone (T) and basal and stimulated values of gonadotropins are reported and compared with the data obtained in an agematched control group (n=9). In cirrhosis a significant increase of the median E2 (28 vs 12 pg/ml,P〈0.01) was found, whereas the changes of the blood levels of E1 (88 vs 76 pg/ml), A (63 vs 111 ng/dl), and T (0.30 vs 0.15 ng/ml) did not attain statistical significance in comparison to controls. Within the study group, however, a significant positive correlation with the degree of decompensation of cirrhosis (Childscore A–C) was observed for the steroid hormones measured. Thus, in subgroup C the hormone levels are higher than physiologically expected for postmenopausal women. On the other hand the median FSH (32 vs 48 mU/ml,P〈0.05) is significantly lower in cirrhosis compared to controls with a trend to decreased values of LH. Very low levels of LH and FSH are found in decompensated cirrhosis. The decrease of LH and FSH can partly be explained by the rise of peripheral hormones (i.e. E2, E1, and in some cases T and A). The response of the gonadotropins to stimulation by 100 µg GnRh generally remains unimpaired. Only a few women with decompensated cirrhosis show a hormonal profile (low gonadotropins in spite of low sexual hormones, reduced stimulation after GnRh) that indicates a disturbance of hypophyseal regulation as a result of a central defect of possible hypothalamic origin.

    Notes: Summary We set up a laser nephelometric assay for the quantitation of serum amyloid A (SAA) in human plasma. Therefore monospecific antibodies were raised in sheep and used in parallel to measure SAA concentrations by nephelometry and also by radial immunodiffusion, an assay usually applied for determination of SAA. The nephelometric method is precise, simple and unlike radial immunodiffusion results are obtained within an hour. The antigen concentrations determined both by laser nephelometry and radial immunodiffusion correlated highly (r=0.98). As plasma SAA concentrations were reported to be a possible marker of kidney allograft rejection, the assay was applied to measure SAA concentrations in patients after kidney transplantation. Data were compared with clinical and other biochemical parameters. The period after kidney transplantation is reported for two cases, where SAA plasma concentrations were helpful in diagnosing allograft rejection. The rapid availability of the SAA plasma concentrations by nephelometry makes them a possible additional tool to decide quickly upon antirejection therapy.

    Description / Table of Contents: Zusammenfassung Bei 92 Graviden wurde eine signifikante Zunahme der Glyceride, des Gesamtcholesterins, des freien und Estercholesterins, der Phosphatide und der Gesamtlipide im Serum verzeichnet. Das Maximum der Serumlipid-erhöhung lag im 3. Trimester der Schwangerschaft. Der Cholesterin-Phosphatid-quotient lag durchweg unter I und zeigte eine auffallende Konstanz. Im Wochenbett konnte bereits wieder ein Abfall aller untersuchten Serumlipidfraktionen nachgewiesen werden. Mechanismen, die während der Schwangerschaft zu einer Erhöhung der Serumlipide führen können, werden diskutiert. Die physiologische Schwangerschaftshyperlipidämie darf nicht zu diagnostischen Fehlschlüssen führen.
    Notes: Summary A significant elevation has been found in the serum levels of total, free, and esterified cholesterols, phosphatides, glycerides, and total lipids in 92 pregnant women as compared to normal levels. Maximum levels were found in the third trimester. The cholesterol to phosphatide ratio was constant at a value less than one. The serum lipid fractions rapidly decreased in the postpartum period. Mechanisms responsible for the elevated serum lipid levels are discussed. This physiologic hyperlipemia of pregnancy should not be a source of diagnostic errors.
