    Description / Table of Contents: Summary The effects of varying loads of intraduodenal phenylalanine on pancreatic and gallbladder function were investigated in 32 healthy volunteers using a triple lumen perfusion system. L-phenylalanine absorption rates in the proximal jejunum were measured simultaneously. Intraduodenal L-phenylalanine produced a dose related increase in pancreatic secretion reaching the maximum at the concentration of 100 mM. Bilirubin outputs too increased significantly but did not parallel enzyme secretion. In contrast to the effect of L-phenylalanine, D-phenylalanine 50 mM did not stimulate pancreatic enzyme secretion and gallbladder emptying. The absorption rates of L-phenylalanine increased with rising luminal concentrations. There was a significant correlation between pancreatic secretion and L-phenylalanine absorption rates. This relationship suggests that the release of cholecystokinin, a hormone stimulating pancreatic and biliary function is dependant of the absorption of nutrients like amino acids.
    Notes: Zusammenfassung In der vorliegenden Studie wurde bei 32 Probanden mit einer intestinalen Perfusionstechnik die Resorption von L-Phenylalanin und die Wirkung verschiedener intraduodenaler L-Phenylalaninkonzentrationen auf die Pankreasenzymsekretion und Gallenblasenentleerung untersucht. L-Phenylalanin verursachte konzentrationsabhängig signifikante Steigerungen der Pankreasenzymsekretion, wobei das Maximum bei einer Konzentration von 100 mM erreicht wurde. Nach L-Phenylalaninperfusion kam es zur Gallenblasenentleerung, jedoch stieg der Bilirubinausstoß nicht in Abhängigkeit von der perfundierten Aminosäurekonzentration an. Im Gegensatz zu L-Phenylalanin verursachte D-Phenylalanin intraduodenal perfundiert keine Pankreasenzymsekretion und Gallenblasenentleerung. Die Resorptionsraten von L-Phenylalanin stiegen mit zunehmender intraluminaler Konzentration an. Zwischen Pankreasenzymausstoß und L-Phenylalaninresorptionsrate bestand eine signifikante Korrelation. Die Befunde zeigen, daß die Resorption von Nährstoffen wie z.B. von Aminosäuren die Freisetzung von Cholecystokinin und damit die Pankreassekretion und Gallenblasenentleerung steuert.

    Notes: Summary More than 7 years after the diagnosis and treatment of breast cancer (T1N1aM0), multiple nodular foci were observed in the liver of a 40-year-old woman at ultrasonographic examination. The lesions were confirmed by CT scan, but CT-guided liver biopsy revealed only non-specific alterations. At subsequent peritoneoscopic examination, bluish-brown foci were indeed visible on the liver surface, but guided liver biopsies again failed to corroborate the suspected metastases. Instead, histology showed mild portal fibrosis, moderate steatosis and siderosis of hepatocytes, as before. Only the intense red fluorescence of part of the biopsy material under Wood's light suggested the diagnosis of chronic hepatic porphyria or porphyria cutanea tarda, here presumably as a consequence of prolonged alcohol consumption. Subsequent porphyrin studies in urine, faeces and plasma yielded the typical constellation of latent porphyria cutanea tarda (chronic hepatic porphyria type C). The activity of erythrocyte uroporphyrinogen decarboxylase was normal, which argued against a genetic predisposition. After 1 year of strict alcohol abstinence and low-dose chloroquine treatment the “nodular foci” in the liver were no longer visible on ultrasonogram and CT scan; only proton-weighted NMR imaging (SE 1500/30) still showed ill-defined areas of higher signal intensity. The renal excretion of porphyrins had decreased considerably. The levels are now consistent with the diagnosis of subclinical chronic hepatic porphyria type A. Modern non-invasive imaging techniques are tremendously useful, but they have their pitfalls. Focal liver lesions may present serious diagnostic problems, especially when they are found in a patient with a history of carcinoma at an extrahepatic primary site. A rare example is described.

    Notes: Abstract  An asynchronous, dielectric micro-motor consisting of an Al-SiO2 rotor of 50 to 200 μm diameter was driven with 4 or 8 circularly arranged electrodes in water. The motor elements were fabricated by micromachining. Each electrode was designed to be an oscillatory circuit with a sharp resonance frequency in the upper kHz-range. The resonances at all electrode tips increase the voltage in the stator-rotor gap by a factor of up to 15. As a result, the dielectric rotor operates in a sharp frequency window at 5 Vpp excitation as if driven with more than 75 Vpp. With square wave pulses, the higher order Fourier-components induce several changes in rotor spin direction within two frequency decades. The micro-motor has been driven for hours without noticeable wear. It developed high starting torques and was useful for circular water pumping. The system has the advantage of very sensitive frequency dependence and the low drive voltages (below 10 V) can be produced by most electronic circuits.

    Notes: Abstract Objective Metabolic effects of different caloric regimens were investigated in nonsurgical, medical patients with multipleorgan failure (MOF). Design Seven total parenteral nutrition (TPN) regimens were administered, differing in amount (14, 28, and 56 kcal/kg per day, i.e., hypo-, iso-, and hypercaloric nutrition, respectively) and distribution [carbohydrates (COH), amino acids (AA), long-chain and mediumchain triglycerides (LCT/MCT)] of calories. Each regimen was administered over 12 h. Metabolism was monitored by energy expenditure (EE), body temperature (BT), protein breakdown (PB), and blood glucose and serum lactate levels. Measurements were started within 2 days of MOF onset. Setting The study was conducted in a medical intensive care unit.Patients: Twenty patients with MOF on mechanical ventilation (mean Apache II score $$\bar x$$ =26) were investigated. Measurements and results The mean values of the EE ( $$\bar x$$ =31 kcal/kg per day), BT ( $$\bar x$$ -38°C), PB ( $$\bar x$$ =1.5 g/kg per day), and lactate ( $$\bar x$$ =2.0 mmol/l) and glucose level ( $$\bar x$$ =222 mg/dl) parameters were elevated. EE, BT, and lactate and glucose levels were significantly lower under hypocaloric nutrition than during iso- and hypercaloric nutrition (p〈0.01). Differences in the metabilic effects of LCT and MCT were not significant. PB was significantly elevated under hypercaloric nutrition (p〈0.01). Protein balance was positive under hypercaloric nutrition, and negative under iso- and hypocaloric nutrition. Conclusions In nonsurgical, medical patients neither hypercaloric nor isocaloric nutritional support prevented protein catabolism; in contrast, they enhanced the metabolic burden measured by EE, thermogenesis, urea production rate, and glucose and lactate levels. A hypocaloric regimen is therefore recommended for these patients during the early phase of MOF.

    Notes: Abstract The role of the flow visualization in providing the necessary insight for the development of theoretical models of complex afterbody and base flows is described. Methods of calculating the turbulent base pressure for axisymmetric configurations are discussed. Emphasis is placed on the treatment of supersonic flows for cylindrical, boattail, and flare bodies as well as sharp and blunt cones and base flow nozzles. The guidance provided by flow visualization in the development of theoretical models for the transonic case is also discussed. The current interests in applying finite difference techniques to these-problems indicate that the extensive use of flow visualization data will continue.
