    Description / Table of Contents: Zusammenfassung Es wird über die Modifikation und Erweiterung des Cochlea-Implant-Systems von vier auf acht Kanäle berichtet. Es hat sich gezeigt, daß mit Hilfe der auf dem Promontorium verankerten Elektroden bei niedrigen Stromstärken Hörempfindungen stimuliert werden können. Die Höreindrücke sind differenzierbar und in der Lautstärke veränderlich. Die 8-Kanal-Hörprothese vermittelt den Patienten Informationen über Umweltgeräusche und verbessert das Sprachverständnis beim Lippenlesen. Eine Sprachdiskriminierung ist jedoch mit dem bisher erst 8-kanaligen System noch nicht möglich.
    Notes: Summary It is reported on the modification and extension of the cochlear implant system from 4 to 8 channels. It has been found that auditory sensations can be evoked at low current levels by means of the electrodes placed in the promontory. The auditory sensations induced can be differentiated and varied in intensity. The 8-channel hearing prosthesis provides the patient with information on environmental sounds and improves speech comprehension in lip reading. Adequate speech discrimination, however, is not yet possible with the to-date only 8-channel electrode system.

    Notes: Summary A correlative study of the anatomy and the radiological appearance of the intratemporal course of the facial nerve canal was carried out. Isolated temporal bones and temporal bones of cadaver heads were examined with thin-section high-resolution computed tomography in the axial, coronal and Stenvers' projections, then sectioned with a microtome and the radiologic and anatomic images were correlated. Appropriate projections for visualization of each segment of the facial nerve canal on CT were established. The high-resolution CT Stenvers' projection proved very useful for visualization of the geniculate ganglion fossa, as also of the tympanic and mastoid segments in their full length. The high-resolution CT appearance of lesions characteristic for each segment of the facial nerve canal are presented.

    Notes: Summary Our work on the subject of cochlear implant proceeds from the following considerations: 1. Although at the present stage of development cochlear implant cannot enable the patient to achieve speech discrimination, the single or multielectrode system is, however, still a great step forward. Perception of back ground noises enables the patient to lipread more easily, to feel safer in traffic, and provides him with a closer contact with the environment. He experiences himself as socially integrated once more. 2. As the development of cochlear implant has not yet been concluded, the patient should not be deprived of the help of an improved technique at a later stage through use now of a not yet perfected method. These considerations have led us to work out our operating methods according to the following guidelines: 1. In cases of deafness in both ears, the operation is carried out on the ear which shows the poorest results on the Electrical Promontory Test (House and Urban). 2. The operating technique is based on the exposure of parts of the coil on the medial wall of the tympanic cavity. 3. The membranous labyrinth is only exposed and not opened, and the electrodes are placed close to the membranous wall. Thus no destructions through manipulation occurs within the coil itself. We have examined the projections of the coil on the medial wall of the tympanic cavity in a hundred specimens of temporal bone, and we have determined landmarks for the purpose of locating the seperate turns of the coil. At the same time the thickness of the covering layer of bone in the area of the promontorium was examined. After this preliminary anatomical work we developed an operating technique which then enabled us to use the multielectrode system. The results of hearing improvement so far have been satisfactory.

    Notes: The arteries leaving the intermuscular fissures of the lower leg form six constant rows. Two further vascular groups emerge from the lateral and the medial heads of the gastrocnemius muscle. The vessels form a fine epifascial network, the radial and longitudinal meshes of which anastomose together. The epifascial network is complemented by a weaker, subfascial one. The arteries can be subdivided into cutaneous, musculocutaneous, and cutaneomuscular groups, corresponding to their supplying areas. A significant difference in vessel pattern could not be found between the left and the right lower leg of the same person. The different areas of supply are constant in the middle one-third of the lower leg, but vary considerably in the upper one-third and moderately in the lower one-third. The borders of the different areas show interlocking patterns.

    Notes: The relationships between lymph vessels and veins are of clinical importance for the prevention of injury to collectors and resulting edema after removal of veins. Injection preparations of the lymph vessels and veins showed that on the dorsum of the foot the collectors overcross the veins; however, in all remaining areas they undercross them. Only the great saphenous vein is overcrossed by lymph collectors. It also has accompanying lymph vessels but its vascular sheath, however, does not contain any lymph vessels. The position of the collectors depends on the thickness of the subcutaneous adipose tissue. On the dorsum of the foot the lymphatics are often closely bound up with the corium; on the lower leg, however, they are embedded in the adipose tissue. Some collectors are closely related to perforator veins. In the knee region the lymph bundle ascends dorsomedially to the medial condylus of the femur. On the thigh, where the subcutaneous fatty tissue is thick, the collectors form three layers. The superficial inguinal lymph nodes draining the leg and the external genitalia are situated around the saphenous opening and are closely related to the subinguinal venous star, so that in surgical intervention the collectors of this region are more exposed to danger than in other regions. © 1995 WiIey-Liss, Inc.
