    Notizen: Juvenile xanthogranuloma is commonly seen in the dermis, and only very rarely develops in the oral mucosa. Here we report a case that occurred in the anterior palate of a 9-year-old boy. The lesion appeared as a dark red and well-defined nodule measuring 12×14 mm. Histologically, it consisted of a proliferation of histiocytes and fibroblastic stroma intermingled with foamy cells. Many lipid droplets without limiting membrane were observed in the cytoplasm under electron microscopy, but no Langerhans' cell granules were observed. The proliferative histiocytes were positive for lysozyme and macrophage HAM56 under immunohistochemical observation, but not for S-100 protein. From these findings, the lesion was diagnosed as juvenile xanthogranuloma. The post-operative course, now amounting to 7 years, has been uneventful.

    Notizen: Abstract: Matrix metalloproteinase (MMP)-2 is expressed in osteoarthritic cartilage and synovial fluid and is thought to be involved in the degradation of cartilage extracellular matrix. However, MMP-2 expression and osteoarthritic changes in internal derangement of the temporomandibular joint are unknown. In the present study, we have examined the histological relationship between osteoarthritic changes on articular cartilage with or without articular disc perforation, and MMP-2 expression, in 85 mandibular condyles from cadavers. The expression and tissue immunolocalization of MMP-2 in fibrocartilages from these condyles was examined histochemically. The Mankin grade of histological criteria for specimens with disc perforation was significantly higher than that of specimens without perforation. MMP-2 immunostaining was positive in the cytoplasm of chondrocytes and in their surrounding matrix. There was a linear correlation between MMP-2-positive cell rates and Mankin grade. Our data suggest that MMP-2 plays an important role in fibrocartilage degradation in internal derangement of the temporomandibular joint.

    Notizen: Abstract: The purpose of this study was to determine whether or not matrix metalloproteinases (MMPs) in synovial fluid are helpful in the biochemical diagnosis of temporomandibular joint (TMJ) disorder (TMD). We examined the synovial fluid from 38 TMD patients with disc displacement and 20 volunteers by gelatin zymography and immunoblotting analysis to clarify the involvement of the joint pathology from the viewpoint of expression of MMPs. Two gelatinolytic enzymes, MMP-2 and -9, were detected in the samples. The incidences of expression, except for pro-MMP-2, in anterior disc displacement (ADD) without reduction (ADD w/o R) were significantly higher than in ADD with reduction (ADD w R) (P〈0.05). Quantitative analysis showed that the degree of MMP-2 and -9 expression in ADD w/o R were higher than in ADD w R. These data suggest that the presence or absence of disc reduction is a major turning point in the process of joint destruction, and these MMPs are useful as biochemical markers for TMD diagnosis.

    Notizen: Abstract We studied invasion-related adhesion events in vitro using three squamous carcinoma cell lines (HSC-3, poorly differentiated type; OSC-19, well-differentiated type; and KB cells, undifferentiated type). An in vitro invasion assay through matrigel in the transwell chamber revealed that HSC-3 cells were most invasive, OSC-19 cells moderately invasive and KB cells least invasive. Inhibition assay of invasion using synthetic peptides RGD, RGDV, RGDS, RGDT, IKVAV and YIGSR, showed that invasion of the three cell lines was significantly inhibited by RGDV. There were other peptides that inhibited invasion significantly including IKVAV for HSC-3, and RGDS and YIGSR for OSC-19. HSC-3 cells and OSC-19 cells adhered to fibronectin, laminin, vitronectin, and type IV collagen, and KB cells did not adhere to laminin but did to fibronectin, vitronectin and collagen type IV. Pretreatment of cells with RGDV peptide in the attachment assay reduced the ability of these cells to bind to vitronectin and fibronectin more efficiently than pretreatment with RGDS. Anti-αv antibodies inhibited adhesion of HSC-3, OSC-19 and KB cells to vitronectin, but anti-β1 antibodies did not inhibit adhesion. Immunofluorescent microscopic examinations showed that all cell lines were positive for anti-β5 and anti-αv antibodies, and only HSC-3 cells were positive for anti-β3 antibody. α5β1 was not clearly demonstrated in any of the cell lines. RGDV was the most effective inhibitor of squamous cell carcinoma invasion among the synthetic oligopeptides used in this experiment, and it is suggested that it affects αvβ3-and/or αvβ5-mediated carcinoma cell invasion.

    Notizen: Abstract The presence of occult metastasis is the most important factor that influences the prognosis in patients with head and neck cancer. To reproduce occult metastasis of oral cancer cells, we serially resected the primary focus in an orthotopic implantation model to examine when metastasis of cancer cells occurs. Human squamous cell carcinoma was implanted into the tongue of nude mice divided into two groups, non-surgery and surgery groups. Mice in the non-surgery group were sacrificed, and the tongue cancer and cervical lymph nodes were resected simultaneously. In the surgery-group, resection of the tongue cancer was performed, and the cervical lymph nodes were resected on day 28. For the non-surgery-group, the incidences of metastasis were 0%, 9%, 36%, 91% and 100% on days 3, 7, 14, 21 and 28, respectively. For the surgery-group, resection of the tongue cancer was performed on days 3, 7 and 14, and the incidence of metastasis on day 28 was 0%, 82% and 91%, respectively. The occult metastasis was reproduced using resected primary cancer on day 7. This time-based model may be useful to clarify the mechanism of metastasis and to develop new treatments.

    Notizen: Abstract Background A comparison of chemotherapy with PP (cisplatin, CDDP; and peplomycin, PEP) and TPP (4′-O-tetrahydropyranyladriamycin: THP-ADM, CDDP and PEP) in the treatment of oral cancer. Methods This study included 159 out of 199 patients who had visited the collaborating institutions' hospitals, and who had been registered in the Examination Meeting of Chemotherapy for Oral Cancer during the 2-year period from June 1990 to May 1992. Ninety-seven patients underwent PP therapy and 62 underwent TPP therapy. In PP intravenous infusion therapy, 60–80 mg of CDDP per m2 was administered on day 1, followed by infusion of 5–7.5 mg of PEP per m2 from day 2 until day 6. In the PP intra-arterial infusion therapy, in which the drugs were infused in a retrograde fashion into the superficial temporal artery, 50–70 mg of CDDP per m2 was infused on day 1, followed by infusion of 5 mg of PEP per m2 from day 2 until day 6. In both the intravenous and intra-arterial TPP therapy regimens, 20 mg of THP-ADM per m2 was infused on day 1, followed by infusion of 50 mg of CDDP per m2 on day 2, and infusion of 5 mg of PEP per m2 from day 3 until day 7. Results The response rate of TPP therapy was 64.5%, which was not significantly different from the 53.6% obtained with PP therapy. However, the complete response (CR) rate with TPP therapy was 22.6%, significantly higher than the 9.3% with PP therapy. The response rate of TPP intra-arterial infusion therapy was particularly high (92.3%), and significantly higher than that of PP therapy. The response rate of TPP intra-arterial infusion therapy was very high (100.0%) in stage I and II patients and in primary cancers of the tongue, gingiva, buccal mucosa, and hard palate. The toxicity of TPP and PP therapies resulted in a high incidence of nausea/vomiting and anorexia. Skin disorders and epilation were observed at high rates with TPP therapy, and incidences were particularly high (100.0%) in the TPP intra-arterial infusion therapy group. The incidence of leukopenia was high in patients treated with TPP therapy but was not severe. Conclusion The results of this study suggest that TPP therapy is more effective than PP therapy in the treatment of oral cavity cancer. Although TPP therapy was associated with skin abnormalities, alopecia, and leukopenia as side effects, there was no influence on subsequent treatment. TPP intra-arterial infusion therapy appears promising in the treatment of oral cancer.
