    Notes: Laparoscopic surgery has become popular in recent years, but few studies have addressed analgesia for this type of surgery. We conducted a prospective double-blind randomised trial on 36 cases of laparoscopic colorectal surgery to determine the influence of intrathecal morphine on postoperative pain relief. All patients received a subarachnoid block with local anaesthetic in addition to general anaesthesia. One group also received intrathecal morphine. A patient-controlled analgesic (PCA) device was prescribed for pain control postoperatively and the visual analogue score (VAS) was used for pain assessment. The group who received intrathecal morphine used significantly less morphine. There were no adverse cardiovascular effects of the combined anaesthetic technique. Nausea and vomiting remained the main side-effect of intrathecal morphine but this was easily treated with anti-emetics.

    Notes: Abstract. Gastric volvulus is an infrequently encountered clinical entity characterized by abnormal rotation of the stomach around an axis made by its two fixed portions, the cardia and pylorus. Surgical treatment by anterior gastropexy is usually indicated for chronic gastric volvulus to prevent complications and treat the underlying pathology. We report a patient with chronic gastric volvulus who was treated with laparoscopic anterior and diaphragmatic gastropexy. Laparoscopy not only identifies underlying predisposing conditions but also allows effective fixation for gastric volvulus. Laparoscopic approach minimizes the access trauma and is superior for benign upper gastrointestinal lesions.

    Notes: Abstract. Gastric polyps account for a small proportion of gastric neoplasms. Many of them presented with acute or chronic bleeding. Endoscopic polypectomy is the preferred treatment modality. However, endoscopic polypectomy is often incomplete and impossible for large polyps. Large and intramural polyps require laparotomy and excision. From June to October 1995, four patients with bleeding gastric polyps received laparoscopic resection in our unit. All were women, aged 40–78, with polyps 2–6 cm in diameter. One patient underwent emergency surgery for active bleeding. The others received elective operations. Operating time ranged from 90 to 120 min. There was no perioperative morbidity or mortality. Full diet was resumed by the 4th postoperative day. The average postoperative hospital stay was 5.3 days (4–7). Laparoscopic resection of gastric polyp is a preferable treatment option when endoscopic treatment fails.

    Notes: Abstract Background: Kidney entrapment is generally considered to be a difficult step in retroperitoneoscopic nephrectomy (RN). Normally, a large device is needed to accommodate the specimen, which may measure ≤14 × 10 cm. There is just too little space to maneuver either the entrapment device or the specimen. Herein we describe a technique that can overcome this difficulty and even turn the space limitation to advantage. Methods: After the nephrectomy, the specimen is pushed caudally. A 15-mm Endocatch is inserted via the posterior cranial port. It is positioned strategically underneath the specimen so that when the entrapment bag is released, the specimen will have no room to move (because of the limited space) and will simply fall into the bag (because of gravity). Results: We have used this technique for our last four cases of retroperitoneoscopic nephectomy and found that it remarkably shortened the operative time for this step. Conclusion: One of the criticisms leveled against retroperitoneoscopic surgery concerns the problems posed by the space limitation. This technique turns the space limitation into an advantage and guarantees successful specimen entrapment.

    Notes: Abstract Chemical analysis of the eggs spawned by fiveSinularia soft corals (Octocorallia, Alcyoniidae) collected in the central region of the Great Barrier Reef, Australia, revealed the presence of the known cembranoid diterpenes epoxypukalide and/or pukalide in all cases. Examination of the colonies from which the eggs were released one month before spawning and also some time after spawning failed to detect either diterpene in the coral tissue. The three dimensional structures of pukalide and epoxypukalide, as determined by the single-crystal X-ray technique, are reported for the first time. The wide distribution of pukalide and epoxypukalide and of related compounds within the octocorallia is discussed.
