    Notes: The superconducting oxide Ba2YCu3O7−δ is known to exist in two crystallographic structures, being orthorhombic or, at higher temperature, tetragonal. Thermogravimetric experiments have been performed to study their transition at different oxygen partial pressures. Weight loss curves, recorded by heating samples at various constant rates, each exhibit two inflection points. In our interpretation, such points delimit the regime within which the two structural phases coexist. Limiting values of temperature at these points, calculated by extrapolation to zero heating rates, and the corresponding magnitudes of oxygen nonstoichiometry are presented. Such data have basic significance as well as relevance to the thermal processing of materials.

    Notes: Objective To assess whether transcervical intrauterine instillation of local anaesthetic agent reduces pain during diagnostic outpatient hysteroscopy and endometrial biopsy.Design Prospective, randomised, double blind, placebo-controlled trial.Setting Outpatient hysteroscopy clinic in a university teaching hospital.Population Ninety women undergoing outpatient diagnostic hysteroscopy with or without endometrial biopsy.Methods Transcervical intrauterine instillation of 5 mL of 2% lignocaine into the uterine cavity before performing the procedure.Main outcome measures Evaluation of pain at different stages of the procedure using a visual analogue scale and changes in blood pressure and heart rate.Results The use of local anaesthetic did not alleviate pain experienced during hysteroscopy and endometrial biopsy. It did not prevent the occurrence of vaso-vagal reactions; however the incidence of these was low.Conclusions Transcervical instillation of local anaesthesia neither reduced pain nor prevented vasovagal reaction during hysteroscopy and endometrial biopsy.

    Notes: The structure of high angle boundaries in the (BiPb)2Sr2Ca2Cu3Ox system, which is compositional and structurally modulated along the boundaries, was investigated by high resolution electron microscopy. These compositional variable boundaries tend to facet onto the (001) plane, leaving steps of height equal c/2 for the (BiPb)2Sr2CuOx and (BiPb)2Sr2Ca1Cu2Ox phases with amorphous material at the corners of these steps. These data suggest a weak link is formed at high angle boundaries where there are such lower Tc or insulating phases. © 1995 American Institute of Physics.

    Notes: Artificial bulk "zero-angle" boundaries parallel to the c axis have been engineered between large melt-processed YBa2Cu3O7−δ (YBCO) grains and observed to carry a transport supercurrent at fields up to at least 5 T at 77 K. The temperature and angular dependencies of the boundary resistance have exactly the same form as those of the grains, which is evidence that the grains are intimately coupled. The limiting mechanism for current transfer across these boundaries is, therefore, not a simple weak link or Josephson effect. This joining technique is extremely promising for production of macroscopic engineering artifacts. © 1998 American Institute of Physics.

    Notes: Large, single grain NdBa2Cu3O7−δ (Nd123)–Nd4Ba2Cu2O10 (Nd422) composites have been fabricated up to 2 cm in diameter in a controlled 1% O2 in N2 atmosphere using a top-seeded melt growth technique. The irreversibility field as a function of temperature has been obtained from magnetic hysteresis loops for magnetic fields applied parallel and perpendicular to the c axis of small specimens ((approximate)2×2×3 mm3) cut from individual grains. These specimens are observed to exhibit a pronounced peak effect for current flowing in the a–b plane and a five-fold anisotropy in the irreversibility line over a wide temperature range for fields applied along the major crystallographic axes. An exceptionally high irreversibility field (〉12 T at 89 K) is observed in this material for field applied perpendicular to the c axis, which extrapolates to well over 40 T at 77 K. © 1999 American Institute of Physics.

    Notes: Objective To evaluate the efficacy and safety of paracervical anaesthesia in reducing pain during outpatient hysteroscopy and endometrial biopsy.Design Prospective, randomised, placebo-controlled, double-blind study.Population One hundred women undergoing outpatient hysteroscopy and endometrial biopsy for abnormal uterine bleeding.Interventions Paracervical block using 10 mL of either 2% lignocaine or normal saline before the procedure.Main outcome measures Evaluation of pain at different stages of hysteroscopy using a visual analogue scale together with blood pressure and heart rate monitoring.Results Compared with placebo, paracervical anaesthesia significantly reduced the pain only at the time of insertion of the hysteroscope, but not at the subsequent stages of the procedure. However, paracervical injection of lignocaine resulted in a higher incidence of bradycardia and hypotension.Conclusions Paracervical anaesthesia not only fails to reduce pain during outpatient hysteroscopy and endometrial biopsy, but also carries a risk of inducing bradycardia and hypotension, which is probably a result of inadvertent intravascular injection.

    Notes: We conducted a randomised, controlled study to investigate the effect of adding a background infusion to patient-controlled epidural analgesia for postoperative pain relief. Forty-two patients scheduled for elective lower abdominal gynaecological surgery received patient-controlled epidural analgesia postoperatively using a mixture of 0.2% ropivacaine and 2.0 μg.ml−1 fentanyl. Patients in group B (n = 20) were given a background infusion of 5 ml.h−1, whereas those in group N (n = 21) were not. There was no difference in pain scores or patient satisfaction scores between the two groups. Patients in group B had a higher total drug consumption (156.8 ± 34.8 ml vs. 89.5 ± 41.0 ml; p 〈 0.0001) and incidence of side-effects (71.4% vs. 30.0%; p = 0.007). Motor blockade during the 24-h study period was also greater in group B (median [range] area under the curve 7.5 [0.0–39.0] h vs. 3.0 [0.0–36.0] h; p = 0.035). We conclude that the addition of a background infusion to patient-controlled epidural anaesthesia is not recommended as it confers no additional benefits.

    Notes: Pachydermodactyly refers to a rare form of digital fibromatosis involving the proximal portions of the fingers. There are only nine cases reported in the literature, ill idiopathic and occurring in young men. We report a 5-year-old Chinese boy with tuberous sclerosis who presented with localized pachydermodactyly since birth. This congenital form of pachydermodactyly may represent an additional cutaneous sign of tuberous sclerosis.

    Notes: Forty-two neurotic children and thirty children with behaviour disorder were followed up from 1 to 4 years after they attended for psychiatric advice. The mean follow-up period for the former group was 2·7 years and for latter 2·9 years. At follow-up 78·6 per cent of the neurotic children but only 53·3 per cent of the group with behaviour disorder were symptom-free or significantly improved.
