    Notes: Summary The treatment of scleromyxoedema is notoriously difficult. We present a patient with long-standing diffuse scleromyxoedema associated with functional impairment who developed chronic idiopathic neutropenia complicated by recurrent life-threatening infections. Treatment with recombinant granulocyte-colony stimulating factor led to normalization of the neutrophil count, prevented further systemic infections, and unexpectedly was associated with a striking clinical improvement of her skin disorder and decrease in mucin deposition in the dermis.

    Notes: Twenty patients with bronchial asthma, on long-term oral therapy with theophylline, demonstrated an increased number of suppressor T-cells and impaired graft vs host reaction. Ten asthmatics on other therapy, not including theophylline, as well as ten normal healthy controls, failed to show similar findings. Elimination of suppressor T-cells corrected the above immunological abnormalities in the theophylline-treated patients, while addition of serum from theophylline-treated asthmatic patients to lymphocytes from normal healthy controls, affected the graft vs host reaction of these lymphocytes. We conclude that theophylline induces quantitative as well as qualitative immunological alterations by increasing the number and activity of suppressor T-cells, which most probably secrete a serum factor, responsible for some of the abnormalities observed.

    Notes: Subgingival bacteria exist within a biofilm consisting of cells and extracellular matrix which may afford organisms protection from both antibiotics and components of the host immune system. MIC values for planktonic Porphyromonas gingivalis treated with metronidazole were compared with those obtained for the same strain in biofilms associated with hydroxyapatite (HA) surfaces. The treated biofilms were examined for growth and studied by scanning electron microscopy. A broth assay resulted in an MIC of 0.125 μg/ml for metronidazole against P. gingivalis. P. gingivalis biofilms exhibited growth after treatment with 20μg/ml metronidazole, which was 160 times the MIC for planktonic organisms. The results of this study indicate that biofilm-associated P. gingivalis may be resistant to metronidazole at concentrations which are usually attained by systemic administration.

    Description / Table of Contents: Résumé On a procédé chaque année, pendant une période de 9 ans 1/2 à un examen catamnestique d'un groupe de 449 malades psychiatriques chroniques qui, en 1959, étaient hospitalisés dans un hôpital de l'Etat de New York. La plupart des pertes étaient causées par le décès. 11% des hommes et 8% des femmes, seulement, étaient sortis en 1969. Davantage de patients étaient sortis pour de courtes périodes, pendant les 9 ans 1/2, mais avaient été réadmis à l'hôpital. En 1969, le résultat a été mis en relation avec l'âge en 1959, le diagnostic psychiatrique, la durée d'hospitalisation antérieure à 1959, l'état civil et le lieu de la naissance. Parmi ces variables, l'âge et le diagnostic psychiatrique sont celles qui avaient le plus d'influence. 90% de ceux qui souffraient d'artériosclérose cérébrale ou de psychose sénile étaient morts au moment de l'examen de 1969. Avec le temps, depuis 1959, une proportion toujours plus grande des survivants du groupe était des schizophrènes, pratiquement tous à l'hôpital. On a conclu que ce groupe constituait un noyeau de patients à long séjour en hôpital qui avaient relativement peu de chances de sortie.
    Abstract: Zusammenfassung Über eine Zeitspanne von 9 1/2 Jahren hinweg wurde in jährlichen Intervallen bei einer Gruppe von 449 chronischen psychiatrischen Patienten in einem staatlichen New Yorker Krankenhaus der Befund erhoben. Dabei war die Ausfallquote am häufigsten durch Todesfälle verursacht. 1969 waren nur 11% Männer und 8% Frauen entlassen. Während der 9 1/2 Jahre waren mehr Patienten kurzzeitig entlassen, und wieder in die Klinik aufgenommen worden. 1969 stand der Befund in Beziehung zum Alter 1959, zur psychiatrischen Diagnose, zur Klinikaufenthaltsdauer vor 1959, zum Familienstand und zum Geburtsort. Unter diesen waren Alter und psychiatrische Diagnose am einflußreichsten. 90% von jenen, die an cerebralarteriosklerotischen oder senilen Psychosen litten, waren bei der Nachuntersuchung 1969 verstorben. Im Laufe der Zeit von 1959 an war unter den am Leben Gebliebenen der Untersuchungsgruppe ein zunehmend größerer Anteil von Schizophrenen, von denen fast alle in der Klinik waren. Daraus wurde gefolgert, daß diese Untersuchungsgruppe den harten Kern einer langfristigen hospitalisierten Gruppe bildete, der relativ geringe Entlassungschancen hatte.
    Notes: Summary The outcome of a cohort of 449 chronic mental hospital patients resident in a New York State Hospital in 1959 was determined at yearly intervals over a period of nine and a half years. The commonest means of attrition was death. Only 11% males and 8% females were on discharge status in 1969. More patients had been discharged for short periods during the nine and a half years, but readmitted to hospital. Outcome in 1969 was related to age in 1959, psychiatric diagnosis, time spent in hospital prior to 1959, marital status and place of birth. Of these, age and psychiatric diagnosis were most influential. Ninety per cent of those suffering from cerebroarteriosclerotic or senile psychosis were dead at follow-up in 1969. With the passage of time since 1959, a progressively greater proportion of those in the cohort who were still alive were schizophrenics, nearly all of whom were in hospital. It was concluded that this cohort was a hard core long-stay hospitalized group who had relatively little chance of discharge.

    Notes: Summary A patient with malignant lymphoma showed severe hypophosphatemia during a blastic crisix. x-Ray microprobe analysis of the cells at this stage revealed a high phosphorus content. Following treatment and a decline in the white cell count the serum phosphorus increased, whereas the cellular phosphorus decreased. The potential prognostic value of serum and cellular phosphorus determinations during the course of malignant disorders of the hematopoietic system is discussed.

    Notes: Abstract Biofilm formation on metallic surfaces in marine and freshwater environments often precedes corrosion and other biofouling conditions. Attachment is mediated by such environmental factors as the presence of surface conditioning films, fluid dynamics, bulk-phase nutrient levels, and surface chemistry. In this study, we utilized a Fowler Cell Adhesion Measurement Module to demonstrate that the changes in cellular concentration and composition of a monoculture ofPseudomonas atlantica biofilms on stainless steel were a function of the applied shear force. At shear forces in the range of 3–10 dynes cm−2 (1.0 liter min−1), attachment as measured by acridine orange direct microscopic counts was greatest at the higher shear forces.14C-Acetate uptake activity on the stainless steel surfaces increased with shear stress. Acetate incorporation ranged from 1×10−5 to 19×10−5 μmol cm−2 between 0.15 and 30 dynes cm−2 for 30 min uptake periods. On a per cell basis, however, activity decreased with shear, indicating a shift in metabolism. Fourier transform infrared spectroscopy revealed that protein and carbohydrate concentrations also increased with the applied shear. Increased biofilm C∶N ratios and total fatty acids were associated with the higher shear stresses. Neither radius of interaction nor biofilm age appeared to significantly influence the relationship between fluid shear and attachment and cellular composition ofP. atlantica biofilms in the range of 1–10 dynes cm−2.
