    Thoracic fracture-dislocations without spinal cord injury: a case report and literature review (1995)

    Notes: Summary Severe fracture-dislocation of the thoracic spine without neurological deficit is rare. Both translational and rotational deformity of the midthoracic spine makes transection of the cord almost inevitable due to the confined dimensions of the cord and spinal canal. Even though associated fractures of posterior elements are frequently seen, they seldom result in neural sparing. The case of a 24-year-old man who sustained a severe rotational fracture-dislocation of T9/T10 with considerable anterolateral displacement is reported. Due to a fractured left pedicle and a right-sided vertical fracture through the posterior aspect of the vertebral body, alignment of the posterior elements in the spinal canal was maintained and there was no neurological deficit. The patient was operatively treated with posterior segmental instrumentation, and was completely asymptomatic at follow-up 5 years later.

    Description / Table of Contents: During a prospective clinical trial with 17 German clinics participating, we examined the indications and treatment methods of 646 distal radial fractures. Criteria for assessment of primary treatment results were collected and integrated in a clinical score.
    Notes: Im Rahmen einer multizentrischen, prospektiven, klinischen Studie aus 17 deutschen Kliniken wurde bei 646 dokumentierten distalen Radiusfrakturen des Erwachsenen eine dem Frakturtyp entsprechende Behandlungsindikation bzw. ein passendes Therapieverfahren gewählt. Der Erfolg der primären Behandlung wurde ausgewertet, indem ein praxisbezogener Score erstellt wurde.

    Description / Table of Contents: Zusammenfassung Von 1975-1979 wurden von 93 Oberarmschaftfrakturen 79 im Hanging-cast und 14 operativ behandelt. OP-Indikationen waren zweit- und drittgradig offene Brüche, Gefäß- und/oder Nervenläsionen, Serien- oder doppelseitige Frakturen, Pflegeerleichterung bei Greisen und Polytraumatisierten. Von 79 konservativ Versorgten wurden 12 wegen verzögerter Bruchheilung nach 8 Wochen sekundär operiert. Von 67 im Hanging-cast Behandelten (Querfrakturen inklusive) wiesen nach 1–5 Jahren 62 (93 %) ein sehr gutes und gutes sowie 5 (7 %) ein schlechtes Ergebnis auf.
    Notes: Summary Between 1975 and 1979 93 upper arm fractures were treated: 79 by hanging cast, 14 surgically. The indications for operation were second- or third-degree compound fractures, vessel or nerve lesions, serial fractures in both arms and for easier nursing of the aged and polytraumatised. Of 79 conservatively treated, 12 required secondary surgery after 8 weeks due to prolonged fracture healing. Of 67 fractures 62 (93 %) treated solely by hanging cast showed very good and good results 1–5 years after the accident, including transverse fractures, and only 5 (7 %) poor results.

    Description / Table of Contents: Zusammenfassung Osteosynthesematerial ist bei instabilen pertrochanteren Frakturen greiser Patienten unter Sofortbelastung hohen varisierenden Biegebeanspruchungen ausgesetzt. Mechanisches Versagen der Osteosynthese ist daher häufig (in einem Drittel der Fälle unseres Krankengutes). Die Fehleranalyse zeigt, daß schädliche Biegekräfte nur durch statisch-biomechanisch sinnvolle Reposition mit überkorrigierender Valgisation und Medialisation gemindert werden. Anatomisch exakte Reposition und stabile Fixation allein garantieren keine Sofortbelastbarkeit.
    Notes: Summary In case of unstable intertrochanteric femoral fractures of elderly men osteosynthetic material is exposed to high bending forces during locomotion. Mechanical failure is quite frequent. The analysis of our ] cases with failure of internal fixation revealed that harmful bending forces are dimished only by suitable reduction with valgus fixation and medial displacement according to static-biomechanical principles. Immediate stability however is not achieved by internal fixation of the fragments in an anatomical position.

    Description / Table of Contents: Zusammenfassung Frakturen mit schwerem Weichteilschaden und gefährdeter Fragmentdurchblutung sind Indikationen zur Primärstabilisierung mit einem Fixateur externe. Angestrebt werden muß eine einwandfreie Fragmentstellung und -abstützung. Wenn notwendig, erfolgt offene Brucheinrichtung unter Zuhilfenahme von Zugschrauben und kurzen Platten. Die Fixierung approximativer Repositionsergebnisse begünstigt eine verzögerte Bruchheilung. Dies ergab die Analyse von 21 am Unfalltag mit einem dreidimensionalen AO-Rohrspanner versorgten Frakturen mit zusätzlicher Minimalosteosynthese in 10 Fällen.
    Notes: Summary Fractures especially those accompanied with severe soft-part damage and injured fragment perfusion are indications for the use of the “fixateur externe”. Primarily a correct reposition and an optimal interfragmentary support should be realized. If necessary open reposition and additional minimal osteosynthesis by screws and short plates are carried out. Fragment fixation with approximate reposition only favours the occurrence of delayed bone repair. These results were obtained from 21 fractures primarily stabilized by the three-dimensional AO fixateur including 10 with additional minimal internal fixation.

Die Ursache scheint in einer verzögerten Bruchheilung infolge Implantation des Knochenzementes in den Frakturbereich zu liegen. Die Indikation zur Verbundosteosynthese bleibt beschränkt auf pathologische Frakturen and auf Patienten in schlechtem Allgemeinzustand zur Verbesserung ihrer Pflegefähigkeit.
    Notes: Summary Clinical experience with 44 applications of compound osteosynthesis, of which 41 were fractures of the femur, showed that in 11 cases fracture-specific complications were found e.g. breakage of the implants, pseudarthrosis etc. The reason for this seems to be delayed healing of the fracture due to the implantation of bone cement in the region of the fracture. The indication for compound osteosynthesis is limited to fractures destroyed by malignant tumors and to patients in bad general condition in order to improve their nursing care.

    Description / Table of Contents: Zusammenfassung Bei einem 15jährigen Mädchen lag nach einer Coffeyschen Operation, welche wegen einer Blasenekstrophie andernorts durchgeführt worden war, bei willkürlichem Sphineterschluß eine völlige Stuhl- und Harninkontinenz vor. Unter der Vorstellung, daß der für die Kontinenz so wichtige M. puborectalis durch die klaffende Symphyse zu weit auseinanderlaufende Ansatzpunkte haben könnte und dadurch in seiner Funktion beeinträchtigt sei, wurde als therapeutischer Versuch eine beiderseitige parasacrale Beckenosteotomie mit Verschmälerung der Symphyse zur Verlaufskorrektur des Musculus puborectalis durchgeführt. Das Mädchen war danach vollkommen kontinent.
    Notes: Summary After Coffrey's operation in another hospital to correct a bladder extrophy, a 15-year-old girl showed total incontinence of stool and urine despite voluntary sphincter function. It seemed that the musculus puborectalis, which is essential for continence, had too widely separated points of insertion owing to a wide symphysis and was thereby restricted in its function. A bilateral parasacral pelvic osteotomy was carried out to reduce the width of the symphysis, constituting functional treatment of the musculus puborectalis. The patient was quite continent after this operation.

    Description / Table of Contents: Zusammenfassung Der Fixateur externe ist zur Stabilisierung von Frakturen der oberen Extremität nur selten erforderlich. Wir stellten die Indikation bei Schußbrüchen, Verletzungen des Ellbogengelenks mit ausgedehnten Knochen- und Weichteildefekten, infizierten Osteosynthesen und zur Pflegeerleichterung bettlägeriger Patienten. Einfache Technik, eine geringe Komplikationsrate, unbehinderte Sekundäreingriffe und die Möglichkeit der frühfunktionellen Behandlung sind die Vorteile dieser Osteosyntheseform bei Problemfrakturen am Arm.
    Notes: Summary External fixation is rarely used in the treatment of fractures of the upper extremity, but is indicated in missile fractures, elbow-joint injuries with extensive bone and soft-tissue defects, infected osteosyntheses and also to facilitate intensive care. The advantages of this method are its simple technique and low rate of complications. Further operations are not obstructed and functional treatment can be started immediately.
