    Notes: Arteriovenous malformation (AVM) is usually congenital, but an acquired type is also known, of which most are due to a previous injury. We report six patients with acquired digital AVM, all having a small patch of AVM localized to the tip of one finger, and therefore quite different from ordinary AVM which consists of a large pulsatile mass. Histologically, dilated venous and arterial vessels were present in the dermis. Tumour-like growth was absent; thus, these six cases could be differentiated from arteriovenous haemangioma. Because of the characteristic anatomical site, unique clinical manifestations and histology, this appears to be a new and distinctive entity.

    Notes: The bone dynamics under denture base has not been clarified sufficiently. The purpose of this study was to evaluate histomorphometrically the bone dynamics in denture supporting tissue with relation to the intensity of the masticatory pressure. The removable denture bases, which exerted four different regulated masticatory pressures (0·0, 1·0, 20·0 or 40·0 kPa), were inserted to the hard palate of the molar region of 22-week-old rats. The palatal tissues were excised from 1 to 12 weeks after denture insertion at intervals of 1 week and embedded in methyl-methacrylate. No bone resorption was observed throughout the experimental period in the 0·0 and 1·0 kPa groups. In the 1·0 kPa group compared with the 0·0 kPa group, bone formation parameters except for bone formation rate significantly increased at 4 weeks, but there was no significant difference between these groups in the other experimental periods. In the 20·0 and 40·0 kPa groups, bone resorption was observed until 3 and 5 weeks, respectively, and the amounts of bone resorption (AoBR) were 45 ± 25 and 104 ± 21 μm, respectively. After bone resorption, bone formation parameters in these two groups showed transiently significant decrease compared with the 0·0 kPa group, but were the same time course as the 0·0 kPa group following this decrease. In conclusion, masticatory pressure caused different time courses of the bone dynamics in denture supporting tissue depending on the initial intensity of the pressure. The AoBR responded to the initial intensity of masticatory pressure. Bone formation following bone resorption did not cause equivalent recovery of the bone surface level to the level observed in the case without bone resorption in the present study.

    Notes: Summary We present 57-year-old man in whom ectopic extramammary Paget's disease (EMPD) affected the upper abdomen. Although the clinical appearance was suggestive of Bowen's disease or superficial basal cell epithelioma (BCE), the biopsy specimen showed EMPD histologically. Only 12 cases of ectopic EMPD have been reported (including this case). In our 20 year experience of 129 EMPD, this is the first ectopic case. Thus, the frequency of ectopic EMPD is 0–78% (one of 129) in our study. The male/female ratio in the reported 12 cases is 2 : 1. nearly the same as EMPD in general (2:1:1, in our 129 cases). The mean age of the 12 patients is 65.8 years, which is not significantly different from ordinary EMPD (66–;4 years, in our 129 cases). Comparing ectopic EMPD to ordinary EMPD, clinically and histologically. we could find no difference. As they appear to be the same disease, ectopic and ordinary EMPD may share similar origins and mechanisms of occurrence. We support the hypothesis that Paget's cells originate from the remaining pluripotential germinative cells which are able to differentiate into many kinds of secreting glands.

    Notes: The presence and distribution of carcinoembryonic antigen (CEA) and lectin-binding sites in benign apocrine sweat gland tumours was investigated using immunoperoxidase and immunofluorescent techinques respectively. CEA was present in apocrine hidrocystoma, hidradenoma papilliferum, and syringocystadenoma papilliferum. The distribution pattern of CEA was indistinguishable in each of the three types of tumour. The distribution of lectin-binding sites was similar in all cases of a given type of neoplasm, and the staining patterns were similar but not ideitical in hidradenoma papilliferum and syringocystadenoma papilliferum. However, the distribution of lectin-binding sites in apocrine hidrocystoma was different. These results could be useful in elucidating the nature of the cells of origin of benign apocrine sweat gland tumours and providing a basis for their classification.

    Notes: We report a case with rapidly growing subcutaneous and intra-muscular tumours that were identical histopathologically to a pre-existing left atrial myxoma. No invasion of visceral organs or skeleton was found. This is a very rare case of metastasizing myxoma with wide dissemination and rapid metastastic growth, despite the benign appearance of the original tumour.

    Notes: Lectin-binding sites in sections of formalin-fixed, paraffin-embedded eccrine sweat gland tumours were investigated using fluorescein isothiocyanate (FITC) conjugated peanut agglutinin (PNA), FITC conjugated Ricinus communis 1 agglutinin (RCA-1) and FITC conjugated wheat germ agglutinin (WGA). In 22 benign eccrine sweat gland tumours, lectin-binding sites were noted primarily on the cell surface, and in the secretions. In five malignant sweat gland tumours, all showed cytoplasmic lectin-binding sites in variable proportions of malignant cells in addition to cell surface staining. These results indicate that cytoplasmic lectin-binding sites may be a useful marker of neoplastic transformation of eccrine sweat gland tumours.

    Notes: Summary Acquired digital arteriovenous malformation is an abnormal connection between the arteriole and venule in the finger, fed by the digital vessels. Lesions are characterized by small, slightly raised dark-red macules on the distal part of the fingers. In the past, similar cases had been described using the terms ‘cutaneous keratotic haemangioma’ and ‘periungual and subungual arteriovenous tumours’. In this article, we would like to discuss three additional cases that were studied using epiluminescence microscopy. Pathological findings and the possible pathogenesis are also presented.

    Notes: The presence and distribution of lectin-binding sites on neoplastic cells of Paget's disease was studied using fluorescein isothiocyanate (FITC)-conjugated peanut agglutinin (PNA), and FITC-conjugated wheatgerm agglutinin (WGA), and compared with such lectin-binding sites on keratinocytes, and cells of eccrine glands, apocrine glands, and mammary glands. Neoplastic cells of both mammary and extramammary Paget's disease showed cytoplasmic staining with both lectins. There were however fewer stained cells in mammary Paget's disease than in extramammary Paget's disease. The cytoplasmic staining of lectin-binding sites in cells of apocrine glands was in sharp contrast to the cell-surface staining seen on keratinocytes, or cells of eccrine glands or mammary glands. These results indicate that the lectin-binding sites of neoplastic cells of Paget's disease more closely resemble those of cells of apocrine glands than of keratinocytes, cells of eccrine glands or cells of mammary glands.

    Notes: We report 10 patients with hereditary benign telangiectasia (HBT), in whom the age ranged from 1 to 19 years (mean 7·5). The male/female ratio was 1 : 2·3 (3 : 7). Four patients (40%) had congenital lesions. Image analysis with Doppler echogram, angiography and thermography revealed an arterial component in the lesions, consistent with arteriovenous malformation (AVM). This is the first report of HBT in association with AVM.
